    At 55, my knee collapsed after decades of intensive sport (soccer, badminton, table tennis, alpine skiing with a season ticket, jogging up to the marathon). Diagnosis of severe knee osteoarthritis. I had constant attacks of pain for 4 years, even at night. Three orthopedic surgeons said the same thing, try swimming and cycling, get injections. After exercise it was always better for a few hours. Various knee injections only helped for a few weeks. Then I saw your videos with the reference to muscle imbalance. My thighs were very strong, my glutes were weak. So I followed your advice and rebuilt my muscles in the gym. I have now been pain-free for 9 months and am looking forward to the ski season. I am more than grateful to you. Warm greetings from Germany.

    When I brought this up to my PT he told me that no, you need to strengthen the quads not the glutes. So I will be doing your glute exercises at home on my own!

    I have been in knee pain since June & visited the physiotherapist consistently. It gives me temporary relief. I saw your video the other week & started watching your other videos & at the same time activated my glutes when walking. I'd say that you really helped me tremendously.     I started watching your videos a couple years ago. I went from hardly walking to little pain. I recently tripped and thought I broke something so I went to an ortho clinic. Nothing broken but I saw two different doctors and both were amazed I was even able to walk. One told me that most of the patients they have scheduled for knee replacements do not have as much damage as I have. I walk up and down stairs several times a day, and the day before I fell I walked 10,000 steps. You have a lot to do with this!! Thank you! You literally are a life saver.

      Nancy are you OK? Try foot raising. That’s keeping your heels down and raising your toes while standing holding onto a chair. Clench your toes, on and off, at the same time. This is a great exercise to help stabilize your walking,posture and balance. I was dragging my left foot just enough to sometimes catch on a crack in the sidewalk. This exercise helped me improve that imbalance.

      @@NuNugirl I will definitely try that! Fortunately I had some bad bruises from my fall. But it was an eye opener. I want to avoid knee surgery if I can. Learning about muscle imbalances and exercise I feel have put me on the right track.

    Over 15 years, from a relative young age, I have knee pain. Went to several physical therapists, they did not help me even if they were aware of my issue. Now it has come so far I have arthritis. Very thankful for your videos, they help me very much, life changing!

      I am HORRIFIED of knee surgery. Diagnosed with ARTHROSIS and bone on bone. I'll diet and exercise to death rather than have surgery

      @@edith2511 I focus on strengthening my glutes and it goes better. It’s very important to perform them well (I naturally try to avoid using these muscles), in front of a mirror. Still ups and downs, but I’m hopeful. Good luck!

      @@edith2511my knees still hurt, some days quite a lot, most days much less. At the moment focused on Pilates and lifting weights. Tennis did not do me well this summer. How are you doing in your journey?

    I totally agree. I’ve had stenosis and had an operation. As a result, some of my leg muscles were weakened which affected my right knee. I followed your exercises to keep my hips level, develop my quads and lower thigh and I stand on my toes a few times a day. Easy exercises. From crippling pain at night and in morning, I am almost 100% pain free with no painkillers.

    When I got prp and stem cells the dr said not to do anything except take collagen. It was 4000. and didn’t work and hurt more as there was zero support for my joint. I repeatedly asked if there’s anything else I should do. The pain drove me to do a crazy amount of research. 4000. and a year later, I get what I have to do and this video validates all the dynamics I have figured out. Thank you for this detailed multi factorial explanation. You are awesome!!!

    This makes so much sense! I’ve been doing the glute squeezes for a few weeks and am starting foot exercises. I have less pain… some stiffness. I can walk further comfortably. I started taking turmeric curcumin. All in time! I’m so thankful for Dr D!!!

    I am completely blown away. I am a very healthy 59-year-old. I am the personal trainer by trade and a health coach and I’m embarrassed to say that I was one of those people that trained the quads over and over again and I also have very high arches so I had incredible imbalance. They recently diagnosed me with Osteo arthritis. I was devastated! I never follow routine and I knew I need to look outside the box to find healing and I found it. Seven days into following El Paso Therphy procedures relief came and I’m feeling normal again. I am so happy. I am strengthening my glutes every single day, also something the doctor did not tell me I was wearing a knee brace on my right knee for comfort the whole time the pressure of the knee brace made it worse.because of the pressure put on my kneecap nonstop, but the doctor didn’t say a word about that.     Performing leg extensions and squats have declined the knee pain by 90%. After these exercises I can run 12 km effortlessly. Then the pain appears again lightly. That's the moment I start walking to finish off my running routine. Thanks a million for sharing your findings.

    I was told in 2008 by a surgeon who just did arthroscopic surgery to remove the debris from a torn meniscus that I need a knee replacement! I’m trying to avoid this knee replacement. I’m totally on board with this type of physiotherapy for arthritis in my knees! I’ve exercised all my life but nobody helped me when I torn the meniscus in my left knee! I’m going to start this program because the regular run of the mill physiotherapy that give me handouts that are the same exercises I was given in 2008! This program makes sense to me in 2024!

    Thank you for your dedication. I have benefitted so much from your YT videos. As a handicapped person on a fixed income, I cannot afford your knee courses but would appreciate reading an illustrated book or implenting your knee exercises displayed on DVD. Would either of those be available or in the offering?
    Also, would it be possible or doable to make your courses available through medicare or insurance coverage?

    My knee pain is only from a torn meniscus. I have bone-on-bone arthritis but have no pain from that. I have excellent range of motion. The surgeons refuse to fix the meniscus unless I have a total knee replacement. So I walk carefully, concentrating on foot positioning to avoid torque that causes the torn meniscus pain and hope I can stay active without the replacement.

    I was on crutches a year ago two knee surgeons said I needed knee replacement from two sets of MRI's and X-rays. To avoid the surgery I have heavy focus on glutes (also hamstrings) and now avoid any quad work. Im 6,1", 215 lbs - I also do yoga, eat as healthy as possible, minimise booze. I don't take any meds. Im not really comfortable to go jogging yet but Im off road hiking, cycling and swimming just fine (a bit tender some days) - but no surgery :-).

    This makes soooo much sense. Almost 1500 patients is a significant data base, and 90% opted out of knee replacement surgery because of your muscle strengthening program. I'm in!!     I thoroughly enjoy your videos. I don't have any knee issues at 65 but I do not want any so I still do your exercises to prevent it from ever happening. I believe your exercises prevents knee issues even to those who don't have any knee issues. I share your videos with my family. I have osteoporosis and have severe foot issues and arthrosis in my feet so this is extremely helpful to me. Thank you for giving this information and taking your time to help others.     Dr. David Middaugh is so right! I have been following his advice for glutes and feet exercises for a year now and it made my knees much better (osteoarthritis in both knees, age 60). But I am strengthening my abs now too and it is changing for the better still. I have a problem though that nobody was able to address; I have a big ganglion cyst on the front of my right knee and that is hurting. Doctor refuses to drain it and wants me to have a knee replacement ;-) Nobody has helped me as much as this channel did anyway! I recommend it!

    For perspective: Tony Hawk is 56 years old and walks with a cane. I will soon be 66. I worked hard for over a year so I can walk again without feeling overwhelmed and uncomfortable. I will never jog again let alone run and skating is out of the question. Good luck everyone,keep going and never give up.

    True, glutes are vital. Tyfs. I first learned from John Gibbons. A Dr of therapy wrote a book about it. I was researching for my left shoulder aches. Not just knees. I was shocked when I found out he had a patient with the same condition I had, all due to glutes not firing properly. Thank you for this video.

    I have medial knee arthritis in both knees and wear offloading braces for support. A few weeks ago, I made the mistake of jogging in the park for 10-15 minutes without my braces. The next morning, I woke up with severe pain in both hips and knees, as well as a burning sensation at the top of my glutes, just below the belt line. Now, I’m experiencing hip osteoarthritis symptoms, primarily in my left hip, and I'm awaiting the results of an X-ray.
    One thing I’ve noticed is that my left foot is often rotated outward, almost 45 degrees, when I stand. This has made me wonder if there's an alignment issue or if my femur is rotated, which could be contributing to my medial knee arthritis.
    My question is: how can I identify the root cause of my medial knee arthritis so I can address it properly? I've already started strengthening my glute muscles, but if anyone knows of other exercises or videos that target muscles related to inner knee arthritis, I’d appreciate any recommendations.
    It would be incredibly helpful if Dr David could create a video specifically about medial knee osteoarthritis, its root causes, and ways to fix it.

    I've been working with your hip arthritis program but I also have some knee issues so I'm watching this video. I'm interested in what you say about strengthening your feet muscles. My feet have always tipped outward rather than inward (supination?) with wear on the outsides of my shoes. Do you have a video that deals with that issue and how it affects the knees and hips?
    Also...is backwards walking actually good for knees?

    This man has saved my 93 year old mom from hip pain at night. Glute squeezes totally eliminated the pain. I thank the good Lord that I found this wonderful man. I had the same problem and the simple glute exercise totally relieves the pain. If I stop doing the exercise then the pain comes back.

    Along with my chiropractor treatments, I was told not to exercise, they gave me a rebuilder, that totally helped, then they added exercises one at a time, glutes and core were stressed to me . This all takes time, and I am healing!
    It is possible!
    I love my tailgate swings too, and I add weight. Just let it hang swing small opposite. Never lift that weight however.
    But this helps in cartilage regeneration

      The tailgate swings are super helpful, as you say, hang weights on the feet to pull the joint apart When swinging. It can also help to slowly pull your knees up to your buttocks 100 times every morning in bed while lying and carefully push them back again. This also nourishes the knee joint without weight loading. If you do this in the water, it's even better because of the weightlessness.

    Thanks doc. Been following you and Your explanation makes total sense . I stopped my Physical therapy and started following your standing glute workout for the last week . I’m told I have knee OA and currently my knees are swollen. Can you pls advise how often I should do the glute exercises (from your standing glute exercise video)

    very good video, I think you got me pegged. I'm told I need two replacements that I dont want, but my low back slides around on right side cause cartilage is gone or shot and no fix for that from car accident. dr said he cant put a cage in my lumbar til I get both knees done cause I have to be able to walk to rehab. I think you are on to something with muscle imbalances. makes a lot of sense ,worth trying anyway. I've two therapist, both made me worst so I could not walk. quads and calfs cramping as well. thank you for making this honest video.

    I was doing a golf swing and I felt it in my hip and down my knee then down my lower outer leg and I just tried a few of your exercises and it felt way way better but I do believe my glutes may have been the problem I’ve had this pain for at least a good month until I came across your video. Any other advice would be great.

    I have osteoarthritis and shredded meniscus and torn ACL. Have it in both knees. Left had total knee replacement in April. Right knee is worse.

    So would this help with a torn meniscus? I have swelling and pain in my right inner knee under my kneecap. After I had an ACL replacement 1 1/2 years ago I had a lot of pain so the Dr did another MRI and discovered my torn meniscus.

    I wish I could make a trip to El Paso. I just had my third set of injections. Both of my knee caps are to the outside, and I'm not sure what else is wrong. I think you had a video about strengthening the inner theigh to bring the knee caps back in. I'm all for avoiding knee replacement. 🤞

    What about hips? Is it the same principle? 70 yrs old and scheduled for 2nd hip replacement Oct 30. Surgeon told me the replacement will outlive me. Get it now because waiting will make recovery harder.🚶‍♀️

    i am obese i have lost some weight like 30, but I never had knee problems until I injuried my leg got caught in a door and pulled it and it took a year to heal and I walked weirdly because it hurt so much but i am doing better doing some of your exercises, but I still have that little spot on the side of my knee that hurts when I walk, I know it is because I dont have arches, too after watching this, and my abdomen is very strong from all that throat clearing I do (I have stomach issues) I am working on my diet, doing a ketogenic diet, cant keep it perfectly but I stick 90 percent of the time each week. and that spot doesnt always hurt too some times I can hike for hour or more without pain. it is bothering me today, I have been doing some glute exercises but not alot, I Hike 2 hours one day a couple of weeks it was doing very well no pain, but I had shoes with arch support. these shoes have no, I wear bare foot shoes when I ride my bike because regular shoes snag on the arms of the crank since I ride a semi recumbent. bike.

    So what glute excersises should we do? I can't kneeling my knees. Both my knees are bone in bone.

      This video DOES make us VERY curious about strengthening the glutes as it is mentioned so many times. Dr M does have YT videos on this as do other PTs. You just need to search for them. 😊
