I don't remember if before or just after defeating the soldiers with Meryl disguised, but it implies the vent glitch to go to the torture room without having the Lv. 6 card. In fact, after escaping the cell once we do the glitch, Johnny will be completely uncensored 😅

Johnny in MGS3 is indeed the grandfather of Johnny from MGS1, 2 and 4, yes. MGS3 Johnny's son was called Johnny too. So at least 3 generations were called Johnny and we get to see the grandfather and the grandson. We never see the father/son.
Crazy theory, but are these deceased camera angles not from angles used in cutscenes? Essentially, this would mean that the gameplay is just reusing cutscene camera framing for immersive and added effect.
