I took a welding class once and the instructor had explained what typically happens to oxygen cylinders that have their integrity compromised. If they fall over and the valve stem breaks off the cylinder will become a "rocket" that will bust through a wall or if the walls of room are too solid to penetrate the cylinder will ricochet and bounce around the room with devastating effects to anyone/anything in that room!

MacGyver used this as a get away method, knocked the valve off a gas bottle to turn it into a rocket and bust through a wall! It was brilliant! They should have mentioned that!

I've seen this happen with an acetylene tank. There looked to be flame out the nozzle as it shot forward. Just missed one of the new guys working for us before blowing through wall. It stopped about 250-300' away in the field behind the shop. None of the dairy cows grazing in the field were hurt either. Lucky all round, really. It could have just exploded. Woulda killed or injured everyone in the shop if it had. Lotta questions got asked after that.

There was a scrapped MythBusters episode relating to an acetylene tank. The myth was that one was accidentally left on in a van all night, just a small amount, and in the morning, when the van doors were unlocked with the remote key, the whole van exploded.
They tried to replicate the myth by filling a van with acetylene and unlocking it remotely. Nobody was expecting it to work, and they figured they would inevitably have to just blow it up through some other means.
But the van was instantly obliterated by a violent explosion the moment they unlocked it. Leaving them with not enough footage to use in a regular episode.

Seen one stored away and marked as "empty" fall and take damage to its valve, flew INTO a cinder block wall. Had it been full it would have gone all the way through.

Seen that happen at a job I had. I heard the tank fall and I looked up just in time to see it skidding across the warehouse and blast through some boxes of freight and damage a wall. Left about a 16" hole in the wall. Pretty scary. Glad it moved away from me.

My late uncle saw a compressed gas cylinder break through the wall of the repair shop he worked at. He said he had never been this scared in his life.

Links to CAT appear to be broken after the first level link.
As for R Diaz's wanting real videos, are you prepared to pay the cost of production to produce such a video? They are not inexpensive.

Maybe make a better design for something so dangerous?

Get onto it then. This is the best design so far, short of not having anything at ever manufactured out of metal ever again, that is.

The best solution if possible is to change over to Bulk Storage tanks for Oxygen, Nitrogen and C02. Those are low pressure and a lot safer to work around. You don’t have to worry about handling high pressure cylinders. Unfortunately every facility doesn’t have the space for Bulk Storage Tanks.
