    A full thickness tear of the supraspinatus tendon can affect blood supply. I vote for surgery as I've been dealing with mine for 2 years now. I waited. Did PT and injections. PT made the tear worse. How do tears mend themselves? They DON'T.

      They CAN heal. There are thousands of articles and histology books - you can check that for yourself. However, you ARE right, some partial tendon tears don't heal with conservative treatment - as I stated on the video.

      I mean no disrespect at all, but my surgeon , who innovated the scope for rotator cuff surgery , also surgeon for the Boston Celtics, told me full thickness tears do not heal on their own. He has been in practice since 1979.

    I was desperate looking for more information about full-thickness tear of the supraspinatus tendon but God knows how I found Diogo Trevisan PDT TH-cam channel and I got the chance to visit him all the way to Florida from Philadelphia to get the right therapy because I wanted to try first anything than get the surgery. My arm is strong now, I never get the surgery. Thank you so much Diogo Trevisan God blessed you and your family!!!!

    Your video has shed a good bit of light to my issue. Last July I came down with a partial supraspinatus tear, which tue shoulder specialist had dismissed as tendonitis though I suspected it was something more serious given the shoulders asymmetry. He tried to reassure me by saying he'd have me undergo an ultrasound so I'd be calm about it. I was right and he was wrong. I got turned down the physical therapist on grounds that I was too strong for the sessions to work and referred me to a physiotherapist bodybuilder who performs a painful 1-hour long muscle release session once week.

    This volume on this video is so low and Diogo's voice so soft I can't make out all that he is saying. I think he's really got a lot of good information to offer and wish I could hear it better. I am a full volume on my computer...anyway you can turn up the volume at your end Diogo?

    They don't treat it as and emergency. They take month to get u in for appointment so by the time u get mri and it's full tear u wait 2months to get in to a Ortho. Then it's too late to fix it

    Rehabbed it with partial tear. Was never really fully stable with lots of work. finally had subsequent full thickness tear of supraspinatus and infraspinatus getting surgery. rather not let those stabilizing muscles atrophy and hope for the best.

    Full thicknesss tear is noted invovling the distal supraspinatus tendon at its insertion
    measuring approximately 1 cm in maximum anteroposterior dimensions. There is retraction
    of fibres for approx. 1.3 cm from greater tuberosity. There is mild fluid distension of the
    overlying subacromial-subdeltoid bursa. Dr suggest me for arthroscopic surgery what are your views on this

    Thanks. Thank you very much. Just i found trough mri that my sholder tendents full tear of thickmussel.
    I have sport injury due to Taekwondo 17 years a go. I suffer in pain for 2 or three months. Then the pain go slowly. After that i continue active even i go to gym TKD sports, until i represent state few times in national tounaments.
    After i 45 i slow down my sports...
    Then now reach 50 i start active my exercise. Now i feel bad pain in my shoulder back. Then i do a mri i found that my thickmussel fully tear.
    I still can rotate my arm 360° and move fast but the only problem when carry weight in gym i get back very havy pain.
    My doc sugest to jep steroid first. If still the pain there he sugest to do a surgery...
    Now when i hear your advice i get relif...i have a hope to recover by naturaly. But it will take time...in mean while i must not strain my arm....

    I have this injury probably for about 4 months I've been experiencing pain however I have still been able to overhead press weights as high as 240 pounds since I still have strength is it possible to recover without surgery while continuing to train lifts such as the bench press and over head press?

    One doctor told me it was a partial tare and another doctor is telling me that it's a full tare. First doctor said that it was 6mm which isn't even a centimeter. So what do I do,now?

      I'm sorry to hear about your shoulder. It's impossible to give consultation through youtube. You should get checked by an orthopedic surgeon and/or a licensed physical therapist.

    Thank you so much for posting this video. I had an MRI and it indicates that I have a full-thickness tear of the supraspinatus tendon which measures approximately 1 cm in transverse dimension by 2 cm in AP dimension. Surgery was recommended but I have full mobility. This happened about six months ago while doing pushups. Since then, the pain has dissipated. Seeing that I have full mobility and no pain would surgery still be wise? What could be the repercussion be if I choose not to have surgery? I would truly appreciate your input.

      I have the same issues as you. Spoken to many specialist and alternative medical experts. I also spoke to people who had surgery for the same condition. I was advised not to have surgery if I hv full mobility and no pain. Like you, I am also concerned about the repercussion of not having surgery. Really in a dilemma

    Thankyou for posting this knowledgeable video. Well I have a partial thickness intratendinous tear of the supraspinatus tendon at 1 CM from its insertion.The tear measures 3.4mm on sagittal and 3.1 mm on coronal images of MRI.The muscle belly shows no sign of fatty atrophy. I am amateur calisthenics athlete. I perform handstands and handstand push-ups.Sir , what would you recommend?? Conservative treatment or a surgery. Please do comment. I can perform all basic activities, but overhead activities do cause discomfort after a while . Adduction movement’s do cause discomfort. Empty can test also causes discomfort. The X-ray & MRI revealed that my AC joint is fine . The acromiun is type 1 and also I do not have impingement syndrome. Sir eagerly waiting for your reply.

      Abhishek Kamat Hey Abhishek! To be accurate on whether or not conservative treatment can help you I’d have to assess you myself. However, I would highly recommend you to seek a physical therapist that has experience in orthopedics and that does manual therapy. You can always try conservative treatment first and see how it goes. Partial thickness tears can heal OR evolve to a full thickness tear. I wouldn’t be able to tell you. But like I said, try to find a physical therapist with experience and try conservative treatment 😉. I’m sorry my answer was not precise, I hope you understand why I can’t really give you an answer just based on the MRI findings.

      Diogo Trevisan Thankyou Sir , But as a brotherly advice what would you still recommend? Can intratendinous tears be operated upon? I’m into second month of rehab/ recovery.. I mean I’m on conservative treatment.Will I be able to return back to at least 70% of my erstwhile strength by incorporating conservative treatment? Realistically how much time does a tendon take to heal fully?I know sir I’m pestering you with these questions but I just want to know your opinion. Thanks and regards.

      Abhishek Kamat intratendinous tears can heal or evolve to full thickness tears. Honestly, no one can tell which way it’s going to go. You are doing the right thing doing rehab first though.

    What's the difference between a full thickness tear and a rupture?

      A full thickness tear means the whole thickness has been torn on some portion - either where it attaches or where it rubs on the bursa, A complete tear or rupture is where it is no longer attached to anything it's just loose and flopping, over time it will retract and at that point nothing can be done to fix it, it's like it dries up. The other 3 muscles do the load lifting to compensate.

      I would recommend you speak to your doctor! As you probably saw in the video, tendons CAN heal without surgery, the outcomes of post surgical and non-surgical treatment can vary from case to case.
      Just based on the information you gave me I wouldn’t be able to tell you, but keep in mind that it is possible to recover function without surgery. In some cases the patient may lose strength and develop some instability in the shoulder. I’d recommend you speak to your physician and your physical therapist so they can assess you 😉
