In this rehab video I go through form beginner to advance rehab exercises to isolate and strengthen the subscapularis following a intrasubstance tendon tear. Usually, these ones if they are small do not need surgery but it's imperative that the rehab / strengthening work is done to help with pain, further degeneration or deterioration and weakness.

Wow. Good stuff. My pain is just above mid humerus, near base of delts, especially with lateral rotation. I've been guessing it was referred pain from the infraspinatous. Now I'm thinking it might be subscap. No mri.
Doctor thank you for the video, I have the following: Mild tendinosis changes of the rotator cuff tendons, particularly for the subscapularis with
a small tear at the bursal fibers of the myotendinous junction of the supraspinatus.
Doubt on the presence of a small tear at the glenoid labrum at 2 o'clock.

I have a 3 mm tear in superior subscapular tendon. What is a realistic timeline to rehab following your direction?
I re injured it a few days ago i had a tear a few years ago and it feels like i did it again. Should i rest a few weeks before doing this?

When can I begin these exercises? I partially tore my bicep and subscapular tendon 6 days ago?
I get real pain doing the exercise at 11mins. Is that exercise specifically working the subscapularus or is there another muscle/tendon likely to be the culprit? Thanks!

Maybe a sign of instability but if it doesn’t hurt then slowly work on it!
Got the same partial thickness injury with an 18mm delaminated component. Rehab is a very slow process.

@PipeRhymes nope it has not! Physio told me the tear itself never heals. Focus is on thickening what’s left and making it stronger.
but you have any improvement with physio? how long to you see the improvements?
@PipeRhymesnope. Tendons have very poor blood flow. Tears don’t ever heal the way muscle or tissue does. All you can do is try and build up / thicken the intact portion of the remaining tendon. Going to give it 6 more months of rehab, before thinking about need for surgery.
@@Fanfafifabeen told it could take up to 12 months before you see any major improvement. Very expensive when you factor in all the physio sessions and lost productivity over a year. I’m constantly working on it with exercises etc. Still very unstable though.
The first exercise is pain full, but I can tolerate it, I should do it anyway ? The second excercise doesn’t pain to much.
The pain should not really be there with rehab. Either reduce the load or range of movement
I might have this because of bench pressing in the gym, should i stop doing that for a while?
If it hurts it yes. Best to get it diagnosed

I have a low grade parial thickness tear, how long should I rest before starting these excercises, I mean will the tendong heal if subjected to tension!!?
There is a rest and healing time but you also need to prevent it from weakening and developing a painful tendinopathy. If in doubt contact your physio or surgeon

The logic of physiotherapy : tendon torn so you make it better by doing exercises that pull on the tendon.
To the extent it "works" how does it compare to doing nothing and just letting it heal?
The load is below what would make it tear more. Tendon tears become more tenopathic on the tendon and the pain becomes worse. If the tear is too large that exercise makes it worse then surgery is considered
