It's interesting tho because if he really thinks that he heard something-which he seems to-then Realmuto's reaction is further evidence that he was correct, meaning he shouldn't overturn it. In the end I think he got the call wrong but 1. I could be wrong, and 2. From his perspective he had multiple pieces of evidence that he was correct so why would he review it

It is hard to tell, but Realmuto's reaction tells it all. There is nobody in the world that is a good enough actor that they would react THAT instantly by sheer fakery. He winced RIGHT as the ball passed by his elbow. Unless he was PREPARED to fake it before ht pitch, it just wouldn't have been that quick.
That was the face of someone that got hit...luckily not that hard, but you make a face just out of surprise. Think of what most people do when they band their shoulder going around a corner, or they open a door into their leg. Not hard. But they still say "Ow" or something like that. That is basically the face Realmuto made.

Actions influencing the ump is a very important part of the game, how many walks or strikeouts have been called cause of how the player reacted instead of what actually happened, the game moves so quick at points that the players reaction is the only thing umps have to go on. With that in mind look at the guys face in replay he looks like he got hit, could a player think that far in advance to perfectly time making that face to pretend to get hit by the ball?

Start the video at 1:19 and stutter the play/pause really fast to try to get it frame by frame. I don't follow either team and don't have a horse in this race. It looks like it grazes him from that angle and his facial expression looks a bit like pain to me. The top down angle doesn't seem to have a high enough frame rate to capture what happens imo.
However the ump probably did call ball then change his mind based on what the player said. He put himself in an awkward spot by doing so. First he yields to the player, then yields to the manager? You can call this one ego driven, and maybe it is, but he also could be trying to save face too. It still might be worth looking at again though, idk what the rules are, if the ump calls it a ball initially, is that the defacto ruling and the other team would need to challenge? Or does he have the right to change his mind after he makes the call?

I get what you're saying. But athletes have been known to bend the rules since the beginning of sports. How many times do batters lean in and say they didn't move? Too many. I can't remember who they were playing, but in STL a few weeks back a batter said he was hit. Watch the replay and the ball MAYBE hit the lace on his shin guard that was 3" long. If umps start listening to players for this, you have to listen when they say they beat out a ball, made a catch instead of trapped it, etc. Players are always gaming the system.

It's not a planned reaction, but the grimace could just be from expecting to be hit by the ball because it was scarily inside either way. A hitter knows their timing and knows when to brace themselves when a pitch might hit them. I certainly wouldn't say a grimace or any player reaction is foolproof evidence. Either sound or trajectory change is proof.
