  • When discussing chronic disease and crucial organs, we don’t always think about the role the liver plays in our body. Yet, the liver has a role in every metabolic process in the body, including ensuring our blood sugar levels are stable or ensuring that amino acids are converted into fuel.
    Today on The Dhru Purohit Podcast, Dhru sits down with Kristin Kirkpatrick & Dr. Ibrahim Hanouneh to shed light on an epidemic affecting one in four people: non-alcoholic fatty liver disease. Dr. Hanouneh and Kristin share the biggest risk factors that lead to fatty liver disease including, metabolic dysfunction, diabetes, and insulin resistance. Dr. Hanouneh and Kristin discuss the rising prevalence of non-alcoholic fatty liver disease, emphasizing major risk factors such as metabolic dysfunction, diabetes, and insulin resistance.     Dr. Ibrahim Hanouneh, MD, is a gastroenterologist and hepatologist specializing in liver disease. He has served as Chief Gastroenterology Fellow and associate staff at the Cleveland Clinic and later contributed his expertise to Minnesota Gastroenterology and Mayo Clinic in Minnesota.
    Kristin Kirkpatrick, MS, RDN, is a celebrated author, speaker, and accomplished dietitian with over 25 years in the field. She is currently serving as a dietitian at the Cleveland Clinic Department of Wellness & Preventive Medicine. Teaming up with Dr. Ibrahim Hanouneh, Kristin co-authored the bestselling book Skinny Liver in 2017, and their upcoming release is called Regenerative Health.

      I've cut out obvious sugar like cakes etc, but it seems to be in everything. What do recommend to eat please?

      I saw this on Istagram - 1/2 oz. olive oil, 1/2 oz. lemon, pinch of cayenne pepper. I tried it for 2 weeks. My liver enzymes had been 5 times the NL limit. When I retested they had come down to Normal levels. Good luck, see if it helps you.

    A lot of it is conditioning…i fasted for a week or two several times…when i did it I was shocked that you can actually function and not lose energy and feel better. I realized that ‘hungry’ was not connected to need. Breakfast is the most important meal of the day - a joke. U need carbs - another joke. Essential grains - a joke. Go overseas and u will see healthy people doing it all kinds of ways

      I've never been a breakfast eater because I find I have much better energy all morning long without it. Eating breakfast makes me sluggish at first, then ravenous as the lunch hour approaches. I experience neither of those things when I fast all morning.

    Very alarming! A low carb diet and fasting are the best things for your liver. TMG and taurine will also help a great deal! Some benefits of doing occasional extended fasting: High blood pressure is lowered to normal levels very quickly while fasting. Fibrosis/scarring is reversed over time, including in the heart and lungs.
    Fasting stimulates phagocytosis, the ingestion plaques, growths and pathogens by the immune system. This will also remove spikes quicker, whether natural or unnatural in origin!
    Blood clotting is lessened.
    Blood sugar and insulin are lowered when fasting, reducing inflammation and allowing the immune bodies to move freely through the body.
    Vitamin D plasma levels are increased as fasting improves metabolic health, and vitamin D in turn increases autophagy.
    Reflexes and short term memory are increased.
    Your body recycles up to 1/3 of all immune bodies in a 72h fast, rejuvenating your entire immune system. This helps prevent the onset of new autoimmune conditions, which develop through a leaky gut and damaged immune system.
    Fasts from 36-96 h increase metabolic rate due to norepinephrine release!
    Fasting increases anti-aging Yamanaka factors and increases average telomere length in stem cell pools.
    Fasting can help with MS, Depression, BPD, Autism and seizures.
    Fasting restores your circadian rhythm to normal over time.
    When you move out of MTOR your body shuts down the building blocks of the cell required for viruses to replicate.
    The hunger hormone ghrelin also lowers with extended fasting and rises from dieting.
    What breaks a fast? Anything with protein or carbohydrates in it will break a fast but most teas and herbs are OK. Supplements and meds often break ketosis directly or contain a filler that will. Many meds are dangerous to take while fasting.
    Does fasting lower testosterone? No, it raises it when the fast is broken by increasing lutenizing hormone. Fasting also increases insulin sensitivity, which helps with muscle building.
    Fasting sactivates autophagy (literally self eating). This will cause cells to recycle damaged proteins and foreign matter such as viruses.
    Lowering insulin via fasting virtually eliminates chronic inflammation in the body.
    Weight loss from daily caloric restriction has 1/4 to 1/3 of the weight lost as lean tissue while many studies show fat loss from 36 h fasts without losing any lean tissue!
    The obese will lose loose skin while fasting, but the frail will have increased growth hormone release, which helps to make more lean tissue and reduce frailness.
    Fasts of 36-96 will not affect short term female fertility or affect menstrual cycle.
    Fasting reduces pain and anxiety by stimulating the endocannabinoid system, just like the effect of CBD oil
    Just 24h of fasting can cut your leptin levels in half! This reduces leptin resistance, which impairs immune function.
    Stomach acid is reduced over time while fasting and can allow for the healing of treatment resistant ulcers. Some patients may need continued acid reduction medication while fasting.
    Your brain also prefers to burn ketones at a rate of around 2.5 to 1 when they are available in equal quantity to glucose. Except for brief periods of very intense exercise, your body mainly burns fats in the form of free fatty acids.
    It increases mitochondrial function and repairs mitichondrial DNA, leading to improved ATP production and oxygen efficiency. Increased mitochondrial function also has the added benefit of increasing your metabolism, fighting infection and cancer prevention!
    Fasting releases BDNF and NGF in the blood which stimulates new nerve and brain cell growth. This can help a great deal with diseases like MS, peripheral neuropathy and Alzheimers.
    When not in ketosis, the brain can only burn carbohydrate, which produces a great deal of damaging ROS the brain has to deal with.
    Fasting also increases telomere length, negating some of the effects of aging at a cellular level.
    When you fast, this stimulates apoptosis in senescent or genetically damaged cells, destroying them. Senescent cells are responsible for many of the effects of aging and are a root cause of the development of cancer.
    A fasting mimicking diet for 3-5 days in a row also provides many of the same benefits as water fasting. FMD usually has 200-800 calories, under 18 g of protein and extremely low carbs.
    Exogenous ketones can aid with fasting, making it easier in healthy people and allowing some people with specific issues to fast in spite of them without worrying as much about hypoglycemia. They also help with dementia and many other issues even if you take them while not fasting!
    Glycine and trimethylglycine can also be useful supplements while fasting that won't break ketosis and have many benefits.
    Children, pregnant or nursing women should not fast for periods longer than 16 hours. People with pancreatic tumors or certain forms of hypoglycemia generally cannot fast at all. Type 1 diabetics can also fast but it is more complicated and should be approached with caution as it could lead to ketoacidosis. If you experience extreme symptoms of some kind, especially dizziness or tremors, then simply break the fast and seek advice.

    My 74 year old husband went to his physical therapy appointment Wednesday.
    His therapist told him his eyes were “ yellow.”
    Unfortunately my husband takes a statin, takes more acetaminophen ( Tylenol) than he should and likes his vodka .
    Isn’t that a possible sign of jaundice/ high liver enzymes?

    I started researching healthy eating because I use to have prediabetes. I also have dementia and alzheimers according to the doctor. I have a problem with short term memory loss. Im 64. You mentioned most of my favorite must eat foods for good health. So i think this is great. It just confirms to me that I'm on the right track. If i had to add one thing, i would say get hemp hearts to add to almost any dish. A light nutty buttery taste. And i add walnuts to both albacore tuna salad and canned chicken breast chicken salad and chopped apple and craisins to my tuna and chicken salad.

    Personally trying to decode this. I don’t consume suger, highly manufactured products, alcohol, rarely any dairy, Not overweight. I am so puzzled to be dealing with FLD. Frustrated.

      What about people like myself who dont eat sugar, eat low carb and no processed food, yoga, walking , strength training, sleep 8hrs , no alcohol and my cholesterol is up and i have fatty liver.

    Foie Gras is a French delicacy made of duck liver. Before slaughter, over some weeks ducks are force-fed with mostly maize/corn to develop a big, fat liver.

      Corn is processed in so many of our foods: corn starch, corn syrup etc. That is why processed foods should be avoided as much as possible

    For the pass 6 years, I alternate between keto, low carb high fat, ketovore, carnivore and always gluten-free. My AST/ALT are in the teens and triglyceride is 42. I don't have fatty liver.

    I found what best works for me is a zero carb diet. Its nearly all meat through out the week and on the weekends i allow myself a 10 gram carb limit. Prior i did keto for a few years, didn't lose any weight. I was 220 pounds. Stopped eating vegetables and lost 50 pounds in a few months.

      Yeah, for me, it is eating almost all salmon or shrimp and avocadoes with a side of cruciferous veggies. Drinking lemon water all day with cucumbers, ginger and mint. For some reason, that seems to really help me lose weight.

      Yes!!! I am a skinny one. I have damage from autoimmune hepatitis but at some point from too much coconut oil and coconut milk use, I developed fatty liver. Everything I looked up had to do with weight loss when I'm not overweight. It was so frustrating. Thankfully, I cut out the culprits, and I no longer have it, but yes, skinny people getting NAFLD needs addressed.

    I just found out I have it !they were taking a CT scan. I have a kidney stone and discovered I have fatty liver and fatty pancreas. I’m not really over weight OK maybe 10-15 pounds tops and now that I’ve changed things up I dont feel like my pancreas hurts sanymore.😮

    So you can't say that everyone can eat certain things in moderation..
    For some of us, it just doesn't work...

    Good stuff. Americans are being preyed upon. When you travel to other countries that are healthier the main thing you see is less consumption. They just plain eat less. Now they also eat healthier stuff but often times its stuff they tell us not to eat here - like MEAT! Cheese, dairy, etc

    My 2 cents worth: Most liver problems in today's society & with general health is mostly caused by the hideous foods that are put out for people to eat due to the additives in foods or lack of vitamins in veggies and fruits to do depleted soils in Big Agri. .
    Whole foods and organic are too expensive for many. In most other countries you don't find as many health issue as in USA... they don't allow the herbicides, GMO's, vaccines or medicine in animals going to market as is permitted in USA.... Many countries won't allow our foods or meat into their country because of this. I know that's simplified but it's the truth.
    Most all our food is contaminated or depleted soil with no nurishment for plants by Big Agr.!!! and REFINED everything... Fluroide in water. It's slowly coming out (Back to the Future) Raw milk, saturated fats, eggs and grass fed animals are as they have always been, the best for our natural bodies !!!! SUGAR, that's another issue.... THEY put it in most everything you buy and most of time it's not whole cane sugar but processed & turned to poison.

    But aif you are someone who already has a fatty liver or even fibrosis or cirrhosis, and you want to try to reverse it, you do not want to be having that 1 tablespoon of honey per day. You would do better to give up all forms of sugar and carbs in order to heal the liver.

    😂 "I only use Agave" 😂 Agave, for the uninformed is roughly 78% fructose. So about as bad as it gets. It's also the reason tequila can make you drunker faster than other same proof alcohols. Some of the remaining sugars are acted up on immediately, as the liver places fructose and alcohol at the same priority, which allows for slower clearing of the alcohol, which means higher blood concentration.

      Yes it can be reversed. Cut out all sugars and processed food immediately. Continue not drinking alcohol. Don’t eat fruit since fructose is a form of sugar. Do an extended fast. My AST levels were at 318 back in late November. Normal levels should be between 11-51. I was over 6 times the healthy levels. My ALT levels were at 70 and they should be between 13-39. That was a little over 2 months ago. I immediately did a 108 hour fast (4.5 days) then I did grass fed grass finished steak once a day. So that’s 23 hours fasting and 1 hour eating. I would also do a 36-48 hour fast once a week. So for the last 9 weeks I would eat about 7 meals a week (on weekends I would eat an extra steak as a reward. In my 1 hour window I would eat grass fed grass finished beef, free range chicken eggs, wild caught fish and raw milk. I would have some tudca and milk thistle every now and then. My levels are now ALT- 41 and AST-20. My doctor called me the first time and wanted me to get an ultrasound and possibly get on a liver transplant list. When I was younger I knew about fasting and how it cures all types of diseases so I did some more research. It’s so much easier to come across that knowledge thanks to technology. I told my doctor over the phone on his first call that I was going to change my diet. He said that was not enough. Fast forward to late January after my blood test when I got my results through my doctors offices portal he couldn’t even call me, he had a nurse call me to tell me the good news. Idk man most doctors want you sick 💰. I hope this helps

      My husband and I have both had our gallbladders removed. We were told that everything would resume as per usual. What we never were told, and later learned, is that it can impact your inability to metabolize fat soluble vitamins. You need sufficient bile to absorb things like vitamin d3 for instance. I was deficient in so many things. When you do not have a gallbladder, your liver has the duty of producing bile. There is no stores built up in a gallbladder. Sometimes the liver doesn’t wind up producing adequately enough. Taking digestive enzymes is helpful. Also taking liquid supplements.

So if anybody has susceptible alcoholism predisposition due to genetic liver issues it wud be wise not to touch alcohole or drugs to aviod alcoholism n addiction.     My son is 18 and has 105 ast and 157 alt. He has hyper pigmentation in his lower body. I do not see any symptoms.
    He is idle cos he has adhd.
    He doesnt drink coffee.
    He and my elder son are high on carbs badly and keep drinking sugary drinks.
    I wept today cos i am worried and i dont know how to handle this.
    I am a lone mom.i am gonna start him with milk thistle, turmeric, nac, selenium, omega 3 in moderate amounts cos everhhting is so expensive. I have some choline as well.
    Can i use cacao powder to make something with erythritol to help him have some sweet sensation.
    Any suggestions to me please help.
    He does not eat veggies and fruits. I dont know how and what.
    He loves meat.
    I want to give him some beef liver once a week so that we can see some changes to his liver.

      Do not substitute sugar with erythritol, it is a sugar alcohol. Also damages the liver (like alcohol). Try 1/2 once of olive oil & 1/2 once of lemon juice and pinch of cayenne pepper in an empty stomach. Stabilizes liver enzymes.

    If sugar (carb) is the culprit, then most veganism advocates and followers (high carb, low fat) should be FLD patients but somehow this is not case, they’re healthier when it comes to liver disease compared to other who follow other diet.

    Overall, the podcast was not great. The most important part was the 12 foods that helps fatty liver but you did not go over each food.
    Improvement is needed.

    They didn't mention that the liver and pancreas are coupled organs and if you want to improve your liver function you should improve your pancreas function. Also coffee enemas are one of the best ways to flush out the liver.

    You can have a body that does not drink alcohol, nor fruit juice, no soda, nor junk food, no breads, no desserts, no honey, no processed foods, low carbs, and still get fatty liver. We still do not have an understanding of all the causes for this metabolic syndrome. Be careful on how you characterize the disorder. Be honest about not knowing everything about the cause of this problem. We do not know everything about the cause of this problem. Going low carb and getting sugar out is the first step to metabolic health. It works with some. Not all.

    Again, and again and again, I have asked if anyone knows anything about Crohn's Disease, when talking about how to eat healthy?? Ok my brother died of fatty liver disease; I have Crohn's disease, I cannot eat the fiber without being in pain. I can't digest "healthy fats" because of the crohn's, and because I don't have my gall bladder anymore. Every time I hear someone talking about eating "healthy" nuts and seeds I cringe! Please, please, please talk about crohn's disease and how to manage it without mentioning biologics and steroids and the like. There has to be on this planet, food that is healthy and contains bio-available nutrients, vitamins and minerals our bodies need to function properly, that we can digest and facilitate. WHY doesn't anyone talk about Crohn's Disease????

      Just on TH-cam alone there are a bunch of videos about it. Maybe it's one of those hard to treat conditions? I have a recurring foot issue/injury that fits the too hard category. It sucks to not be able to walk. Lot's of talk, too few solutions.

      Check out Dr Anthony Chaffee and Kelly Hogan’s (My Zero Carb Life) Stop eating plants. Stop eating carbs and let your colon heal. Only eat fish, meat and eggs. And cut out ALL meds. My father had Crohns but completely reversed it by eating that way. Good luck.

      I had PCOS it's no fun. I cured it by losing about a stone in weight, cutting the fizzy drinks and snacks. It can be done but you have to lose weight. I wasn't much overweight but it still made a difference losing 15 pounds. They say having PCOS makes you put on weight, but I think it's the other way round. Good luck

    Great info but ive been listening for an hour and a quarter and you keep mentioning ' the book'. Nobody has said yhe name of the book or even your names so i can find it.

    Talking about eating a donut... she's wrong in some cases. For some of us with food addiction it's like touching alcohol. Sugar hits the brain like other substances including alcohol and heroin according to research.

      Without sugar it is likely that it would cut alcoholism and drug addiction numbers by at least half and probably by more than that.

    The reference she made about the Japanese women and breast cancer wasn't correct. The increase in breast cancer for those women was 100% because of the decline in IODINE in their diet when the diet changed. Japanese people eat the most Iodine than anyone else in the world and they have the lowest cancer rates because of it; especially breast cancer.
