Correct, but as a southern Chinese (I am from Guangdong and living in Australia), I think mainly northern Chinese would ask surname, especially to elderly people. Nin Gui Xing (您贵姓，what's your honoured surname). Southern Chinese, especially Cantonese people, and especially young people, would ask: Zen Me Cheng Hu Ni (怎么称呼你，how should I call you).

Yes, you'd better ask the Last name(姓)，您贵姓? And you can call 王叔uncle Wang，李阿姨aunt Li，张哥brother Zhang，etc.
