I think the game is doubling Gragas’ bonus max hp due to spaghetti. Gragas ult might summon an invisible Gragas clone or something (minion?) on top of the player, but that’s just a theory.
Zeri isn't bugged, the passive does no damage but neeko also applies her own passive too and her ad which makes it bugged or at least a rito game design moment

1 Barrel makes you full, 2 makes you even more 'fuller' xD idk
I got a bug with that augment that resets basic abilities on kill and with vladimir that caused me to be unable to use any ability.
