A biochemist once mentioned that omega 3 fatty acids are highly unstable and will oxidise at the drop of a hat. If you take a supplement that's a lot of highly unstable fat that will turn rancid in the absence of lipid soluble antioxidants. Eating seafood will digest slowly and so the fish oils are released into the blood stream at a rate less likely to overwhelm circulating antioxidants. Then there is lipid peroxidation occurring while the oil is in storage from ambient light and heat. I think I read about Krill oil that has astaxanthin? a red pigment that is also an antioxidant in krill oil and so it may be less likely to produce reactive oxygen species.

I'm both an RN who works for the VA and also a vet myself. The VA discontinued this due to lack of supportive clinical evidence (that it really does much of anything) and difficulties in product acquisition.
That said, nothing is preventing you from going out and buying fish oil on your own.
Here is the actual letter that providers were given in January 2024:
Over the Counter (OTC) Fish Oil Supplements No Longer Provided by VA
      Dear VA Healthcare Provider:
This letter is to inform you of a decision made by the National Formulary Committee (NFC) that OTC /nonprescription fish oil supplements will no longer be provided by VA. Reasons for this include the lack of supportive clinical evidence (see below) and difficulties in product acquisition. Over the past few years, the VA Pharmacy Benefits Management (PBM) contracting department has struggled to award a contract for an OTC fish oil supplement that meets VA high standards for quality and consistency. As outlined in VHA Directive 1108.08 VHA Formulary Management Process1, supplements can be provided if the following conditions are met:
      1. Supplement possesses sufficient scientific evidence to support its safe and effective use in the treatment of a specific disease state or condition.
      2. The product satisfies or supplements an unmet need in the treatment of a medical condition or offers an advantage over FDA approved medications.
If those conditions are met, the PBM identifies and gives preference to those manufacturers who comply with good manufacturing practices (cGMP) and carry either a “seal of approval or certification” from an independent company (e.g., USP Verified, NSF, etc.) that certifies their product is produced in a quality manner, does not contain contaminants or impurities, contains the labeled dose per tablet/capsule and is properly labeled.
Evidence for Use of Fish Oil Supplements (Regarding Condition #1-Supporting Evidence):
      Cardiovascular Risk Reduction:
There is a lack of evidence supporting use of OTC fish oil supplements for reducing cardiovascular (CV) risk. Clinical practice guidelines from professional organizations do not recommend routine use of fish oil supplements since evidence does not support use for reducing CV risk.
Professional Guideline/ Level/Strength of Recommendation Population Recommendation
      AHA/ACC/ACCP/ASPC/NLA/PCNA
      20232
      COR-NB
LOE-B-NR Patients with CCD Use of dietary supplements, including fish oils, is not beneficial for reducing CV events.
      VA-DoD 20203
Weak against recommendation Primary or Secondary Prevention Recommend against use due to lack of evidence for reducing CV events
AHA=American Heart Association, ACC=American College Cardiology, ACCP=American College Clinical Pharmacy, ASPC=American Society of Preventive Cardiology, CCD=Chronic Coronary Disease, COR-NB=class (strength) of recommendation, no benefit, CV=cardiovascular, LOE-B-NR=level of evidence, level B, nonrandomized, NLA=National Lipid Association, PCNA=Preventative Cardiovascular Nurses Association
      Non-Cardiovascular Uses:
There is lack of evidence to support the safe and effective use of OTC fish oil supplements for treating non-CV conditions (e.g., Alzheimer’s disease, Rheumatoid Arthritis, dry eye disease, polycystic ovary syndrome [PCOS], etc.) and therefore, use of fish oil supplements is not recommended.
FDA Approved Prescription Omega-3 Fatty Acid Medications-Potential Alternatives to Fish Oil Supplements in Eligible Patients (Regarding Condition #2-Unmet Need):
There are two FDA approved prescription “fish oil or omega-3 fatty acid” medications that can be considered in eligible patients including omega-3-acid ethyl esters (e.g., LovazaÒ and LovazaÒ generics) or icosapent ethyl (VascepaÒ). Although there are FDA approved generics for icosapent ethyl, they are not yet available. National Criteria for Use (CFU) are available for both agents and generally reflect clinical trial evidence and FDA labeled indications (See below). VA National CFU (general inclusion criteria)-Accessible via: VA Formulary Advisor
      · LovazaÒ and LovazaÒ generics: Patient with triglyceride (TG) level > 500 mg/dL on 2 occasions.
o Secondary causes of hypertriglyceridemia are considered and addressed/managed.
      · Icosapent Ethyl (VascepaÒ): Reducing CV risk in patients maximized on statins, history of ASCVD, LDL between 41-100 mg/dL and fasting TG > 150 mg/dL.
The effect of Lovaza, Lovaza generics or icosapent ethyl on risk for pancreatitis in patients with severe hypertriglyceridemia has not been established.
Statins and fibrates may be considered for reducing triglyceride levels, if clinically appropriate, in selected patients.
Supplements are not FDA Approved for Safety or Efficacy for Any Indication:
Although nutritional supplements, such as fish oils, are regulated by the Food and Drug Administration (FDA), they are not subject to the rigorous approval process used by FDA for reviewing the safety and efficacy of drugs prior to entering the market. Because of this, supplements are not permitted to make claims of treating, diagnosing, preventing, or curing disease.
Additionally, since supplements are not as closely scrutinized before entering the market (and can enter the market without notifying FDA), there is the potential for obtaining a product that does not meet VA’s established high standards for quality and consistency unless there are companies willing to seek certification from independent companies providing such a service.
Therefore, consistent access to a certified high-quality OTC fish oil supplement product is no longer sustainable through VA and contract-awarded companies have been unable to meet supply demands over the past several years.
These challenges have led the National Formulary Committee (NFC) to determine that because of the lack of evidence to support use of non-FDA approved OTC fish oil supplements for CV or non-CV indications and because of the inability to ensure VA is providing certified high-quality fish oil supplements to its Veterans, VA will no longer provide fish oil supplements. However, for eligible patients, there are two FDA approved prescription options available through National CFU.
      Provider Guidance:
      1. For patients receiving fish oil supplements from VA, consider the indication for use and determine whether a prescription medication is indicated in place of the OTC fish oil supplement (e.g., based on VA CFU or other evidence).
      2. VA pharmacy will discontinue any active prescriptions for OTC fish oil supplements over the next several months.
      3. Inform patients of reasons (lack of evidence for use, inability to ensure consistent access to a high-quality product) VA is no longer providing OTC fish oil supplements and offer the patient letter as additional information. (Link to patient letter will be included)
      4. A discussion should be held and ideally documented regarding the paucity of supporting clinical data and the need to ensure selection of a high-quality product, if continued. However, while OTC fish oil supplements are commonly available from a variety of retail and online sources, VA does not make any recommendations for the purchase of OTC fish oil products.
      5. If a shared decision is made between provider and patient to continue with OTC/non-prescription fish oil supplements, the patient can obtain from a non-VA source at their expense like other supplements that are not covered under the VA formulary plan. Remember to add the OTC Fish Oil as a Non-VA Medication in CPRS / Cerner-Oracle given the potential for drug-drug interactions and side effects (e.g., bleeding).
