I recently attended a conference about security and compliance (put on by APA) for therapists and the presenter mentioned, in passing, that the most common infraction brought against clinicians by various governing bodies, is when the therapist did not release the clinical record to the patient as requested (or did not do so in a timely manner). My hunch is that maybe some of these clinicians did not really keep their records up to date. Very stressful when asked (I've experienced this a few times). Thanks again for your videos!
