  • The final video in a series of lectures on the use of perfusion CT of the brain in patients (with suspected) acute ischemic stroke. In the previous we talked extensively about the spectrum of abnormalities that can be seen in patients with acute ischemic stroke, how to analyze and interpret these imaging findings and the role they can play in selecting patients for reperfusion therapy when they present outside the time window.     Hi, I have a question regarding the carotis stenosis case (starting at 8:16): Why is the hypoperfusion most severe in the posterior watershed area between MCA und PCA? As we have a stenosis in the ICA, we should expect that the PCA (which originates from the basilar artery) still works fine. Therefore, blood should still reach the posterior watershed area and it should not be the area with the worst perfusion in the brain. Instead I would expect the anterior watershed area between ACA and MCA to be hypoperfused first.
    Am I making a mistake here?
    Anyway, thank you very much for this great video series!
