Pineal Gland Cysts: imaging findings and approach
This video is about the imaging appearances of pineal gland cysts and how you should deal with them. Pineal Gland Cysts are a frequent finding on MRI studies of the brain, present in up to 23% of the general population, but also a frequent cause of unnecessary follow-up and patient anxiety. The main worry is that a pineal gland cysts might be cystic pineal gland tumor, which is almost never the case. This video provides a general overview on anatomy of the pineal gland, the imaging characteristics of simple and more complex pineal gland cysts, and how to differentiatie them from pineal gland tumors.

      Good luck and I hope you will find the correct diagnosis and correct treatment. Radiology is just part of the puzzle, my advice would be to get a second opinion from a neurologist specialized in MS and inflammatory brain disorders, I'm sure they'll be able to put all the pieces together and provide you with the correct answers to all your questions.       @@ebbamarie It's not perfect, but this is how I do it (sometimes I change it a little but it gives you an idea) Inflammatory changes (if present): Maxillary sinuses: Frontal sinuses: Ethmoid air celles: Sphenoid sinuses: (report mucosal thickening, frotty change, fluid-levels, hyperostosis etc...) Ostiomeatal complex: Frontal recesses: Spheno-ethmoid recesses: Anatomical variants: Olfactory fossa: (Keros type ... ) Lamina papyracea: (report if infact / Haller cells / adherent uncinate process) Onodi cells: present / absent Sphenoid pneumatization: (type; also mention dehiscent carotid canal, optic strut, vidian canal, sphenoid septum insertion..) Ethmoid notich: (supra-orbital pneumatization above ethmoid notch yes or no) Nasal septum: (deviation / bony spur?) Nasal concha: (concha bullosa, paradoxical turn, hypoplasia etc...) CONCLUSION: Sinusitis yes or no (if possible suggest acute or chronic, but not always possible, only reliable signs are fluid-levels for acute disease and bony sinus wall hyperostosis for chronic sinusitis). Report pattern if present (infundibular, OMC, etc...) Report predisposing factors if present. Report surgical risks if presents.

Have several things in progress, problem is I get easily distracted and jump from one thing to another... at the moment got some stuff on imaging of sinonasal, temporal bone and pituitary pathology; something in spinal cord lesions, and somthing very basic on interpretation of head CT for radiology newbies. Knowing me, chances are the next update might be none of those and something complete else ;)

      There is little valuable learning material in the literature regarding vascular neuroimaging, including aneurysms (your cases would be irreplaceable here), AVMs and other vascular lesions. Moreover, focal changes in the skull are a very interesting topic - they rarely receive much attention, including metastates, primary growth changes, and developmental anomalies. These are my loose thoughts and suggestions for interesting, valuable topics. Your appearances have a very high scientific value and it would be great if you continued your lecture activity on YT.
