    Telehealth in so many areas is an exciting development. I saw a therapist over the phone and it really worked well - did it during my lunch hour.

    To what extent do you do a physical assessment with a patient, either remote or in office?

      Thank you for your question.
      Physical assessments are rarely performed as a PMHNP except when you are performing such assessments as described in the video (AIMS). There are providers who perform basics such as heart and lung sounds, but keep in mind that this is really out of the scope of a PMHNP. If anything is found with a physical assessment, including labs, a PMHNP should be deferring to a medical provider for further evaluation and treatment d/t scope of practice.

    Hi Dr. Camille, I am a PMHNP student and I’m about to start my clinical rotations. May I ask what EHR system you use for Telehealth and is there a scheduling component embedded into the system?

      @Catherine Nicholson Thank you for your great question. Congratulations on where you are on your journey!
      EHR systems vary greatly. I have used Epic and AMD at larger organizations, which can be very similar to any system you would use in a traditional setting. Some have scheduling capability; others have another system that "talks" with scheduling. Keep in mind that some may also require another application for the actual video visit, while the EHR may focus on just documentation. Does this make sense?       Thank you so much for your response! Yes it does lake a lot of sense. When I managed the FHC, we used Cerner (Genesis) for the military and those systems “talked” to one another so it was great. I do have another question which may seem silly but you were discussing 1099 Contract vs W2…would you mind elaborating more on that? Lastly, do you practice with someone or have your own practice? I’m in a Full Practice Authority state so I’m looking forward to doing both while I build my clientele. Thanks again for your knowledge and sharing so many great tips!

      @@cnic77 Ah, I'm glad that makes some sense.
      W2 vs 1099 really comes down to benefits in a nutshell. There are many private organizations that hire you as a 1099 worker, where you work as a contractor. The general rule is that W2 positions come with benefits and automatic tax deductions. There are some other differences - I did a video on this as well here on this channel but I am happy to talk with you offline if you'd like.

      @Phoenix the best Thanks for watching and for your question. Pay for telehealth services is universal for the most part, as most insurances now recognize telehealth visits. Reimbursement is based on ICD-10 codes for each visit. PMHNPs are usually paid per usual, meaning per visit, per hour, salaried, etc. This may vary in private practice.

      @@phoenixthebest9731 There is no way to give accurate numbers. This varies greatly based on insurance, state, setting, etc.
      Are you familiar with the various models of practice as a PMHNP? Are you a PMHNP?
      If you haven't explored this already, most settings do not may PMHNPs per visit. But even with this model, a % of pay may go to insurance, some reimbursement provided to PMHNP based on rates. If you want to see what the reimbursement rates are for each insurance panel, you would need to request this from the insurance provider. If you are working for a company that pays per visit, this would be a good question to ask when interviewing.
      This can get a bit complicated, but please reach out with additional questions.

    Great video do you currently work as a 1099 or W2 employee. I’m currently working BH outpatient practice with large health system doing both in person and telehealth visit for the last 3 yrs . Currently FNP recently completed PHMNP preparing for boards soon. Also work a 1099 telehealth position. Thinking about doing this full time for flexibility. Which do you prefer.

      I currently work as a W2 employee, which I recently transitioned back to after being a 1099 contractor for the past 2 years. While there are perks to being a 1099 contractor, I don't prefer it unless there is a consistent means of establishing patients. For me, being a 1099 contractor worked better as a part-time job d/t unpredictable volume of patients.
      Which do you prefer? I'd love to know your thoughts.

    Thank you for sharing your experience. I am graduating in 90 days and was wondering if you think this is a viable option for a new grad? It seems like it would be just as easy to message a question as it would be to wait until a provider/ colleague is not seeing patients to ask a question? Thanks again!

      @Brigitte Lee Thanks for watching and for your question. I do think telehealth could be a viable option for new grad PMHNPs, under certain circumstances.
      Really, what I have found most important is the need to have readily available support as a new grad. Support could be more limited in telehealth although the ability to ask a question could be as simple as a message. I also think that being able to read body language and good assessment skills are important to hone as a new grad, which can be more limited by telehealth.
      Ultimately, it is your decision, but when seeking such positions, I would suggest asking questions around the level of support that you will have. Often times, supervising physicians are overseeing many more APRNs than they would be in-person, so keep this in mind.

    I just started watching your videos. Im currently an RN in critical care and am beginning to think about pursuing further education. PMHNP is one area I am interested in pursuing. Some questions for you that may help my decision:
    Is over-saturation an issue you have experience with? One of my main concerns would be going back to school, graduating, then not being able to find a job or not making enough more to justify the schooling costs.
    Do you feel psych rn experience is a necessity? I have floated to the psych floor at my hospital and do not feel like I would enjoy working as an rn there.

      Thanks for reaching out! Congrats on the point that you've reached in your career thus far!
      I know it can be somewhat of a challenge to think about what may be best going forward.
      Saturation has not yet made its way to the PMHNP career. In fact, there is a high demand for psychiatric providers currently (that may vary based on geographical region), but telehealth has broadened this scope as well.
      There are a few things that I would consider when making this decision:
      1) What do you think you would enjoy about the career?
      a. What is it that you don't like about working the bedside in psych? Some of these same aspects may be a part of working as a PMHNP, although the role is much different.
      b. Psych experience is not always necessary, although it could give you a good basis for working with the population and can help you to hone skills. Some programs do require experience.
      I have done videos on all of these topics, so feel free to peruse around the channel, and let me know what other questions come up.
