This is RL 169 if you started as Wretch and use Godrick's Great Rune.
I am at RL 190 and started as Astrologer, so I put all extra points on INT (useless for this build).
Overview
Body: Bloodboil Aromatic (+30% for 60s)
Aura: Rallying Standard (+20% for 30s)
Weapon: Royal Knight's Resolve (+80% for the next hit within 10s)
Passive: Twinbird Kite Shield (+5% when HP is below 20%)
Passive (madness): Aged One's Exultation (+20% for 30s), Black Dumpling (+10% for 60s), Madding Hand (+7.5% for 30s)
Passive (poison): Kindred of Rot's Exultation (+20% for 20s), Mushroom Crown (+10% for 20s), Poisoned Hand (+7.5% for 20s)
Passive (blood loss): Lord of Blood's Exultation (+20% for 20s), White Mask (+10% for 20s)
Talismans: Axe Talisman (+10% for charged attacks), Two-Handed Sword Talisman (+15% for two-handed attacks), Spear Talisman (+15% for thrusting counters), Red-Feathered Branchsword (+20% when HP is below 20%)
Tears: Bloodsucking Cracked Tear (+20% for 3m but HP is drained), Opaline Bubbletear (+90% damage negation for the next hit within 3m)
Armor: Rakshasa Set (+2% per piece)
Extra: Old Lord's Talisman (+30% spell and AoW buffs duration)
The Old Lord's Talisman extends the duration of Rallying Standard, allowing small mistakes.
Why Opaline Bubbletear?
The fights unfolds like this: we charge the attack, Radahn hits us, our HP go below 20%, we hit him. Without the Opaline Bubbletear Radahn kills us.
One could raise VIG (39+), but I wanted to keep the RL as low as possible.
The main weapon is the Quality Zweihander +25 (two-hand, charged, counterattack). Why?
Radahn has 20% damage negation to pierce, 0% to holy, and 40% to everything else.
Counterattack bonus damage (+30%) applies only to piercing attacks.
The Zweihander is the best weapon for piercing charged counters (highest base damage + can be buffed with many talismans).
Some bonuses/buffs do not apply to holy damage, so Sacred would not get x17 bonus damage.
Damage check
Base damage (two-handed, charged, counterattack, no buff/equip): 3,176
Bonuses are multiplicative, for example 30% + 20% + 80% is actually 1.3 x 1.2 x 1.8 = 2.8
Buffs bonus: x17.769...
Total damage: 56,436 (math checks out perfectly, I didn't expect that lol)
With buffs from the Spiked Cracked Tear and the St. Trina's Smile, the damage can go up to 77,575 • Promised Consort Radah...

Charge the heavy attack 0.5-1 second after Radahn starts the animation for his dive attack
DO NOT LOCK! Manually aim 45 degrees to your left and HIGH
Radahn will hit you first, your HP will go below 20%, then you'll hit him
Quickly heal after the cutscene or you'll die because of HP drain
Note that we wait 1-2 seconds before using the Warming Stone: if we use it too early, we won't have enough HP to survive; if we use it too late, HP won't go below 20%.
Original build by @EmberBorne and later improved by @videogamebutter. Mine further improves it with the following changes:
Madding Hand, Poisoned Hand, and Rakshasa Helm for more damage.
No Shriek of Sorrow (it wears off as soon as we two-hand the Zweihander).
Old Lord's Talisman gives a bit of extra time.
Lower RL. Lower VIG is compensated by using a Warming Stone.
Can survive the fight.
