    I am a paediatric specialist who have just finished my training in Singapore and I have obtained MRCPCH. I am planning to apply through the approved jurisdiction pathway. Several questions I would like to ask: 1) Do I need to do the MCCQE Part 1 ? (or can directly goto the Royal college exam?) 2) Can I apply for clinical fellowship before taking the Royal College exam? 3) Through the approved jurisdiction pathway, I can be specialist in paediatrics by Canadian Board after passing examination without going through residency? 4) If I would like to further pursue career in paediatric cardiology after that, how can I apply to enter into a fellowship programme?

      There are two types of clinical fellowship available - where you as an IMG will get paid or you bring in funding yourself. I assume you want to join something where they will pay. For this you will have to explore fellowships and showcase a strong application!

    Im a psychiatry resident from AIIMS. I will be completing my residency in next 2 months.. So the best route u mentioned is to get a fellowship in any province.. Did i get that Right? 1)When should i preferably start looking ? 2) If my experience and competence is good i can continue to apply work in higher posts without needing to write give exams or repeating residency ?

      Dr. Adharshana, in my opinion, the best route is fellowship. You should start now if you intend to move. I don't mean to say everyone who gets into a fellowship will get into province based pathway but yes that's right for Q2.

      Dr. Jorge, in my opinion, Canada offers a province based pathway because they are short of doctors and they want to build their research capability through IMGs. The expectation is that you continue academic career but if you chose to do clinical practice after going through the process and have an independent license with no restrictions, then you should be able to!

I am a little confused about after fellowship if you can practice right away? Can you work on restricted license after fellowship while you are trying to complete the royal college exam?       If you are board certified by the American Board of Radiology, it signifies that you have met the rigorous standards set by the board. However, it's important to note that licensing requirements vary by province in Canada.
      After you finish your fellowship you must satisfy the licensing requirements of the province where you intend to practice, obtain a license & then start practicing.
      If you don't qualify for the licensing based on your years of training or any of the other reasons, there are other interim solutions people take based on their individual backgrounds. The earlier you take care of this, the better it is!

      As a nephrologist from Brazil, you can apply directly to do a clinical fellowship in General Nephrology (or advanced nephrology fellowship if you are eligible) at Canadian Universities. Once you get successful in this, then you look out for options for a job under the province-based specialist pathway as I have explained in this video. I personally would approach this by applying for fellowships and not focusing on exams at this stage. These are things that are not advertised. I know this by experience & being in the system - hence the reason for sharing the 'IMG Secrets'.
      Yes, the process is similar for all IMGs with specialist training from Brazil or other low-middle income countries.

    I am Pathologist in Iran where pathology residency is 4 years long and as i noticed accepted minimum in Canada is 53 months. I am assistant professor with 5 years experience, will my academic position compensate the shortcoming of education schedule to apply for PER ? I mean the 4 or 5 months that my education have been shorter than Canadian pathology residency? Also, the other route that you mentioned in the end am I eligible for that?

      In general the training period should be the same. They may run an exception for outstanding candidates. To apply for PER, there is considerable expense. To me this appears to be a tricky situation. If you are ok risking that fees, then you could apply to see if they will accept you into PER. I have the same feeling about SEAP. The Royal College pathways are in general are more favorable towards those IMGs trained form High Income Countries. In your position knowing all i know, i would personally explore the fellowship options in Canada and potentially province based pathway because of a 5 year experience or alternatively the USA

    Hello Dr. Rajeev, I really appreciate your sharing about this alternative route through the clinical fellowship. I'm a nephrologist from Taiwan and I've been doing research on the practice eligibility route for months. Could you be so kind as to tell me how could IMGs get their clinical experiences for 2 years in Canada? (This is the issue I couldn't figure out after searching for so long a time.) Would you recommend to get the eligibility for RCPSC's exam for internal medicine first then dive into applying clinical fellowship in order to find a job in the province-based speciality pathway? Or it's easier to apply for a clinical fellowship first? Thanks for your reading.

      The practice eligibility route and the province based specialist pathways are two different routes. You have to do one or the other and not both (thankfully!).
      You have two options:
      1. Practice eligibility route, if you apply to this and get eligibility to write the Royal College Exam in Internal Medicine, getting clinical experience will not be difficult.
      2. Enter Canada as a clinical fellow directly in Nephrology & explore Province Based Specialist Pathway. The fellowship is definitely easier to apply for and does not require any exams. However, getting into Province Based Specialist is not easy and depends on factors that I have explained in the video.
      If I were in your position and knowing so much about the different pathways in Canada, I would explore the practice eligibility route more seriously. I would also simultaneously apply for nephrology clinical fellowships in Canada to get Canadian experience on my CV!

Though I haven't got any response from them yet, I remember that their application mentioned that if I wnat to continue working in Canada after fellowship, I need to inform them beforehand and for that, I need to give seap exam.. .? Could you please shed some light on this? Coz in your present video, u mentioned that seap means v give exams, both written n OSCE type n yet, v get sub speciality license only. Am I right, sir ?

    I have applied to all Univesities except in the french speaking provinces for Fellowship as per your Suggestion and thank you very much for that information. I have yet not recieved any reply from most of the Universities. I am considering other options (Practice Eligibility Route ). I have done a Residency in Russia for 3 years Have compleated a D Ortho in India which was 2 years. Also Compleated my Part A MRCS from Royal College of Edinburgh. Should I wait and only apply after my MRCS part B reults ?

    Hello Dr.Rajeev, thanks for your informative video. I am a board-certified dermatologist trained and working in Taiwan ( 4 yrs of dermatology residency training + 3 yrs experience as consultant) . I am thinking of pursuing the Practice eligibility route (PER). However, i am confused by the following 2 issues: 1) how strict is the royal college in determining "Time and content in the specialty training must be substantially equivalent to the Royal College specialty training requirements" ? Since its never possible for 2 different countries to have the exactly same training time and requirement, even the jurisdictions in the approved jurisdiction route differs greatly in training requirements. Have you seen any successful experience in be eligible to write the royal college specialty exam theough PER? 2) If I managed to pass the royal college specialty exam, is it possible that I would have very little chance of getting the requested "final 2+ years of practice in a continuous practice location in Canada"? I am worried since you have said in a previous post that there are very limited work positions which are highly competitive in the field of dermatology in Canada. Thanks in advance for your help~

      Where is your microbiology PG from? If it is from India you are not eligible to take the Royal College Exams unless you complete residency in Canada. Your best bets are the job based, fellowship options or re do residency.

      Thank you Dr. Munjur. Are you asking would you get Canada PR based on your MRCOG? If so, Canada provides PR based on a point system. So Education wiht a master's degree or PhD equivalent will score high. My suggestion is that to try getting it through Canadian Experience Class and it really makes a positive difference based on what i have seen!

    Hi Doc! Am not a fresh IMG. Not completed specialty either. Did MCEE 20 y ago and USMLE part 2 as well. Worked as an RN for amost 20 yrs here in Canada in Geriatrics.

    Amazing video thanks Dr.I have a question ,I am a Urologist from Iran and an Endourology Fellowship which in total i tarined for 6 years.(4years residensy +2 years fellowship)
    The point is in Iran urology residency duration is 4 years but in canada it is 5years.
    Do you think i am eligible for PER on not considering 2 more years i have taken for fellowship?

    I have checked the royal college pathway from the website, and it was stated that 5 years of experience is needed which can be done in the jurisdiction countries, but 2 years of the job experience must be in Canada continuously, May I ask you how difficult it is to find a job after passing the exams ? As we won’t have the license to practice at that time !

      To state in a simple way, the sequence is medical training, then residency and then fellowship! You could do residency in Canada or any outside country. Both will make you eligible for a fellowship. If residency is from Canada or USA, then you will get a full license, if residency is from an outside country then you have to work your way after fellowship through different routes for the full license. Does this make sense?

      Are you asking who are IMGs? In my videos, i refer IMGs are any doctor trained outside USA or Canada. If you asking are you eligible for the pathway i mention in the video? yes if your medical college is World Health Organization approved then you can apply for fellowships. It is competitive but if interested worth trying

If I have to simplify the whole process, then - To apply for an alternate entry path the IMG has to complete post-graduate training in the home country, apply & then get selected for the alternate entry path program. In the USA, you need USMLE for an alternate entry path. In. In Canada, no exams are required.
      If you enter through residency, the USA requires USMLE and Canada requires MCC exams.

    I am currently in my early 50s , graduated from one of the top three medical colleges of india, specialised in Internal medicine and sub specialised in HIV Medicine. Have more than 30 original research articles in international and national journals, i have fellowship of indian college of physicians, American college of physicians, I am on reviewership panel of some of the top medical journals including British Medical Journal, American Journal of hypertension, Journal of association of physicians of India and more. I am not keen to write and wait for Medical Council of Canada results for approval to immigrate. What sort of pathway do you recommend for me?? Does getting a recommendation letter from a canadian physician would do?? Thanks a lot, your clarity of thought and expression is worth appreciating.

    Any specific reason why you shifted from Canada to Usa? Canada have long timeline to start practice or getting job in Canada is more difficult?

    I’m a Canadian citizen I have done my fellowship in Obstetrics and Gynecology from Pakistan presently working in KSA as a consultant
    I’m 50 years old.
    In may I will be writing my MRCOG part 3 last part. I have passed Canadian MCCQE PART1 in 2014
    I want to come back to Canada my kids are studying in Canada. Kindly suggest me what is suitable for me
    And during early 3 years of fellowship will I earn or not. I have to support my kids. Cannot work without money.

    I'm a radiologist and got eligibility to take 2024 march radiology board exam of Canada. I applied practice eligibility rout.
    I'm not LMCC ,but I need 2 years experience in Canada to get royal college 's certification. my question is can I find job without taking MCCQE1?
    And another question is can I apply for fellowship without MCCQE1 and have you any idea if it helps ?

      Dr. Vaibhav, Canada offers both options. If you find a funding source, of course, they will be more than happy to take you in.
      If you are from a place where you cannot find funding then you will have to go for a fellowship where the hospital has to pay you. So yes a clinical fellowship is paid - not a stipend but a salary!

      OBGyn is not an easy specialty to get into. Your best bet is to apply for a clinical fellowship in a low-competitive OBGyn subspecialty to enter Canada. You may also have to repeat residency unless you show exceptional achievement to get selected into the province-based pathway! No exams are required to apply for a clinical fellowship.
