    Pediatrician here and I just found this video. Love it! I remember tasting different liquid meds in residency. Clindamycin tastes like burning garbage (no wonder the kiddos hate it so much), so now it's my last choice of medication to prescribe unless I have no other option. Also yes, one of the biggest pet peeves in peds is the use of the word "lethargic." There's the parent definition of the word, then there's the medical definition of the word. If you use the word "lethargic" to describe a child in a medical setting that means they're not responding to anything you do to them and you're sending them up to the PICU.

      Why not just ask the parent some follow-up questions to better clarify what behavior they are labeling as being lethargic? Aren’t you in the profession because you like to help people? Don’t miss the forest for the trees. I would rather a parent use the word lethargic than to not even bring their child to see the doctor at all.

      @@raychelpanton9851 That's exactly why I said that there's two different meanings of the word depending upon who says it. Of course we ask follow up questions and examine the child to determine how sick they are. It's when one medical personnel is talking to another medical personnel that we're very cautious about using the word "lethargic."

      I remember having to take clindamycin as a kid and hating it! But my mom would do “frat boy chats” (basically just chanting drink or chug over and over) for me so I took every dose. Also always had a big cup of cola to use as a chaser replacing the strong bad taste with a strong good taste. Not sure how it looks from the outside but it works!

      @@sarahb7626 I learned this the hard way a few nights ago. We were taking care of a 3 year old who was lethargic. My ignornant ass was thinking that the kid was tired no big deal. Turns out the kid literally couldn't lift their arms when I went into the room to shadow the exam and one of their eyes was deviating. Ending up having to tube her like an hour later. A lethargic kid is a sick kid indeed.

      I think it depends person to person never bothered me still doesn't neither does the constant crying but for some residents it's unbearable

    When I was an ER Scribe, I was very careful to (1) teach new scribes about the difference between lethargy vs fatigue vs weakness and (2) NEVER put letharg or weakness in a medical cart, even if parents used those specific words, without the physician's say so. With the EMR we used, if you chart certain symptoms like lethargy or weakness and the physician DOESN'T do a fullblown workup and/or admit the kid, the chart will be marked for review and the physician gets "dinged".
    Same issue with Shortness of breath vs Dyspnea, Tingling/Parasthesia vs Numbness, Syncope vs Loss of Consciousness, and a few others.

      there was an actual study which showed there IS a correlation which makes sense cause it means you have a better foundation of knowledge. Id argue theres no correlation to personality tho

      You don’t need to, just ask the Coordinators or unit secretaries for some stickers. I’ve worked at a couple of Childrens Hospitals and they are everywhere. The stickers on your badge is perfect though. It’s a great conversation starter and a good distraction/focus area when doing exams/assessments.

    As someone who finished an adult residency (adult psychiatry) and now taking up a child psychiatry fellowship, I can relate. 😆 Treating kids is so much of a different world of its own, no wonder there is fellowship for this.

    The overuse of the word “lethargic” to describe a kid is my biggest pet peeve.
    They’re not lethargic unless they’re dying 😠

      What word would you prefer? Asking seriously. My son is usually very active but when he gets sick he is totally different (obviously) and just wants to lie on me and watch videos. To me the difference is so stark that I would use the word lethargic but if there’s a better word to share with a doctor/nurse, I would love to hear it!

      ​@@Sprickles58 'Low energy' or 'fatigued" would probably be better; since 'lethargic' means they're too weak to talk/smile/respond when awake and hard to wake up from sleep.

      @@Sprickles58 Decreased activity level, increased sleepiness, fatigue, and malaise are the words and phrases I usually use to describe that when doing my documentation. “Lethargic” means the kid is basically unresponsive and I’m worried about some severe altered mental status.
      If you still want to call your child lethargic when talking to your doctor that’s ok though! When I ask more questions I can figure out if that term is medically appropriate or not and use the correct descriptions for my documentation. But even healthcare providers misuse the term sometimes and that’s where it becomes my pet peeve.

    At least in young patients, definitely looking in mouth is worse than the ear. Check ears then mouth and then abdomen last cause they hate laying down

    If you’re good at summarizing a patient and know the 2 things that you have to tell the cross cover, do NOT spend a jillion minutes updating the written sign out.
