Safe blood transfusion is an immensely vast topic, where donor selection is one of the crucial subject to discuss. Directed blood transfusion is one of them. For long, it had been established that, 1st degree relatives (eg. Father, Mother, Sister, Brother and your children) are the ideal blood donors. But in recent years, medical science has revealed that transfusion among this category may result in a very severe complication like Transfusion Associated Graft vs Host Disease, where donor's T-lymphocytes attack the host T-cells without being countered by the host immunity. Because in some rare circumstances, host immunity fails to recognize the donor's T cell as foreign and ultimately those foreign T-cells remain without counter attacked. This results in severe cytokines storming and host reactions, that causes severe tissue damage in the host body and at the end it produces multiple organ failure following erythematous maculopapular skin rash, Liver failure, GIT disorders and bone marrow aplasia. The incidence of this reaction may be much lower (only 0.1-1%) but, the fatality of TA-GVHD is nearly about 100%. So, it is high time to make good concern among the general people to have some knowledge about this fatal complication of directed blood transfusion to avoid unavoidable circumstances.
