The most horrific toilet scene I ever saw was in a motorway service station on the M1. I walked into the gents and there was a row of cubicles, all closed, except for one open door near the end. As I approached it and the inside came into view, I honestly couldn't believe what I was looking at - there was shit smeared EVERYWHERE, all over the pan, the walls, the floor, even flecks on the ceiling. Most disturbingly though, there was a full set clothes discarded on the floor - socks, jeans, pants, shirt, belt still in the hoops, all on top of a pair of trainers - and that too was covered in shit, with a single neat solid log on top. What happened in that cubicle, I can't even begin to imagine.

My theory is that someone had an attack of terrible, terrible diarrhoea, shit themselves before reaching the service station, and so took a change of clothes into the toilets with them. But then they found there wasn't any toilet paper, and so in a fit of crazed rage they chose to shit some more on the discarded clothes on the floor instead of in the toilet, and for good measure scoop up that shit and smear it everywhere in revenge for the lack of paper.
Years ago I encountered a toilet in a Mexican fast-food restaurant in Berkeley, CA, that still haunts my dreams. It looked like it hadn't been flushed in three days. It had been used for three days, just not flushed. I really had to go, and since public toilets were sparse in that particular area of town, I relented and start peeing into the mix, all the while gagging.
As someone who has visited Kolkata many times to see family I can confirm this story is 100% true. Things are much better now tho but I still wouldn’t use them.

I got off a flight from Hong Kong to Manila in 1983 -- must've got a bad oyster in HK, so I raced to the airport toilet at the Manila terminal and they were selling the toiltet paper by the single sheet.
I find one in Madrid, Spain, which was completely covered with shit... I felt bad for whoever have to clean that mess... maybe the guy mixed indian and mexican food, Idk... xD

He’s right! As someone who’s travelled across all South Asia I can say unfortunately the hygiene in India is close to non existent especially in the rural areas. Smells of shit and piss all over. Neighbouring Bangladesh, Pakistan and Sri Lanka are better in that aspect.
