I work on a cardiovascular IMU. We use heparin for the inpatient. Xarelto or Eliquis are prescribed for the patient being discharged. Also, the inpatient is started on Xarelto or Eliquis before being discharged. Basically, the patient is being "bridged" before he/she leaves. Warfarin is less commonly prescribed. Regardless, this video is awesome and very helpful. I have already watched the "Heparin" video. Another awesome video from the plethora of videos by Ninja Nerd.

As a Medical Laboratory Science Professional that runs this PT INR test: INR is calculated by taking the patient's PT/average normal PT, raising it to the power of the ISI = (Patient PT/Avg PT)^ISI. PT does not equal INR. Normal PT is typically around 12seconds. Just a small correction that wanted you to be aware. Great videos though. I teach Hematology Labs for a Medical Lab Science program and your videos are super helpful!

But I was wanting to know if you could make videos in the future on the extreme damage Diabetes can do on the human body and the organs. Thanks!!!
Amiodarone could also affect Warfarin metablisim and increase INR. You should mention that on your video as it is commonly used drug for AFib although it is only in in-patient setting.
What do anticoagulants and rat poison have in common? Coumarins. And an O.D. of coumadin and poisoning involving coumatetralyl compounds has 1 antidote: phytomenadione (Vit. K)

and both are correct: a) if you drink a lot one day - EtOH molecules compete with other molecules for spot in available cytochromes. warfarin can't be metabolised enough, so there is more warfarin in blood. b) if you drink for a long time - your organism feels that he need more cytochromes, because you give him substrates like every day. so in response - you have more cytochromes to metabolise molecules like warfarin, what leads to reduced concentration of warfarin in blood.
