    Is the shaft assumed to be rotating if it isn't stated within the problem? I thought it would operate in a nonrotating mode due to the sole bending load

    Not sure if this is correct. Be careful with this example. I've got a=39.9 from my text boob and b=0.995. He also never mentioned if the shaft was rotating, which would affect kb.

      I agree with that. We have to use an equivalent diameter "de".
