For very simple designs and for just a few t-shirts, yes, you can use paper stencil.

For thick(plastisol) inks, pushing is much easier to print. But in terms of print quality(smoothness), pulling is better. The problem with pulling is it requires a lot of strengh in you arms/wrists...

Use mockup and decide your design size that can look best on S/M/L/XL/XXL... It's NOT "one size fits all". But you have to choose one size only for screen printing. Digital printing surely has advantage on this with adjustable print/design sizes...
