    Thank you so much for your videos. I have successfully passed my gastroenterology partial exam thanks to your videos. I would like to add to this video that it is possible that a patient has both low levels of vitamin B12 AND folate. In this case it is bad to give only folate. If I understood my professor correctly in that situation you need to give vitamin B12 first and folate later OR give both of them. Teaching via TH-cam is a noble thing, I hope I will also contribute with some tutorials to the world.

    You left out celiac disease. Nearly 34 years gluten free and I still fight macrocytic anemia. I get tired of being asked if I'm an alcoholic. The answer is no! I seldom drink any alcohol...once in a blue moon I'll have a glass of wine with dinner, that's all. I also didn't appreciate being diagnosed with possible bone marrow failure. I have a very real small bowel biopsy diagnosed case of plain old ugly non tropical sprue

    Question, when you did the clinical scenario and you expanded the question to include that the PT MMA and homocysteine levels were elevated, you said that meant b12 deficiency. But can it also point to Folate deficiency existing in conjunction with b12 deficiency? As it could have in the scenario before the MMA and H levels were added ?

      Dharin Sawh Yes. It's possible to have those findings if there is a B12 and folate deficiency.

    Hi Paul! I got a question for you.
    Why would you keep giving patients b12 orally for life? I mean, if the patients change their diet why we should keep giving them b12?
