Yes, you are good to submit via mail( Get a tracking# from USPS) , check will be deducted within 30 days once they receive the docs . Also, receiving the receipt notice may take time.

Yes, you need to select the Preparer’s statement 7a and fill your information in the preparer’s information in part 5.

Never Submit anything original. I just read, as per USCIS they will destroy the original documents unless its been asked b y USCIS
