Retinoids are central to the treatment of acne because of their multi-pronged attack on the acne pathway.     I’m in my fourth week with 0.05, and love it so far. Bit sensitive and flaky sometimes, but the glow I have is insane. The spots and pimples go away so fast. Can’t wait for next month

      @@silavie1378 in my first 2 weeks I used it once/ twice a week. Then 3 times a week for 3 weeks. And now since two weeks I use is every other day.

      @@ITSmindgrowing thank you for helping me ! I don't know why but in my weird experience with tretinoin i notice sagging skin and i'm still in my early 20s ! It's realy helful in case brightening and glow but my cheeks are more down saggy now 🤯 did you notice anything like that ?

    I have been using .1% tretinoin for 20 years. That is my skin care routine along with moisturizer and Tizo-3 sunscreen tinted and non tinted. I have been to several dermatologists and they are amazed on how my skin looks. And a plus, when I go out with my 33 year old son people think we are siblings. I’m 59!

      @@MomoSimone22 I use either vanicream face lotion or cerave skin renewing night cream. I had extremely light peeling along my jaw the first week or so but that was it.

    I'm in my fourth week with tretinoin 0.025 and it works incredibly 😭❤️ I don't mind with the purging because when it heals, it glows and make my skin less oily. God luck for everyone who's fighting with acne 💪

      @@jamienoe7531 hey, I don't have huge pores again right now. I think it's because Tretinoin makes my skin look plump. I was an acne fighter for 6 years since high school. Now I'm almost 26 and never have concern about acne anymore 😭 Everytime I met someone struggling with acne I will always tell them to wear tretinoin at night and SPF in the morning. That's it!!! game changer! I don't even use toner, essence, and other serums again. 😭 It's not a myth tho, it's scientifically proven that Tretinoin is one of the best anti aging ❤️

    I’ve been using tretinoin for about 2 years consistently and my skin has gotten so much better!!! I will get a couple pimples through out the month still but a significant improvement! It’s expensive here in the US but it’s worth it!

      In India we get it for 160rs which is about 2$. India is a haven for health care and access to good medicine with cheap price.
      I've started my tretinoin journey and it's about 3 weeks. I can see the improvement and im so happy 😊😊😊

    0.025 made me purge, especially in my chin... I almost gave up but I didn’t. 3 months later, my skin is finally clear and it looks so nice, I don’t even wear make up anymore. My question is, do I have to use it for the rest of my life? How do I know when I need to go up?

      When you get the results you want it is recommended to slowly decrease the use of it. So if you use tretinoin daily, start using 4 days a week, then 3 days a week, and if you face still looks clear you can then stop using it. It is not recommended to just stop using after you got the result because the acne can go back as fast as you think.

      You can obviously stop using it but don’t be mad when ur acne comes back , ima use it for the rest of my life , its even good for anti aging .

    i have been using 0.05% tretinoin for about 8 months now approaching november as of date to combat my hormonal acne(oof Im embarrassed to show my face sometimes haha). it is said that dramatic results would normally show after 3 months but for me it was 4 months. occasional acne flairs gradually decreased and in addition to using azelaic acid, completely halted. Now after 8 months, my complexion improved by leaps and bounds!. For the irritation, I used a moisturizer that contains panthenol (cosrx) , I still experience irritation but very very mild as I'm increasing the dose gradually. All in all, tretinoin is a miracle worker, my complexion is smooth and dewyyy now!. i started tretinoin because of your past videos and also dr dray. Thank you!!

    Tazarotene is so much better for my skin. After 15 plus years of Retin-A-and I even try Altreno (tretinoin) 0.05% lotion--I tried and fell in love with Tazorac 0.1% cream. OMG this retinoid had less irritation, no peeling, and gave me better results.

      0.05 is best for acne, and 0.1 if the acne is still persistent after using 0.05 for a decent amount of time.

      Yes it does have a stronger effect, but you start with 0.025, so your skin can adapt and you will minimise side effects like irritation, flaking, redness, etc.

    Yes! Tretinoin in cream really worked for me. I had a long time history of stubborn acne.. i cannot adhere to the oral tretinoin because of the side effects on my blood tests.. so i started using the tretinoin cream 0.05 four months ago and believed and be patient on the process.. after four months of using, i definitely achieved the results!! My skin is clear and looks fresh! My friends noticed the big changes. Im still continuing to use it as a maintenance. I love the results !

    Been using 0.05 Retin A cream prescribe by my doctor for two years now and my acne has not improve at all . The first 6 months my skin got worst but now after two years its the same acne level has when I started.

      You might need to try the oral form, isotretinoin (known as Accutane or Roaccutane).

      The Tretinoin gel works better for me and unlike the cream it doesn't break me out....however, I can't seem to be able to find a compatible moisturizer with it...everything I've tried balls up into a stringy mess...ugh

    Tretinoin is an effective long-term treatment for the treatment of acne. While it won't work for everyone, studies show that tretinoin works to encourage cell turnover that can even skin tone, treat breakouts, and decrease the appearance of acne scarring.

      Yes, that's what I've found. It helps to diminish the scarring as well as the acne itself, whereas benzoyl peroxide (a more damaging ingredient, in my opinion) seemed to make it worse in my case.

    Which one would you pick between tretinoin and adapalene? I've been using adapalene to treat acne and worked very well, but I want to switch to tretinoin because of its cumulative benefits against aging and acne.

    Question for Dr. Sam: what time should we stop tretinoin application before pregnancy? Usually the metabolism of vitamins in our bodies is two months. Is it enough for vit A? I am planning pregnancy some time later and wanted to know when to stop applying it. Thanks! It would be highly important for those ladies, who like me, look forward to babies

      I would say it's more important to stop eating foods containing high amounts of Vitamin A, such as liver, paté and fish oils (which don't come with any health warnings!). Tretinoin is applied topically so is unlikely to cause liver toxicity in mum or baby. Only trace amounts will be absorbed into the bloodstream (unlike the oral form, isotretinoin/Accutane). However it's recommended to err on the side of caution, so perhaps just apply a very thin layer of the lowest strength (0.025%), no more than once a week during pregnancy? Or switch to retinol if you're worried. Might be worth consulting your doctor as well.

    I was wondering why the benefits seemed slower for the last few months. I’m on the middle strength. Then I heard you say at some point I can increase from pea size to finger tip size amount.     Used it for a while and it made such a huge difference! It is flaky on my chin so I don’t use it often there. Also, you are more prone to dryness and sunburn. I had horrible sunburn once with it so I stopped using it for a bit. If your gonna use it use a thick hydrating moisturizer . I use la roche posay cicaplast baume b5

    I used tretinoin for a year,it cleared up my acne and my skin became so smooth,,,i also got compliments about how good my skin has improved,,,however😬😬😬 ,i stopped using it and thats the biggest mistake i did😪...Im starting again now, im starting by using a retinol from Inky list,once im done with it ill jump back to tretinoin and ill make sure NEVER to stop using it this time.

    My daughter has been using tretinoin .025 for four months and is starting to break out.Her skin improved really well the first three months.Is this a temporary purge?I watched a recent video on an ingredient in this product that can irritate the skin.I cant remember the name but she said it was in the cream and not the gel.

    I started using 0.5% Trevino in cream and I have yet to see results. It has been 5 days since I started. Am I being impatient? When does it typically start showing results. My dermatologist said that you can see improvements in your skin 6-10 weeks of use. I’m just impatient

    When would you recommend using tret over differin? Or changing from differin to tret? Since differin was developed for acne?

      You'll be getting the same acne benefits with tret with the addition of the proven anti-aging as well as the proven melanoma prevention benefits. Differin can theoretically have these benefits as well, but there's just not as many studies being done on the anti-aging or melanoma prevention for Differin as there have been for tretinoin - mostly because of how new adapalene is in relation to tretinoin.
      tl;dr, Differin is great for people who only care about acne, with some POTENTIAL other skin benefits that aren't guaranteed. Tretinoin is for someone who wants the full package guaranteed.

      I'll often start with Differin and then escalate to tret, depending on tolerance. But if they're concerned with premature ageing from the kick-off, then I'll go straight to tret.

      @@TheDiamondLadder I saw your comment about having been on accutane (I also got acne coming back after accutane 😞 and differin never fully cleared me, but what has worked for really clearing and less irritation is prescription tazorac gel .1% and I use it with short contact therapy, where I apply it for 5 min or longer once you have worked your way up, then I wash it off with a gentle cleanser. This has made the biggest difference for my clogged pores and my deep breakouts. I also use benzoyl peroxide every other night on my chin (my worst breakout area) as a leave on under moisturizer. Elta MD pm moisturizer is the only one that doesn't break me out and in the morning elta md spf 44 tinted, it is dark enough for my tan olive skin. I just saw your comments and I know the struggle, so I thought I'd share. Tazorac is expensive, but I get 3 tubes for $100 through insurance so it isn't that bad cuz it last a long time. Hope you are having good luck in your clear skin journey.

    Im on my 3 months of using tretinoin. I realized that the acne itself doesn't flare up again. But, the PIE or redness and acne scar still last on my face. On a daily skincare routine i always put sunscreen on my face and reapply every 3 hours. Any suggest/recomendation ingredients to handle the PIE/redness and acne scar? Thank you.

      Best thing for my pie is niacinamide and sun screen, any la roche posay products with in it I love I use the effacular H wash and effacular H moisturiser

    I used 0.1% and it worked! I saw improvement in my fine lines and scars. I used it for about 7 months.. But then I suddenly irritation attacked me. So I downgrade to 0.025%. I haven't touched my 0.1% again. Maybe it was too strong for my skin.

    Thank you Sam. I'll be starting tretinoin next week as I have malasma (how to spell it) and many years ago I burnt my face with hot soup (a whole other story) and it removed my freckles. Consequently, although (luckily) no scarring, I have these kind of non freckled patches on my left cheek (on my face).

    My doc won't prescribe me tret, I think because I seem to also have some early rosacea. Adapalene and Azelaic Acid for me!

      I am using also differin and azelaic acid 15% and no iritation. I put it over my moinsturiser. It works for acne and redness👍

    I find the gel is great for the feet and the cream for the face and the scalp; didn't know it helped with hair follicles which explains a lot, my hair has never been more healthy. One note, there was significant peeling for about a week of usage but after that all side effects subsided and my skin, hair, and feet have never been better 😊

    I have a tube of tretinoin from the dermatologist, but I see that it has isopropyl myristate in it which I’ve seen on lists of ingredients that are highly likely to cause acne. Is it still safe and effective to use on extremely oily and acne prone skin?

    Hi Dr. Sam I'm Leah from Philippines. I started tretinoin 3 weeks ago I still experiencing purging but because of your videos I am committed to stick to it. I hope it goes well 🙏. My question is I am working out almost on a daily basis and we need to wear mask while working out. I noticed it makes my skin so red. Any advice on this?     My daughter has struggled with acne for two years. We got it under control for one of those years and decided to try a combo pill to control hormones more to go off of antibiotics for her acne. She felt unwell on microgynon 20 for some reason. Very dizzy and nauseous. However, after going off the pill, she is having the worst breakouts of her life. She is now in minocycline 100 mg and used retrieve .05 every night. It just wasn’t working even after seven weeks of Retrieve. So we have switched to Duac 5% for the past four weeks. Slightly better but she has heaps of PIH and although lesser, new acne is still coming. We are sooo tired and worried. A 17 year old shouldn’t have to deal with this. She also has evidence of hursutism on sideburns but blood tests come out ok on the hormone front. Lost. Been to an endocrinologist and no answers. Three dermatologists. Can we use retrieve at night and Duac in the morning? Would blue or red light therapy be better? She we go in oral tretinoin again here a generic formulation in NewZealand. She tried it two years ago but into month four, it caused the most terrible cystic outbreak in her skin ever. 10 mg daily dose.

      The problem with antibiotics is that on long term use it becomes resistance to the bacteria and while our skin has both good and bad bacteria the good ones being the one that helps retain our moisture barrier function, antibiotics kills it too causing more acne breakouts in future.

      the acne will come back worse after the antibiotics finish. avoid at all costs. it will also harm the liver. i speak from experience.

      As it says it don't work for everyone. So just try one more month and if you don't get over the purging stage, then quit it and start using isotretinoin if you have severe acne or if you have mild to moderate, you can start using epiduo gel

    It worked for me....decades ago! I had embarrassing acne as a young teen, and this product along with a few others did the trick. My skin went from loathsome to lovely in just a few months. I never forgot that! It was a miracle for this kid!

      @@chelabella5635 I believe whatever the dermatologist prescribed were all Retin-A-based. One was for nighttime only on the problem areas. It was a gel. The other was a Retin-A with a tint in it so, I used it as a foundation. The third product was a brown oval-shaped soap called Salicylic Acid Soap. It was manufactured by the Steifel Company. I washed with that every day. I used the tint every day. I used the nightime gel every night. Within 3 months...my skin was gorgeous!! You'll need to go see a reputable dermatologist. Good luck, hon.

    Hi Dr. Sam,
    Do you find that the vehicles that Tretinoin cream and Tazorac cream are in can clog pores. The gels are a bit harsh for me and I struggle with melasma & acne. I feel like the Taz gel might be making melasma worse even though it has cleared me.
    I'm nervous to try either Tret or Taz cream due to some of the ingredients.
    Do you see either of these creams clogging pores? or is that not possible? Thank you!

    Tretinoin is the best thing I’ve ever done to my skin.
    Suffered from hormonal acne for over 3 years. I changed my diet to a healthy whole foods based diet but still broke out. Purchased expensive products from the esthetician. Worked for a little bit then failed. Tretinoin was the only thing that has worked and my skin looks beautiful. Now to work on scarring 😊

    Dr Sam, i've a question. I've been using adapalane for about a year now, and done with the purging phase and my skin is improving a lot. The acne has been reduce significantly to almost none. But right now, i left with acne marks and some hyperpigmentation. I'm wondering, if i switch to tretinoin microsphere 0.04%, will it be another purging phase ?

My skin stopped reacting after adding moisturizing products like Hadalabo premium lotion, Vanicream daily moisturizer, Purito Dermide Cica sleeping pack. I still get some acne due to PCOS but its way less than before

    at first I had a very bad experience with tritinoin because of the purge but believe me now its been a year and I don't get any pimple and break out for the first time in my life something has work for my skin iam so thankful to tritinoin even tho i almost gave up in my purge days but I did start it again after few weeks and now my skin is clear it feels like everything below my face has recovered and all the acne causing infection has gone that is why my skin isn't catching any more pimples...
