I'm 25 yrs old and diagnosed with essential thrombocytosis. I'm taking hydroxyurea twice a day. Symptoms, many. Extreme tiredness or weakness often,itching,redness or sores on skin, fast heartbeat, numbness on feet, severe allergic reactions.
I'm two years in from having treatment for ALCL. No sign that's coming back, though prior to my lymphoma diagnosis, my platelets were coming up at around 550. I am JAK2 negative. My platelets have continued to rise and are now just over 700. I've had various symptoms, including vision disturbances, severe headaches, nose bleeds, strange purple bruising, but worse, I've been diagnosed with RA and after watching this video and seeing the mention of erythromelalgia, I believe this is what I have. Severe burning, pain and swelling to hands and feet that comes in flares. I'm having a bone marrow biopsy in a few weeks to look for MPD. I think erythromelalgia needs to be looked into but presently have a rheumatologist with little to no interest.
Not sure what I have if anything. I have many symptoms mentioned. Recently had a bone marrow biopsy 20th Nov 2023. ( platelets 1072). They've increased over 3years. Follow up 11th Dec 2023.
Mayo clinic write, "If your blood count is above 450,000 platelets per microliter of blood, your doctor will look for an underlying condition. He or she will rule out all other causes of high platelet counts to confirm a diagnosis of essential thrombocythemia."
For the lat 17 years I have been told by my GP all my blood tests are normal. Around 10 years ago I developed migraine, numbness, fatigue, breathlessness, more recently I have extreme weakness and cannot walk any distance due to fatigue and weakness. I am 57 yr old female, to date GP has been unable or uninterested in identifying the issue. A few years ago I requested a copy of my medical notes to find that every blood test I have had over 15 years has consistently showed abnormal platelet count. I asked to be referred to a hematologist and was refused. The weakness is getting worse and I struggle with basic tasks I am so weak some days. GP says I may never get to the bottom of what the issue is. My platelet could has gradually slowly increased over the years and yet again last week I was told my recent results were normal only to discover my platelet count was 539. I am not at the point of not knowing where to turn, it feels like gaslighting and is now affecting me emotionally.
Do you have / Have you done 1. Endoscopy of the stomach 2. USG Abdomen and Pelvis 3. Vitamin B12 and Haemoglobin Level. 4. Repeated infections 5. Boils on legs

Although I think you should qualify the "Never trust a lab tech" to "never trust a lab tech that trusts the machines" (some of us actually do check manually, yet in mass labs it was seemingly less often run manually unless specifically ordered and, of course, paid for. Once upon a time tests were routinely run manually, especially if the machines said something was wrong, or if they said everything was "A OK!", yet there was reason to believe it wasn't. @@MedicosisPerfectionalis I know was a joke but I have seen your videos oriented to med students as additional preparation for my MLT test. Our training is not just for run machines, we are trained to certify the results in the machine are correct, we have to know about diseases, how to correlate with results. Before release a panic value for example in a CBC we have been prior validated with a smear manually the results. We work in very regulated area, we work with quality assurance, we run quality controls every day, every turn. Maybe if we only run machines without a knowledge in clinical background I could been happily sleeping right now. But I'm here like a med student, studying hard, cause doctors need us. Anyways.... thank you very much you are a great teacher!!!!! I love your work.

I as a Medical Technologist was thrown off by that. We spend hours on QA and QC and ensure the best result is sent out, only to hear the doctor say "don't trust the MT". That got to me I ain't gonna lie.

Well, my teleconference with my new hematologist is next week. 3 recent tests all had my platelets at around 1200. I’m 67 yo male. It’s takin a long time for this consultation. At first I was concerned, but I’m not now. All I know is I feel great. I started a very low carbohydrate diet, walk between 5 and 7 miles a day, stopped all alcohol 6 months ago as I had an embarrassing beer habit, and I lost 45 pounds, from 210 to 165. I’m taking 81 mcg of aspirin and unless this doctor manages to scare the crap out of me, I’m not going to worry about it. My dad is 92 yrs old and did have a deep vein clot when he was 90, he’s doing fine.

If you are highly knowledgeable about things such as platelet counts, a patient's concern about elevated platelets with no symptoms may seem silly to you. This does not mean you ridicule or dismiss the patient at all, as the expert it is your job to clarify and explain. Similarly, if you ask your mechanic if it is okay to put canola oil into your car instead of motor oil, that is a stupid question from the perspective of a mechanic. But you probably dont understand the difference and they should explain it to you nonetheless. I have just been referred to a hematologist. Apparently for the past three years I've had elevated platelets. 2020: 458 2021: 437 2023: 480 All of my other blood counts are fine, and I don't have diabetes or anything else. I was recently diagnosed with PCOS (though I've been symptomatic for years). I'm wondering if perhaps my PCOS is causing inflammation which is causing the increase in platelets?
Yes i have high platet as well as pcos also my glucose lwvels are high these are all because of sugars we eat and hormonal imbalances sadly we develop insulin resistance which is the root cause of it all

The New England Journal of Medicine profiled a 45 year old man who died of a heart attack associated with Essential Thrombocytosis. His platelets were in the low 500’s. The same thing nearly happened to me. This range is dangerous! If you have elevated platelets don’t panic but take it seriously.

Multi doctor is just investigating now to see why my white blood cells are so high so I’ve been doing a lot of screenings to rule things out but I am staying prayed up and I do believe that God is in control so I just try to follow the medical advice given because I do believe that he works thru doctors. The higher the platelet count, the higher the risk of bleeding - the reason is that the overproduced platelets are abnormal. It has also been theorized that this paradox of bleeding risk with high platelets is caused by an acquired von Willebrand Factor deficiency syndrome.
My daughters CBC came back low hemoglobin and her platlet count is 569 her RDW is low and her lymphocytes are High as well as a few others. She is only 9 months. Could this just be from anemia? Shes being tested for bloof disorders. She was born missing a thumb. Thank you

A lot of platelets counts I'm seeing are way better then mine and it makes me scared of what could be possibly causing my counts to be at least in the 600 range 😞 I'm currently seeing a hematologist oncologist and my first follow up is in a few days I'm praying that I dont have something serious wrong with me. That platelet count is not way high, but I have an MPN and started out at 475. If subsequent CBC's consistently show high platelets, you should see a hematologist and have more testing done.
I stumbled in here somehow and boy am I glad I did! I have a platelet count between 450 and 550 and I donate platelets frequently. I was always wondering if I should bother asking my doctor about it since nothing else is "wrong" with me. I'm happy to know that it can wait until my next yearly appointment. This guy has no idea what he's talking about. I can't figure out if he's looking at textbooks from 50 years ago or just likes to make numbers up since they don't adhere to the WHO or NCCN guidelines. You need to see a hematologist, preferably an MPN specialist. I had high platelets for over a decade with multiple doctors telling me it was no big deal . The first 5 years or so, they were in the 500s, then gradually started climbing. Finally last year they hit a million and I was sent to a hematologist. The first one misdiagnosed me with essential thrombocythemia. I went to an MPN specialist at Mass General (Harvard) and had genetic testing (positive for JAK2 mutation) and a bone marrow biopsy which showed I have Polycythemia Vera. (I either had masked PV or essential thrombocythemia that progressed into PV.) I'm now on cytoreductive therapy and aspirin. I count myself lucky that unlike many patients with MPNs I wasn't diagnosed AFTER having a stroke or heart attack (which can occur at ANY high platelets count). BTW, we are high risk for Covid complications due to our high levels of circulating pro-inflammatory cytokines, as well as coagulation problems. Apparently you believe you know more than all the MPN specialists in the world, the World Health Organization, ICC, NCCN guidelines, ASH and the British Hematology Society. The diagnostic threshold is 450,000. That is based on evidence. Your 750k number is based on what? Your opinion? I'm the moderator of an MPN group with thousands of members and I will never back down from challenging you on this. Because it's dangerous and leads to unnecessary suffering which apparently doesn't concern you in the slightest. A large percentage of people with MPNs are diagnosed AFTER they have a catastrophic event like a stroke. It isn't only the numbers that matter. The mutation itself causes platelets to be more viscous and even endothelial cells in blood vessels. The WHO had legitimate reasons for setting the threshold at 750k. You need to edit your video. Have the maturity to admit you made a mistake. Don't double down.

How are you? In ET, most clinical manifestations are thrombosis and vaso-occlusive symptoms (pain can occur anywhere like in bones of the arms, legs, chest, and spine) and hemorrhage or bleeding.
That's very high and both essential thrombocythemia and polycythemia Vera can present without symptoms. However, many symptoms are non-specific, such as the major one - fatigue. @@nursia8449 IDA is rarely associated with pancytopenia, the PLT count is usually normal or even high. Of course there's been cases where a severe IDA can cause decreased number of plts, so yes after the appropriate treatment plts eventually will go back to normal.

Listening to you talk I can understand why my friend was killed in the ER by over-medication, my neighbor became ill and died when his intestine was punctured and the tech hid the fact only for sepsis to set in, an acquaintance was not told of her cancer diagnosis by an arrogant doctor the misdiagnosed her only for him to come back and say "sorry, you have stage 4 cancer and less than 30 days" , and why I was given medication for a condition I did not have - the doctor gave me another patients test results. You sound uncaring and condescending. No question is stupid if you don't already know the answer. And then, sometimes we forget and need reminding.

I’m 40 suffer with chronic hives for past year for no apparent reason after numerous visits to doctor for tests eventually had CBC test which came back very high platelet count. Referred immediately to rapid diagnosis central for various tests which came back with cysts on spleen, one kidney stone and fibroids NO CANCER. My GP then told me nothing wrong you have no symptoms no panic it’s just the way you are! So that’s it no explanation for my hives no treatment for what was found and my platelet count is now seen as ok! I don’t know what to do next

I've had a similar issue. Bottom line, it appears the high platelet count is due to anemia. I'm no doctor but maybe you should ck that.

Well, my teleconference with my new hematologist is next week. 3 recent tests all had my platelets at around 1200. I’m 67 yo male. It’s takin a long time for this consultation. At first I was concerned, but I’m not now. All I know is I feel great. I started a very low carbohydrate diet, walk between 5 and 7 miles a day, stopped all alcohol 6 months ago as I had an embarrassing beer habit, and I lost 45 pounds, from 210 to 165. I’m taking 81 mcg of aspirin and unless this doctor manages to scare the crap out of me, I’m not going to worry about it. My dad is 92 yrs old and did have a deep vein clot when he was 90, he’s doing fine.

Ya he's not funny. He's ignorant. Rising or falling counts of anything are not nothing. This is a perfect example of why doctors don't pay attention to shit. God forbid patients have questions. They're apparently all stupid according to this ignorant ass . As a nurse I find this this man highly offensive. As a lupus patient I'm disgusted. While my platelets don't get horribly high when they rise it indicates something is going on. Stupid man.

If you are highly knowledgeable about things such as platelet counts, a patient's concern about elevated platelets with no symptoms may seem silly to you. This does not mean you ridicule or dismiss the patient at all, as the expert it is your job to clarify and explain. Similarly, if you ask your mechanic if it is okay to put canola oil into your car instead of motor oil, that is a stupid question from the perspective of a mechanic. But you probably dont understand the difference and they should explain it to you nonetheless. I am sorry that doctors have been dismissive to you in the past, but no need to insult this guy who is doing a lot to teach us and using humor to make it more engaging.

I have just been referred to a hematologist. Apparently for the past three years I've had elevated platelets.
2020: 458
2021: 437
2023: 480
All of my other blood counts are fine, and I don't have diabetes or anything else.
I was recently diagnosed with PCOS (though I've been symptomatic for years). I'm wondering if perhaps my PCOS is causing inflammation which is causing the increase in platelets?

Yes i have high platet as well as pcos also my glucose lwvels are high these are all because of sugars we eat and hormonal imbalances sadly we develop insulin resistance which is the root cause of it all

The New England Journal of Medicine profiled a 45 year old man who died of a heart attack associated with Essential Thrombocytosis. His platelets were in the low 500’s. The same thing nearly happened to me. This range is dangerous! If you have elevated platelets don’t panic but take it seriously.

The higher the platelet count, the higher the risk of bleeding - the reason is that the overproduced platelets are abnormal. It has also been theorized that this paradox of bleeding risk with high platelets is caused by an acquired von Willebrand Factor deficiency syndrome.

A lot of platelets counts I'm seeing are way better then mine and it makes me scared of what could be possibly causing my counts to be at least in the 600 range 😞 I'm currently seeing a hematologist oncologist and my first follow up is in a few days I'm praying that I dont have something serious wrong with me. And I think I also have iron deficiency anemia because my iron and ferritin where super low but my hemoglobin and hematocrit right on the range for normal

I am 33 y.o I have started gaining weight and my skin is very itchy I got 470 and I also have chronic cough for the past 2 years, I was diagnosed with lung scarring and it's so difficult to breathe now.
That platelet count is not way high, but I have an MPN and started out at 475. If subsequent CBC's consistently show high platelets, you should see a hematologist and have more testing done.
I stumbled in here somehow and boy am I glad I did! I have a platelet count between 450 and 550 and I donate platelets frequently. I was always wondering if I should bother asking my doctor about it since nothing else is "wrong" with me. I'm happy to know that it can wait until my next yearly appointment. I will have to see if you have any other videos that interest me!

This guy has no idea what he's talking about. I can't figure out if he's looking at textbooks from 50 years ago or just likes to make numbers up since they don't adhere to the WHO or NCCN guidelines.
You need to see a hematologist, preferably an MPN specialist. I had high platelets for over a decade with multiple doctors telling me it was no big deal . The first 5 years or so, they were in the 500s, then gradually started climbing. Finally last year they hit a million and I was sent to a hematologist. The first one misdiagnosed me with essential thrombocythemia. I went to an MPN specialist at Mass General (Harvard) and had genetic testing (positive for JAK2 mutation) and a bone marrow biopsy which showed I have Polycythemia Vera. (I either had masked PV or essential thrombocythemia that progressed into PV.) I'm now on cytoreductive therapy and aspirin. I count myself lucky that unlike many patients with MPNs I wasn't diagnosed AFTER having a stroke or heart attack (which can occur at ANY high platelets count). BTW, we are high risk for Covid complications due to our high levels of circulating pro-inflammatory cytokines, as well as coagulation problems.
Anyway, look up the World health organization guidelines for diagnosis of myeloproliferative neoplasms.

A large percentage of people with MPNs are diagnosed AFTER they have a catastrophic event like a stroke. It isn't only the numbers that matter. Most family doctors (General Practitioners) don’t know how to manage many hematology cases, so they refer you to a hematologist.

How are you? In ET, most clinical manifestations are thrombosis and vaso-occlusive symptoms (pain can occur anywhere like in bones of the arms, legs, chest, and spine) and hemorrhage or bleeding.

What if platelets are more than 850000 with no symptoms?
That's very high and both essential thrombocythemia and polycythemia Vera can present without symptoms.

@@nursia8449 IDA is rarely associated with pancytopenia, the PLT count is usually normal or even high. Of course there's been cases where a severe IDA can cause decreased number of plts, so yes after the appropriate treatment plts eventually will go back to normal.

Listening to you talk I can understand why my friend was killed in the ER by over-medication, my neighbor became ill and died when his intestine was punctured and the tech hid the fact only for sepsis to set in, an acquaintance was not told of her cancer diagnosis by an arrogant doctor the misdiagnosed her only for him to come back and say "sorry, you have stage 4 cancer and less than 30 days" , and why I was given medication for a condition I did not have - the doctor gave me another patients test results.

I’m 40 suffer with chronic hives for past year for no apparent reason after numerous visits to doctor for tests eventually had CBC test which came back very high platelet count. Referred immediately to rapid diagnosis central for various tests which came back with cysts on spleen, one kidney stone and fibroids NO CANCER. My GP then told me nothing wrong you have no symptoms no panic it’s just the way you are! So that’s it no explanation for my hives no treatment for what was found and my platelet count is now seen as ok! I don’t know what to do next
I think you should seek a second medical opinion.

I checked my platelet count by automated machine it was 500000, same day I checked it manualy it was 261000 ,what is the reson for it
