I'm on my 17th week of my left Total knee surgery. The Doctor had a rough time at surgery because of PHDD, 2&1/2 hrs long. However I'm happy with it. Still working out the kinks and some mild pain. Oh, thank you about lifting up & trying to scoot & get rang of motion. Also slowly riding my I-Fast bike. Thank you for being on UTube!

I’m a year out and finally feel way better than I did pre op with just the occasional night ache. Now preparing for left knee in September. Hasn’t been easy but well worth it. Thanks for your informative videos Doctor.

I am 10 months out from my TKR. I was doing well until this past December when I started to have some right foot numbness and right buttock and leg pain. I had my TKR on the left. I was also having back pain. Went to back surgeon and had CT scan and it showed I have lumbar stenosis plus arthritis. Have had two epidurals which has not given me any relief. Bone spurs are pressing on some nerves. Talk about a let down. Been in a low mood since just trying to keep going. I thought my new knee wound let me get back to my regular activities. I am only 63 but I feel 90! I keep telling myself to keep going. Thank you for your podcasts and your expertise and wisdom! Take care and thanks!

The reason we had the TKR was to get back to our previous lives. I am 9 weeks post op and have pain daily, spasms, and can't sleep. Wished many times I hadn't gotten it done. Turning 70 this year.

Unresurfaced patella -- since my double knee replacement in 2022 I've complained to my otherwise crackerjack ortho about still feeling arthritis pain beneath my patella. I "joke" about the "phantom arthritis pain" and whether it will subside and he maintains he's hopeful that it will, but evades my direct questions as to whether my patella were resurfaced or not. Will have to ask him again.

Although this is a very informative video of the possibilities of what could be wrong… With my pain in my initial four month visit, I was just given the greenlight to just be patient and the pain will go away… would the x-ray have given them the answer for the pain persisting??if The implant is too big too small. I would like to see a follow up of what the doctor would do to pinpoint these like misalignment when… In diagnosing whether you have them or not… The bone scan was the only one that you mentioned and the blood test for infection, but the other reasons for pain were not addressed as to, how they would go about diagnosing it…before a year goes by!!! Thanks!

Don't know if this will help, but this is what I share with my patients at the 3-4 month mark -th-cam.com/video/vfrlKst-Ge8/w-d-xo.html

I cant thank you enough for your videos Dr. Rosin. Your 5 week kneeling protocol was a game changer for me. In this video you mention instability. I have been diligent in working on my ROM. At 5 months PO, I am still using a strap to bring my heel in, daily. Is it possible to go too far, and make the knee unstable? I’m sure every knee has a different ROM. At surgery, my surgeon bent my knee to 135 degrees. Without the strap, depending on the day, I can bend to 128-130 degrees. With the strap I can get it to 135-140 degrees. Am I causing any harm doing this. Is it okay to continue working with a strap.? I love my new knee, and don’t want to do anything to harm it!
Thanks again for your informative videos.

I would talk to your surgeon. We do get concerned with patients that push it to the extreme to get their heel to their butt with the fear of stretching out ligaments and making the knee a little 'loose' but I don't know of any study that looked at that or proved it. Anyone with greater than 120 is a rock star in the total knee world.

I’m at 8 months and still have pain all the time. My knee isn’t swollen, but feels warm compared to the other knee. I am in very good shape otherwise, and still walk/hike everyday at least 2 miles, along with lap swimming. My quads are strong. It feels like someone is grabbing my knee all the time. The back of my knee/leg hurts also. My doctor keeps saying, just wait. He says it is stable. I’m so frustrated because of my active lifestyle. No other doctor will see me until after a year. Any help would be so appreciated.

Two and a half weeks out from first TKR and dealing ok with pain finally, particularly excruciating shin bone pain at night, but what has been even more terrible is the number the opiods and acetaminophen have been doing on my colon and stomach! Couldn't keep anything down or up for two weeks with twonvisits to ER. Two changes from morphene to oxycodene, and now acetaminophen with codeine and all had anti nausea pills too, which I'm now just taking half pill because so scared will start throwing up again! Have to get other leg done but not sure I can go thru the drug problem again! DR rosen, any ideas on a better mix to try initially for the second one that might not cause as many issues? Can't seem to find anyone ever talking about that in a video! Your videos are very helpful and honest so appreciate them! Thx!

Great question. I'll put a topic on pain management on my to-do list. Every patient is different - what their pain level is like and how they respond. If I max out a patient on a 'multi-modal' pain regimen I have them start acetaminophen 1000 mg twice a day for two days before surgery, then 4 times a day for a week, then 3 x a day for a week then 2 x a day for a week then as needed. At the time of surgery we use a nerve block + general anesthesia. After the surgery the joint is injected with numbing medicine. They are put on an nsaid like celebrex twice a day for 3 days then one a day for 2 weeks then as needed. Some patients add in CBD daily as an adjunct. We then use oxycodone IR for breakthru pain only. In certain individuals I add in gabapentin 100 mg 3 x a day up to 300 mg 3 a day. I know that might sound confusing but hope it helps answer your question - its a good one.

@dr.adamrosen-totalkneeinfo2898 thx so much. I had the epidural and nerve block etc but the surgeon, nurses and even ER flummoxed at how badly and long the throwing up reaction, which started just a few hours post surgery went on for, two weeks, everything thrown back up and last days, diahrrea until finally change to acetaminophen with codeine plus anti nausea seemed to settle it, and still helping ok with breakthrough night pain mostly, and also, I think, with GI Specialist at ER also stopping celecoxib. Never want to go thru that again! Will keep your response for consideration by my surgeon for the next leg. Thank you. Your videos have been extremely helpful pre and post surgery!

I have a hamstring tendon that seems to be running/popping over the medial implant...I was given an injection to reduce inflammation and re-check in 6 wks....my surgeon said he's only seen this 3 times in his 30+ yrs as orthopedic surgeon....hoping a piece of cement is not floating in there or that he'll have to go in and shave the bone...I'm very concerned that I would have to start this healing process over again...I'm a very active 71 yr old horsewoman ..TKR was 8 mos ago...popping began approx 2 mins ago...

It's rare. I saw it twice in 20 years. Once went away with a cs injection once the tendon had to be released surgically.
