Hi John. Thanks for your compendium of techniques - very useful indeed. New Zealand physio here. Nice technique - I've used it a lot. I find I can get a fair way down the thoracic spine by modifying it - say to about T7 anyway. I use a plinth bolted to the floor - those portable massage tables are too unstable. Then buckle the sitting patient onto it with a seat belt around their pelvis. So they're thoroughly anchored. Then when leaning back, I can hook my fingers (one set of hooked fingers reinforcing the other) over each spinous process in turn, from C7 down to about T7. Because it's so stable, I can keep up a strong, sustained, specific traction of each vertebrae in turn. Say for about 30 seconds or until my fingers give out. I find this long sustained IV traction really useful for stretching tightened collagen around the tight joints. So they can stay freer better, not just tighten up again fast. You can add in more extension as part of the technique, of course.
Wanted to point out, your hands should be at T3 and T4 for the cervico thoracic translation otherwise your going to Flexion the T1 thru T3 when its extension that we want UNLESS its a straight back syndrome.

Would thoracic mobilisation be a procedure you would undertake for someone who has a spinal cord stimulator inserted in his epidural space - and having severe neck and shoulder issues?

You only have herniated disc in the middle cervical area and lower two lumbar so this technique is fine to do, regards JG

This is great what did you say though along the spine, a gulley? What is that?
Either side of the spine there is a room to fit the fingers so I call it a gulley - not an anatomical word but makes sense to me, regards JG
