Way back in the Stone Age when I was a flight medic, we didn’t have commercial tourniquets, so we used a BP cuff inflated to the point bleeding stopped. Now I carry CAT tourniquets in all of my medical bags.
Great informative video! keep 4 North American Rescue CAT torniquets on the rig (1 on each visor and 1 per First Aid Kit (2) plus a SOFTT torniquet on my person. All of them are easy to reach. Would love to see more videos like this. Keep up the great work Doc!
One of the best tourniquets I’ve ever used and haven’t been able to find since was the recon medical C.A.T style/clone, had an aluminum windless with a pin in it, and a hole in the strap so if your hands are slick you can easily grab and hook a finger in.
Doc i came here to the toob to look for just this.. after watching 4 other vids i watched yours.. by far the best and exactly what I was looking for. Thank you sincerely.. I do however wish you had some non am i zon links as i do not patron them after what they did with the whole sum er of most lee peace full stuff. Miss spelled on porpoises to thro off the old al gore rythem and com mints disappearing blues.

I haven't been able to break them...I guess the concern is that quality control might be lacking and maybe some inferior products make it to market...but I haven't seen that

Hi Doc, have you ever tried a ratcheting tourniquet? I like the idea because it seems to take the stress of winding, while wounded and possibly with bloody hands, out.
Nope - just bought two. Concerned about the "knock-offs" breaking or not getting circumferential restriction, but looked at a few other videos, and these seem to hold up well in demos. Just been out of the medical arena too long. Really appreciate the advice 👍
Great question, the tourniquet is meant as a bridge from the field to medical care, medical care is not an option then, knowing how to fix the injury is the only solution, but that is a whole lot of training!
yeah, so would have to loosen it periodically to stop muscle damage, and hope your interventions and natural blood clotting eventually work. @@DocWrangler
I am just getting into the california state militia. we are getting into a lot of in field stabilizing medical, i never expect to get close let alone into a fire fight. but in all my years as a truck driver. seeing a number of accidents happen. even a surviver of a very bad head on. ( chp officer could mot believe there was no one dead). other guy was doing 100 mph. i have never run into much need of a med kit, let alone a TQ.. but in my life i have had a couple trips in a ambulance. guess lucky for me when i have been hurt medical is easy to get to. and yes i have many a time out in the back country. far from everything. never to be found if the worst happened.
If you're short on cash Rhino Rescue actually makes quality ifaks. I bought a few on eBay because they were brand new and I got them for $20 each. Figured I'd just use them to refresh my training but after opening and checking one out I was pleasantly surprised and kept on and gave one to my friend
Hey, former SWAT medic and TCCC officer instructor here: don’t use a RATS. In testing, they don’t reliably stop arterial blood flow. Also, the width of the band is so tiny, you’ll get SIGNIFICANTLY increased pain and soft tissue damage underneath at the site of placement. I know increased pain doesn’t seem like a big deal, but it is when the guy loosens their TQ “just a little” so it doesn’t hurt as bad, and the bleeding just starts up again. 2 inches of width is the CoTCCC standard, and it’s actually for pretty good reasons. Take that advice from a random person on the internet as you will ;)
SWAT-T is potentially pretty good…as a disclaimer, I’ve trained with them a fair bit but not actually applied one to a patient. Biggest thing; they’re a bitch to apply to yourself. I can eventually get one around my leg, but I’ve never been able to fully appropriately apply one to my arm. On the plus side, they’re cheaper than a CAT or SOFT-T, smaller cube space and for wilderness applications are multi-function. I keep them in all my first-aid kits to apply to other people. I’d never use one at work (because it’s not Co-TCCC approved) or for self-application. But I keep them in my personal kits to apply to someone else, and in the MASCAL bag they lives in my trunk.
No TK4? It's the best EDC tourniquet. Low profile, effective, inexpensive, easy to self applicate, and easy to use on others. Safe for use on kids and even animals. I carry 1 on my and I also carry a SWAT-T and a pack of Cellox Gauze. The SWAT-T can be used as a second tourniquet or as a pressure dressing, sling, ect. The TK4 is for the need to stop bleeding NOW! role.
@@DocWrangler Well I used it in testing only but it stopped not only my distal pulses but everyone else's that it was applied too. So I have confidence in it as long as it is applied correctly. Which, is the single most important aspect of any tourniquet. Like James Yeager used to say, when applying a tourniquet. "apply high or die, and tight is right. And if you are not sure if it's tight enough then it's not.".
Wouldn’t purchase any TQ from Amazon though, the CAT ones in particular have been cloned and some of those clones that have been tested have broken when being applied. To me $30 is a very small price to pay for the insurance they provide and NAR ships fairly quickly
I've seen people compare the CAT to the Rhino tourniquets and the Rhino seems to be slightly better constructed and easier to tighten than the CAT. By the way, Rhino tourniquets are not made in China, they are made in Germany and the way I understand it they are also used by German military forces.
Just a headsup, the Rhino Rescue TQs are made in Wuxi City, Jiangsu Province, China according to the manual that comes with the TQs. Definitely not Germany - a production in Germany would double, if not triple the price.

I have to critique your demonstration a little bit. Most of the time when you applied a TQ you did it wrong. You have to put it as tight as possible before twisting. That's why the first TQ is pinching you. Also the way you folded the remaining strap isn't very good either. It's very easy to come undone. When you applied it to the leg it was way too low. It's suppose to be "high and tight". Maybe you just don't know the proper way of doing it or you assume that people will figure out how to do it but if you're going to show how to put on a TQ you might as well do it the proper way. Right now you risk of confusing people that don't actually know and teach them the wrong way. Remember that people put a lot more trust in you because of your title as a doctor. Also telling people to buy TQ from Amazon isn't very smart either because there are a lot of counterfit TQs there with questionable quality.

There are locations that a tourniquet can’t compress (such as over a joint), but other than avoiding those areas, the “high and tight” approach may not always be the right approach. I would rather be compressing a site a few inches above the injury. Yeah if my foot gets bitten off by a griz or an accident I'm not putting the tourniquet on my thigh.. I wanna keep my knee and as much of my leg below it as I can..

Secondly, you could die from tourniquet-syndrome when (I believe but I could be wrong here) toxic substances build up in front of the blocked off veins, and once the bloodflow is regained (when the tq is taken off), these toxic substances can lead to death, but as I said I'm not well versed in the details so I could be wrong here

Not going to disagree…I haven’t seen the failures on the $10 models, but I hear that out there.

What about tourniquets that are on Amazon that are obvious fakes and made so cheaply that the windless bends under pressure or even breaks?
I didnt find any like that…but sounds bad!
