    Today I’m thrilled to welcome my dear friend, Helen Hall back to the podcast. Helen is a movement therapist, a running coach, a pain expert and one of the best coaches in any modality that I have ever come across. She has had a lifelong passion for analysing posture and movement, and her clients include elite athletes, whether they be runners, cyclists or premier league footballers, but also everyday folk who simply want to walk or run pain-free.
    She combines objective clarity from the most advanced motion analysis technology in the world, with 46 years of visual experience and study in the field, to seek out the root causes of chronic pain and injury, that often seem resistant to standard treatment protocols.
    In order to help more people than those able to visit her in person, she first shared her movement philosophy in her wonderful book ‘Even With Your Shoes On’. Since then, she has gone on to create a series of online videos to help more people move pain-free. Helen has also launched an online course called ‘PFM Pilot’ which is getting fantastic reviews - it is aimed at both busy professionals working in the field of movement, pain and injury, who can now have access to Helen’s incredible wisdom but also amateurs who are keen to learn more and help themselves.
    Helen first came on my podcast on Episode 216 back in November 2021 and many of you got in touch to say how helpful the tools shared in that episode were.

      Hi Dr! Firstly thanks for the wonderful material on youtube. when it comes to running, i used to run/jog almost everyday until 3 years back i had a tendon/ligament tear just below and around left knee cap. When i was in india, i ran on cricket playing fileds which are of mud where i was able to jog without any issues. Here in Barcelona, there´s only asphalt roads, which brings back the scare of my ligament/tendon tear, its like a rope with its threads coming off feeling. Collagen helps, but i cant digest it. Is there any hope for me to be able to jog/run again?

    I’m 70, been running from 19. About later 50’s started medium to faster jogging. Been jogging on both knees each with a torn meniscus that only bother me once in a while. I’m careful where I jog. If I get a sore knee a few days it will be ok again for a very long time usually. I can’t imagine living without a good outside jog every day or every other day sometimes little less. It’s a huge emotional and physical outlet for me. I love it. Every day I do it I am fully appreciative that I’m doing it & I still can.🙄

      Are you underweight? Do you think you have lost muscles? jogging tend to do both.

      @bobwa399 I've been a runner for 42 years, but I also walk, ride a bike, and do resistance training. I am an ideal body weight for my height and frame. I have muscle definition in my arms and legs, and I do not have any belly fat. At age 54, my waist still measures 24 inches. Everyone is different, but running has treated me very well throughout my life. ❤️

    RECAP:
    -Running is not bad for your knees, but it depends on how you run. It might be that the way you walk is harming your knees but you notice it when your run. So we have to ask why our knee hurts when we run. It could be that something else related to other joints in our body is affecting our knee. The knee might be overloaded at times even by the weight of our head.
    - For every inch further forward we place our head we are adding the weight of another head. The angle affects the amount of weight we are putting on; every 30 degrees add 18 kilos.
    -The way something in our body moves has an impact in the other parts too.
    - Ego interfers in the possibility that people reach their maximal potential.
    -When running, walking helps the muscles recover. If we only run the muscles can become tired and prone to injuries. Also the body likes to switch and do something different. Also the recovery time after a career is shorter when running-walking. So running long distances can be done in a calm way that you can enjoy :)
    -Many of us subscribe to an invisible system of ideas that keep us stuck, but when we stop doing that a lot of possibilities will appear.
    -To know where our head is positioned we can lay down with our head over something that slips like a tissue and start moving the head as if saying ¨yes¨.
    -The coordinated, organized movement is the result of the harmonious cooperation between the shoulder girdle and the pelvic girdle.
    -Some people when walking do not move the shoulder girdle and this can have an effect on the pelvic girdle, causing pain. Therefore, when running it can hurt but it steams from the way they walk.
    -We can give the body and brain input with our skin about the possition of our body.
    -When we change our visual field to a peripheral visual field and don't look at the ground while running it improves the running.
    -Foot wiping is a way to wakeup our feet and it is important because it desensitizes them. This influences our balance an makes our feet more functional which can even affect the way our shoulder girdle moves.
    -Ticklish feet are hypersensitive, meaning they do not like to be in touch with the ground which is not optimal. Therefore foot wiping can help desensitize them. A runner can improve their speed and potentially go from 5 hours 30 minutes to 5 hours, as their feet will work better.
    -But hyposensitive feet are also not good because they dont transmit the information.
    -Many cultures arount the world don't have problems with their feet (like plantarfascitis) and don't use padded shoes; some dont even use shoes. In some cultures kids don't wear shoes for sports until they are 14 years old.
    -The joints of the foot need to move to activate the muscles, so don't wear tight socks.
    -Minimalist shoes have multiple benefits as they help to be more in touch with the ground, and improves foot strength..
    -The use of shoes is questionable; why do we use them if our ancestors didn't?
    -We don't have to assume that we are not able to move without pain. We have to first pay attention to where our head is and what our feet are doing.

      I’ve had ultra sound and X-rays or what not. They show torn meniscuses 😏I also do skipping and pretty quick backwards walking. I do run up a longer steep hill at the park 3x which gives me my HIIT workout. My hearts really beating a drum when I get to the top. I know it’s a little risky for my knees but it is so exhilarating and makes me feel so good.

    This was a fantastic video! I have been a runner for 42 years, and just last year I started incorporating walks into my runs. I love it! I usually go out for 5-6 miles and alternate between walking and running. I wasn't sure how common this was, and then I watched this video and learned how beneficial it is to the body. I hope to continue this for the rest of my life. ❤

    I’m now 75, was a runner, torn meniscus, right total knee replacement in 2022 and I walk an hour a day. I want to make sure I’m walking correctly. Please guide me to walking correctly. 👍👏❤️

    Been a daily runner since my twenties. Now fifty. My form dramatically changed during a time I worked as a valet at the Ritz-Carlton. I thought I had good form then, but at one point I developed shin splints. The pain was pretty intense but I refused to quit, however I started toe striking EVEN more. (Ridiculously so... in my mind it felt like I was tiptoeing) Within a couple weeks the pain was gone. Form is everything!
    BTW, a running coach who was staying at the Ritz-Carlton saw me running and commented to another valet, "that's perfect form!"

    Wow, this information is so important. And yes I have knee pain, but I also have been teaching in the past range of motion to senior citizens. And I am just now a senior citizen, looking into the mirror and realizing how head forward I am. This is incredible information.

    Helen's advice to think about where my head is has helped me to ride better on the motorcycle; and when out walking; and snowshoeing. I feel stronger and go longer. And, pole walking has been my best friend for the last two years. I love my poles. Thank you for all these great conversations Dr.C. and and guests. I feel better and want to live longer.omu

    I’ve been a Treadmill runner for years. Never had any injuries vs my friends who are road runners. Roads are intentionally sloped for water drainage, full of bumps cracks, constant uneven surfaces etc. The softness and perfect even level of the treadmill machine floor I think is a major factor on landing and avoiding injury.

    I'm 54. I run. I have zero issues with my knees but I also ensure I have perfect form when I run. I also stretch out before and after every run....EVERY time. IMO the people who demonize running are just too lazy to want to run themselves. Sorry but that's honestly how I feel. I am SOOOO tired of people demonizing running. I also do long (4+ mile) walks but there is no way I am going to stop running and it has nothing to do with ego and everything about how it helps me burn fat/lose weight as well as for improved mental health.

      @@binyominezekiel-du7bw Face facing forward, not down at the ground, shoulders relaxed and down, not shrugging them (these are the 2 most common things runners do). Arms going out naturally, not crossing in front of your body.
      I got covid last month for the first time ever (and I work as an RN in healthcare). Other than that it's rare I get sick. I'm sure that's because of my diet more than my exercising. I also get 8 hrs of sleep every night. SO important. Everything starts with that actually. I lost 45 lbs last year.....40 of it within 7 months.

    Excellent discussion. thanks. Whenever I get injured after pushing myself too much, I have this inner voice before that quietly says: "Are you sure you should be doing this? And then I ignore it. Better to make it a habit and plan for it. I am going to pay more attention to posture and keep committed to the run/walk strategy. Currently have a niggle in my low-back that could go at any time. Not worth challenging it but rather take time out to figure out the root cause.

    I have also noticed that activating my core allows me to put my head more in its correct position while doing the floor lying exercise.

    If you're overweight its better to walk and lose weight first. Before running you should have good flexibility helped by squats and leg raises (front, back and sideways). When starting to run go really slow to build strength. Regular runners have thicker cartilages and stronger joints, better circulation, stronger heart and lungs, helps depression etc.

      I still don't have the power to run well. My heart rate is too high when I run, generally. I try to do most of my exercise in Zone 2 or lower. So I stick to walking, ellipticals, and bikes.
      The only people that can run really well seem to have relatively low body weight. Some marathon runners look almost emaciated.

    Helen needs some serious help with her online presence (TH-cam and Instagram). A wealth of knowledge here that is difficult for most people to grasp without being able to see it! The video mentioned in this podcast doesn’t seem to exist and the other videos which accompany the book are hidden behind a rather expensive paywall with no preview available to assess the quality.

      Get a bristly door Matt and wipe your feet barefoot. 2 minutes each evening, I do it before bed.. seems to help my arthritis so far

    I was a runner for over twenty years. I had a total knee replacement on my left knee back in 2011. My right knee it is ok. Not great but I can live with it. I’m 69 years old and I keep my fingers crossed for my right knee. I miss running. But now I walk four miles a day. I had switched to race walking after my knee surgery and I have slowed that down too. I do fast walking and I feel good. My weight is good and I’m on no medications. I hope to stay away from any medication and live a healthy life for what ever time I have left on this beautiful planet

      Daily running for more than 20 or 30 minutes is not for me and I would not recommend it to most people to run daily for 50 or 60 minutes at a medium speed. I would get exhausted and my body would not get enough regeneration. But may be I am of average or less than average physical fitness.

      @@freeeurope9849 I hear you. I was running eight miles four days a week. Took three days in between to rest up. Yes running does take its toll on the body. I was also bodybuilding at this time. Man I was in my early thirties and in the best shape of my life but in the end it did its damage and it’s the same thing with anyone who is an athlete person. Dancers bodybuilders professional athletes. We beat up our bodies and when we get older the damage shows up

    Running on concrete ,long term ,cannot be good for joints . Running on grass ,sand ,soil etc ,seems a healthier choice .Much will relate to posture and form ,for some , knee pain could be related to poor diet ,lack of minerals etc Not forgetting supportive footwear . There is so much to consider !

    Run/walk/run strategy introduced by Jeff Galloway. I have been practicing it for 2 years already? I outrun some people during my 5k Xmas run doing 90/30 seconds intervals. Plus the recovery that is literally on spot... Of course I add stretching and strength sessions but I do feel a difference as I used to run constantly...

    I could never run between the age of 15 to 35 due to severe knee paid - but then I saw a physio....
    Run with correct form and your knees will be fine. But....getting to that good form is complicated...I got there with help from a Physio who specialised in triathlon and it took a lot of work.
    15 years later, 10 marathons and 2 Ironman events and 100's of other races. My knees are great in my 50s - never had an issue with my knees again.

    High carb, low protein won't help with injury and runners are all about the carbs! Most also run too fast too often.

    This is so good! I'm running my first marathon this year (age 59 lady) and a run-walk strategy sounds perfect. Is there a watch run/computer type thing where I can easily set an alarm for 5 mins run, 1 min walk. I bet there is! I just killed mine scuba diving so I need a new one anyway.

      I have a GYMBOSS which is a little thing you clip to yourself - you can set it to any intervals you want and it beeps (was quite inexpensive too)

    I injured my knees from running when I first started doing it 20 years ago. It was due to heel striking; not knowing how to use my calve muscles correctly. Once I developed the calves and shins properly, and learned the correct form, I never had knee issues again.

    As one of 8 kids, as a young child we only had 1 pair of shoes that were saved for school and church. We took them off as soon as we got home from school to extend their wearability. It was bare feet at all other times. As a result I have wide long feet, high arches and very mobile toes 2:26:09

    As much as I enjoyed jogging I am now walking 6 miles in one hour and forty minutes..I feel great afterward and more energetic throughout the day. Add in stretching and yoga either before or afterwards to maximize your walking benefits.

    I started running at 40 and I have done 35 full Marathons, 8 Ultra Marathons (50k to 100k distances) and 30 Half Marathons. I've not had ONE injury ever. I'm now 57 years old. I'm going to run a Miler (161k) in February for turning 58.

    I eat. Carnivore diet and all my joint pains have gone including my one of my knees. I’m 63 and have now started playing badminton and running after a lot of years not doing it because of bad joints. So maybe they should look at diet. When I stopped sugar eggs n my diet first my plantar fasciitis that had suffered from for years disappeared. Food s probably the cause?

      I've tried to find more information on foot wiping as well and didn't find it on Helen's TH-cam page or her website. Dr. Chatterjee's Instagram has a recent video of him demonstrating foot wiping though.

      Seemed like the video that Helen mentioned at this point might have illustrated the biomechanics (and benefits) of foot strengthening and stabilization, and the subsequent enhancements to posture and alignment, and mitigation of joint pain

    Not everyone listening to this runs a marathon, not even a half one. Not everyone is fast. Not everyone is on a high or the most recommended level on various things discussed here. I am not completely unfit but I would like you to talk more also to average or less than average people on the various topics. After all, I guess, the reality is this is most people.

    Just ran my very first 26.2 mile marathon on the trails of Catalina Island off of California at 57 years old.
    I went vegan 18 months ago and lost 80 lbs. Inflammation and two lower back bulged discs pain started to ease. I started to lighted jog and after reading Born To Run. Changed my life!
    I have connected with running in a BIG WAY. I have also, just signed up for a 50k ultra marathon in Tahoe in July 2024.

    No it’s not a myth. I spent six months under a team at guys after a meniscus tear and two operations. I belonged to a running club at the age of 11 and my knee gave way in my mid thirties. It’s never been the same some 14 years on. Knees are notoriously difficult once injury occurs. Running is very hard on the skeleton full stop. Easy to fit theories around populations they apply to and ignore the grim reality. I like this channel but knowing what I went through and the medical experts who got me back, it’s frustrating to hear.

      Kind of agree. I say diet is important - stay away from seed oils, eat real foods, make bone broth and fermented foods. My joints are good, my muscles get some cramps but I can live with that. Running is such a great thing to do when you get older.

    Sorry to say old generation don’t have ideas about posture, body movements, hump scoliosis how it affects the mechanics of walking, running, jogging. We needs more education from checking our hips and back at birth moving forward. A lot of not knowing. We needs to stand in front of a long mirror in the morning to assemble the head, neck , shoulders back down relaxed . Core glutes then were the feet on ground? As we get older balance exercises , preventing falls and humps becoming mandatory. Thanks for sharing this information with us.👍🙏👌🌹🌷🧘‍♀️

    I've also always thought that Marathon's should be time dependent. Elites doing 2-2.5 hours. I don't mind running for 2-2.5 hours but I'm not doing 26 miles! It simply doesn't fit in with my other athletic goals

    I walk an hour a day and suffer for several hours after that from the dull but significant ache in my knees. Running? Even when younger, after a run, I would be lucky if I could walk for the next two or three days. Not buying it.

    I couldn’t possbly listen for that long as far too stretched out so may have missed some. For posture, neck/head and free movement, try Alexander technique, although others things like yoga and taichi will also help. But, importantly ditch cushioned shoes and raised heels. Go barefoot when you can, and minimalist when you can’t. Barefoot is natural and all those nerves will help to train you to run with proper gait, upright, landing on your forefoot underneath your centre of gravity which engages full suspension, not with your heel landing in front with straighter leg, which acts as a break and maximises the landing forces, impacting knees and hips.

    What about hiking? I can run and walk pain free but hiking is painful after about 9-10 miles of hardcore knee pounding. MRI shows bone on bone. Get gel injections in my left knee so I can hike pain free. Have to constantly look down so I don’t trip over rocks and roots. Rock scrambles downhill are hard on my knee.

      @@shoutatthesky i use poles and try to walk backwards. Especially down the rock scrambles. I was told by my PT this may help. I zig zag from my left to my right side. Never straight down. The ADKS are know to the hiking community as some of the worst trails for your body.
