I had this code and was fighting it for months. Everything you say is true. First thing I did was change the pump, that didn’t fix it, then I took the def tank off and looked at inlet screen, it was dirty so I replaced the inlet screen. Couple days later I get the code again, it was on the injection unit this time. It was not a bad injection unit but the gasket around the injection unit was allowing air to mix just a little with the DEF and was crystallize right at the spot the injector was spraying at. It was clogging it up. I changed the gasket out and that finally fixed all my problems
Also, before you change the pump make sure to check the ends of all the blue accordion tubes that send and return DEF. sometimes those little white collar clips will crack and bleed a little bit of DEF and you need to replace the tube
One way you can kind of tell if the problem is on the side of the DEF tank or on the side of the 1 box (SCR box) is by unclipping the inlet line on the DEF pump and to see if had a steady flow of DEF or if it’s dry or barely coming out. Do this with the truck off. If the DEF is not coming out or just barely (a few drops) out of the inlet side of the pump then it’s a problem in the def tank (check the inlet screen) but if the flow is good to the pump then I would just the injection unit. Take the steps off the passenger side, undo the 2 def line and unplug the injection unit take off the 3 Etorx bolts and slide the unit out of the hole. You can pretty much tell right away if there’s a problem there because there will be a lot of white crystallized DEF in the area where the def is being sprayed or in the actual holes on the injector.
This is just how I did it. There are better ways to diagnose this issue, but this is all stuff you can do by yourself without a scan tool. I have had to do these things in Yukon Canada on the side of the road, not fun but at least I figured it out enough to get me and my trailer home.

I'm also a sound engineer for TV shows to the west of you and when you play your radio it's hard to hear some of your very important words. What's the best way to buy the Freightliner Diagnostic Software for Windoze? Do you have to buy it from the stealer or are there other good sources for the software? I'm going to buy two used Freightliners for short haul trucks I need and want to be sure what the real problems are with them. I'm seeing a lot of low DEF and Smoke Swirl Lights on the dash and the RPM only goes to 1500 on the DD15. Derated for sure. Muchas Gracias!

Also to anyone that is wondering if it’s a problem with the DEF pump, I would pay the 100$-150$ diagnostic fee to a experienced mechanic just to confirm if it is a bad DEF pump. Much cheaper than buying a new pump. This problem can be as simple as an inlet screen being dirty or a crack in the line. It’s not always a DEF pump problem. These pumps should last about 200-250k miles from the guys I talk to that work on the GHG14 emissions systems
I have 2016 FRTL whenever I put DEF after 100 200 miles later I gets the check engine light and I have to pull over do Region , went to dealer many times spend so much money but the problem is still there.. any tips?
Hi Francisco, pls advise. I dont have any fault codes. But my def pressure is low 7 psi almost all the time. Replaced def pump, one of the def lines from pump to the def tank cuz i had fault code def heater line # 5, then I replaced it. But i still have this issue. When its 20 F outside i noticed white smoke coming from exhaust pipe on a cold start. And I hear weird cyclic noise on a low rpm sometimes especially when its cold outside (my def pressure 160 psi at that time).
Replace DEF pump and doser ??? if no external leak or any contamination , air bubble in the line , FACTORY procedure is to replace the RS supply kit ( little filter kit for the pump and doser ) wich fix the problem 99% of the time

Can I keep my truck on idle with a bad def pump, unfortunately I'm stuck at the truck stop till pump arrives?

You should clean up or replace DEF metering valve.

Try to use the DEF from the plastic containers, not from the pumps. They are nasty and mostly contaminated.
Are there a fuse or reley for the def pump?
