Where do you position the shoulder pads when the baby is 12lbs+ ? Do they go on the top of the shoulders or on the front of the shoulder?

They should always go right on top of the shoulders. If they go in front of the shoulders, then it won't allow chest clip to be at armpit level.

Newborn seems to have difficulty breathing (chin on chest) when buckled in properly. What can I be missing?

I can't give an exact answer without seeing the little one in the carseat. But there are a couple of other things you can check if you've followed all the steps to buckling the child in the car seat properly.
1. The installation. The base or baseless installation have a certain angle they need to be at. Make sure you are following all installation rules.
2. No extra pillows, headrests, blankets, or any other 3rd party items should be added to Car Seat or placed under the child while they are in the car seat.
3. Removing shoulder pads. Sometimes, the pads can be too bulky for newborn or smaller children. You can sometimes get a better fit by removing them. They are optional.
Try these out. If you're still having trouble with the car seat and getting a proper fit, you should definitely reach out to Nuna for troubleshooting.
