I have been helping care for my grandmother with dementia for the last 6 years - from the Early Stage to currently Stage 6 - and I know that thousand mile stare very well. He shows all the signs of dementia. I have had to learn a lot about it, and my grandmother isn't the first in our family with it - she's one of many... it seems to always affect the women, but none of the men. Biden looks to me as if he's in the middle stages. He's probably less than 3 years from being in the late stages. Anyways, point is, I know it when I see it... any dementia caregiver will probably have the same opinion. The next few years, he will probably rapidly decline to a point where it cannot be hidden any longer.
