@4:23 "The entire box was an image".
That way, the human recipient can read it, but the spam filter sees no text to evaluate it. Yes, the spam filter can see and evaluate the text in the message's header. But to the spam filter, the body of the message had no text to evaluate.
I avoid getting scammed, because when an e-mail message suddenly entices me; when it gives me a knee-jerk reaction to click on something, well... I trained myself to pause, as those are social engineering tactics that scammers use to have people fall for their scams.
When good news falls out of the sky, even something as simple as being granted access to feature or document, etc, I let the pinch of dopamine subside and wait for my self-preservation senses to regain control of my actions.

I have my e-mail filters set up to perform two basic primary tests on incoming e-mail: first, is one of my e-mail addresses in the TO field or the CC field? If not, straight into the trash. Second, is one of my e-mail addresses in the FROM field? If so, straight into the trash. I have additional filters that test whether or not an e-mail is really from PayPal, Amazon, E-Bay, or my bank, but the first two are the most important and the most effective.

That first filter wouldn’t work for my email because my email address IS in the “To” field. I have to discern in other ways, most of the spam/phishing is obvious to me.

If you use a password manager like Chrome or Edge, then when you click on the image, you would be taken to a fake MS login page. The password manager would not recognize the URL, and would not fill in your authentication. Even if you opened the Password Manager and tried to find the authentication, by then it should be obvious that the URL is bogus. Using a password manager can tell you a lot, and at least prevent mindless clicking and logging in. Because when using a password manager, I at least, allow it to suggest a strong, random password, which I never bother to remember. So I couldn't type it in from memory, because I don't have a clue what it is.

Just be aware of one key danger with that approach. An email source can be hijacked. If you implicitly trust those emails, you are at risk--even if the email says "I Love You" with a link (see history on that). :)
bottom line: do not trust links or attachments you are not expecting or that do not have proper context. Something like "this is a funny video" is not context and you need to let the sender know you have not viewed it and to give more information if you want others to view. Sending silly emails with attachments with no context is simply de-sensitizing friends to be susspetible.
