We tested the absolute hell out of this, lots of medics tested it, and we have never seen something like this happen - can you show the full video? It starts before the actual application - did you pull the strap tight first? You can certainly twist the windlass either way, we have tested it many times either way. I will be testing them more, trying to replicate what you have shown here, but the more information you can share, the more I can improve it. If you truly believe that this has a chance of getting people killed, please help me understand what exactly happened and how to improve it. With this video cut how it is, I can't see how you were able to get to the failure. I made this TQ because I wanted to save lives, not make a bunch of money. If it has a critical failure, I need to know.

Obviously you managed to get it all geeked up somehow, but it seems like your application was the exception to the rule. Especially seeing all the other responses similar to mine, where no one else had this happen.
I tried to replicate this with three different ETQs (one wide) and was not able to do it. All seemed to work well. To mee this seems like CAT tourniquets that have been trained too much. If that truly was your first try with it, I think this have been faulty one. Have you tried to do it with different one?

It’s hard for me to put confidence in the ETQ with other options available. I don’t know why I would use it or purchase it over what I’ve seen do better through far worse.

It doesn’t appear you applied it correctly before tightening the windlass.
