Low Back Pain FULL Physical Therapy Evaluation

  • There are 4 parts to a physical therapy evaluation:
    1. subjective 0:00
    2. objective 9:30
    3. treatment 25:19
    4. home exercise program/education 28:50
    In this video, the patient complains of low back pain. You can see how the PT starts by asking him to describe his back pain.
    The subjective physical therapy exam is all about gathering relevant information to help guide testing for the objective exam. After the subjective exam, the PT should have a good idea of what the patient's physical therapy diagnosis is.
    *COULD YOU GUYS GUESS WHAT HIS DIAGNOSIS IS?!?!*
    The objective physical therapy exam is to gather data to rule in or rule out physical therapy diagnosis' and to find impairments that could contribute to the patient's symptoms. This is where you find out the source tissue of the patient's pain.
    The physical therapy treatment is when the PT uses a specific intervention that is appropriate for the patient at their specific time in tissue healing. Some intervention categories include, but are not limited to: manual therapy, therapeutic activity (therex), neuromuscular education, modalities, soft tissue massage, joint mobilization, etc
    Home exercise program/education is when the PT gives "homework" to the patient. This is where the PT goes through recommended exercises and teaches the patient how to properly perform them with good technique. This is also when the PT educates the patient on what is going on, how long the healing process will take, what the patient could do to help themselves, and what the plan is going forward with physical therapy.

    I'm a HHPTA and had LBP the past week an a half after working with a particular patient. As you could imagine, it's difficult to know off the back pathology is causing the pain. This eval was basically to the T of my pain location, and the therex after 1 set of each exercises decreased my LBP from 6 to a 2. Good stuff man. Enjoy the rehab train. It's definitely a workflow I will do well into my 60's.

    Orthopedic sent me to physical therapy and she had me bend to the front, backwards and both sides. She told Orthopedic I am more flexible than her and she gave me some at home exercises. I am doing them more than twice a day sometimes while laying bed, but this is routine. I remember when I first moved in with my bf, I did not realize I woke him up but found him sitting up looking at me. I was in the yoga shouder stand. He told everyone about that one. But I am doing the exercises the PT gave me and the Orthopedic told me to keep on doing them until I go get my MRI. Either a set of nerves or set of muscles on each side of my back not firing according to the EMG results.

    sharp means nerves might get pressed cuz of minor dislocation of lumbar discs, looks like L4-L5

    Really awesome video. I love these case studies. I just have a couple questions.
    1. Is there a reason for why you didn’t check his FABER/ or Fadir test.
    2. For his home exercises I saw that you you gave him a cat/ camel. From my knowledge it only stretches the annulus fibers of the low back. Is this a short term fix, or will it carry over for the long term.
    3. Final question. Would the McGill big 3 be used later in the treatment process or is it not appropriate for this client.

    Hello sir teh diagnosis i feel is rt lumbosacral strain with flexion bias..pls correct me if i am wrong...reason being thoracis dec rom, weak core, tight hip flexor n weak hip extensor ..n flexion relief pain n extension increases pain

      I definitely can! I am posting part 2 on Monday (objective exam) that talks about it a little more, but i will definitely be making a video on that subject in the near future.
      But in short, there are special tests or a cluster of special tests that help you either rule in or rule out certain diagnosis. And you perform these special tests in the objective exam (part 2).
      Hopefully after watching part 2, you will have a better understanding of what that means. But if you dont, dont be afraid to ask!

    I have also a back pain but its left side middle of back and its sharp pain cant even move and sit turn its hurt alots 😔😔its worse what can be done to cure this

      I’m sorry to hear about your back pain Sanjiv. It seems like you’ve injured it to a point where it’s affecting a big part of your life. How did you hurt it? What movements irritate it the most? If your pain is constant and you can’t even sit or turn, I would try to find a position that irritates it the least first. You have to control the pain/inflammation first. Have you considered sitting with a pillow behind your lower back?

      And it really helped

    Way too many questions in the beginning focused way too much on the pain instead of just getting to the treatment.... A lot of the questions were superfluous... Unnecessary... Dribble... But that's how students are trained... In reality you just get right to the basics.. and go!
