I dont understand why the ca2+ concetration increases when you re inhibting the camp concentration. when we regard the alpha 2 receptor, we see a decreasing concentration of ca2+ in order to inhibiting the adenylylcyclase. So why is the ca2+ concentration increasing, while camp decreases?
Decreasing heart strain without compromising circulation should be essential in CHF. So maybe an alternative vasodilator, something like muscarinic agonists? If contraindicated because of CHF I'm guessing ventilation or hyperbaric therapy or antithrombotics could help a little. In emergencies I'm curious whether sacrificing limbs ever be considered the lesser evil?

In CHF, you try to minimize the use of drugs that damage the kidney (e.g. NSAIDs) because the kidney is already in bad shape in cases of CHF...Cilostazol is renally excreted. 2. Cilostazol causes tachycardia (not for the faint-hearted; I mean CHF) 3. It’s clear that it reduces survival in CHF whether I have an explanation or not :)
