    I was shadowing a dentist yesterday, and he felt comfortable enough to let me assist him for a moment while his assistant was with another patient. It was cool and very awkward at the same time, so now I'm going down a rabbit hole of DA tips and techniques. Thank you for your videos!!

    I’m a new DA learning on the job- it’s my second week and I’m def struggling with the hi-vac, the saliva ejector is so much easier but I know I need to use both 😂

      You need to use the hi vac at all times. They can retract soft tissues and a patients tongue and cheeks at the same time providing aspiration. Forget about salivary ejectors. Learn to use wide bore aspiration. Once you've mastered wide bore you won't look back. Good luck.

    Yeah I think I need to talk to my doctor and ask him for mouth mirrors. I did 2 work interviews and in both of them none of their DA’s or doctors used mouth mirrors with HVE. Having a mouth mirror on hand seems super important and helpful. I used the HVE to help retract a patients tongue and suction at the same time and the pressure cause a little red bulb to form under their tongue. Had I been given a mouth mirror that would’ve never happened.

    Great video!! I’ve been a DA for 16 years and just started training new DA’s (mainly externs), they always ask about suctioning and it’s harder to explain to actually see what I’m talking about. I’ll be sending this video to my newbies 😊

    Have you ever considered a video or maybe just a small video topic regarding proper posture while working chair side? I'm a 4 year CDA and often experience back pain myself is why I ask. I hear I'm not the only one dealing with such issues

    I've been assistant for over four years now and you are absolutely right about the saliva ejector being too flexible to retract with. Unfortunately I have a micromanaging boss that keeps making me use it like a retractor and blames it on the brand of ejector we buy. I had to tell her it's not the suctions but the adaptors don't grip the saliva ejectors very well AND SEs weren't made to retract. She's having me buy new adaptors next order 🙄 anybody else hate a wanna be know it all dentist?

      @@DentalVibes I would still use what is best for you and your patient. Wide bore are second to none. Gets the job done. Salivary ejectors cannot provide protractation as they are too soft.

    Sometimes a tongue depressor works better than a mirror to retract those tongues that push back on you. Because when the mirror gets wet with a tongue that doesn't stay still it starts slipping and sliding, and then your hand will hurt. But since the tongue depressor is made of wood it will kind of dry the area of the tongue it's resting on and kind of stick to it, so it makes it easier for you to control the tongue without the pressure on your hand and arm.

    One tip that really helps me out with the HVE is bend the tip of it. That way it's more flexible.

    I have a tip! When assisting treatment on a tooth on the LR ~#28-#31 sometimes I like use both saliva ejector and HVE to prevent the tongue from getting sucked up by the hve but also providing more suction with the saliva ejector.

    Just keep in mind that although you can't retract with the flexible section, it's a good go-to for hygiene assistance when a dentist makes a bite adjustment after an exam so they're just going in with a high speed for like 5 minutes max and not a full-on procedure, so the patient is not numb. Anytime the patient is not numb, be extremely careful where you're putting the large suction. You never want to stab the gums with that- learning from others' mistakes will always will always save you a good deal of frustration and your patients a good deal of pain.

    So adorable over a decade - lol
    My aunt just had her 43 yr anniversary of being a Dental Assistant. She remembers practicing without gloves before Aids became in play.
    She says they used to hand dipped their x-rays too.
    I was a Dental Assistant for about 4 years, yet it was not for me. I absolutely had no interest in becoming a Dental Hygienist either.
    I am a Pharmacist now..good luck ladies.

    The upper left has been soooo difficult for me to get. I keep trying to retract with my HVE and sucking the gums and cheeks just like you said. I will try the cotton roll technique! Thanks so much! I do have a question.. I’m always scared to put down my air water bc I know I need to rinse and dry the tooth but I can’t retract with my air water. I want to use a mirror bc I feel I would have more control with retracting but again.. I worry about putting my air water down. Any insight?

    As many new assistants I am also struggling with the HVE. but the problem I’ve been having is when I’m going back into the mouth to suction, the HVE is so big that the patients open the back of their throat more before I can get the water and it just keeps going further down and I don’t want to gag them trying to reach it. I am very gentle but maybe not quick enough? Any tips would be greatly appreciated!

    i have been struggling w/ suction because my offices assistants do not get to utilize mirrors to retract. we’re expected to use the hve to retract and of course it sucks up the cheek and tongue, but im being told that it’s me being unskilled that is the reason. 🙃 i definitely will be trying to add extra mirrors to our restorative packs.

    My office doesn’t provide a mirror on the assistant side in our cassettes, I’ve been using the AW to move the tongue out the way. Would that work? We have extra instruments in packs on the side so I might just add it to my side for when I assist.

    We usually don’t use a mirror to assist with our suctioning, and the Dr. I work for said to not use both suctions at the same time. Anymore suggestions ?

    with my experience i usually didn’t have a mirror myself. to retract i would use my air water, is that acceptable too?

I have a mock interview tomorrow and I am super nervous because this will be my first time working with a real patient and as a chairside DA!     The patients aren’t supposed to close on suctions because of back flow … that’s what I was told at an infection control class

    Very helpful! Just an idea, when watching the video up close, having both sets of hands wearing the same color gloves confused me a tiny bit at times when trying to figure out what the dentist was doing/holding vs the assistant. Different colored gloves on the dentist might be helpful for us newbies 😊

      The dentist is holding the hand piece. DA's hardly ever use the hand piece. DA is holding the suction.

    My doctor almost never uses the water from this hand piece so I just do my best with my air water to retract and the HVE lol! 😅

    thank you for this! I suctioned up the patients cotton round on my first day. It was TERRIBLY embarrassing! It was placed lingually at tooth #19 and while the dentist was working on the occlusal surface I tried to rest my HVE right above the cotton round and it didn't work out..
    So, when I was observing the lead dental assistant work with the dentist she had her HVE in one hand and the air/water in the other so when he would take a break she would rinse the tooth and suction. She was using the air water also as a retraction device. However I couldn't get the hang of using the air water to retract well and the dentist was using the only mirror we had available in the room. So I guess the question is, if I brought an extra mirror for myself to use then would I have enough time when the dentist pauses for a break to put my mirror being used for retraction down and grab the air water to rinse? I feel like they expect everything to move so fast that I don't want to slow the process down. Also, while she was retracting with the air/water and suctioning with the HVE water was pooling very badly in the back of the throat.. how should this have been handled? quickly suction it with the HVE she already had in her hand? I feel super nervous using the HVE in the back of the mouth like that but would that be the best way to go about it? or would it be better to put the HVE down and grab the saliva ejector? sorry for the long question I'm trying to learn as much as possible in my very short nine week class

      I know it feels fast right now but that’s because you haven’t gotten used yo the pace of the procedure/doctor. Once you get used to a doctor things will be better. There are dental assistant memes that joke about how we sometimes need to grow an extra set of hands 🤣🤣 Even now, we can struggle between passing instruments, retracting, suctioning…all at the same time 🥴🥴 Just like the lead DA you’re currently shadowing, you also have to develop your own “tricks” so that you can adapt to different situations. Here are a few tips:
      -If you have to use one hand to retract and the other to suction with the HVE, but you see water pooling in the back, then yes you can suction that with the HVE. Another option is to alert the dentist that the patient needs to spit, that way you guys can stop and then use the saliva ejector to get that water out. Also, you can hang the saliva ejector by the corner of the mouth (see how hygienists do it) and that way you don’t have to worry about water building up while you are working.
      -You can use an extra mirror to retract and that’s our best recommendation because the air/water tip sometimes isn’t as sturdy. If you use the mirror though, you will just have to get quicker at putting the HVE down and then grabbing the air/water syringe as needed. It may seem like a hassle now, but you will get used to it.

      Most of the time the DA just adapts to the dentist. We need to develop a good communication so that we can work as a team. Don’t be concerned if once you learn things one way in this office you’re at, that you may have to readapt in the next office 😬 But communication with your dentist is key! They may even help you by showing exactly where they need the suction at a given time. Reach out on IG if you have anymore questions ☺️☺️

    in the UK we only use the suctions we dont touch the aspirator its for the dentist only

      That might be tough to shoot lol the plastic mirror really helps and you just have to stay firm holding that monster down 😅 Also, don’t be afraid to tell the patient hey I need you to relax, please don’t try to swallow while we are in your mouth if you have to swallow raise you hand and we will give you a break.

    Lol I retract with the saliva ejector cause for me, I push it in-between the tongue and close to the tooth gum and it helps me a lot cause my patients close to spot every 5 seconds 🙄😅 but I have my tricks .. I just wanted to see how others do it anyways 😁👍

    Do you have, or would you make, a video about how to rinse off the etch? I know it sounds simple, but I swear new assistants blast the etch all over the mouth causing patients to freak out due to the acidic taste. It makes for a poor patient experience and frustration when you're trying to keep the tooth isolated. It's hard to get them to understand how to properly rinse off the etch. Also, how about how to properly "give the patient a good rinse". That can get exciting when they try to rinse out the patients mouth with the HVE and end up suctioning cheeks and tongues (and tonsils). I will look thru your other videos for this stuff.
