Since you said you'd like to focus more on Sacrifice Rooms. You get a guaranteed Redemption drop if you play the spikes there after entering a devil deal and refusing anything. Could be nice if you ever want to go angel, but also need more damage. Red chests count as deals too.
Genuinely, I've been watching you for like 3 years now, and this is the best run I've seen. The ups and downs, the missed diplopia with the Glitches Crown. I sat for nearly 3 hours and enjoyed every minute.
If anyone is wondering, Magic Skin is why he kept dying around the 2 hour mark. If you have 12 broken hearts and extra lives you’re revived to one heart and if you pick anything up it’ll kill you.
But why is it causing him to die picking up shop items with money? Its not like the shop was a devil deal room. EDIT: I see it now he did have all 12 Broken hearts but idk how its removing the glitched heart container from 9 lives revive.
Probably one of the most Insane endurance runs of the known Eden streak since I started watching 2 1/2 years ago. Truly Sin is one of the great TBOI players.

I was in pain for the whole time after you lost diplopia in the shop, but the moment you found R key again at the very end and with the last possible use of the magic skin was the top moment of the run for me (followed by the moment you found death certificate), so I'd say it was completely worth it. Definitely the best Isaac run I've ever seen. Thanks man!
What a run, deserving of the title "Ultimate", the ups from finding that last R key and the downs from missing the Diplopia at 1:40:15 as well as the inability to take items. This Run was a masterpiece of Excitement and suspence, the last few minutes waiting with bated breath to see if he really pulls it off man what a run, and watching this at 1x speed was an experience and a half.
Seeing that diplopia in the downpour 2 shop honestly one of the most infuriating moments especially as I know I would have absolutely missed it as well if I were playing
Nothing has hurt more than watch everything go wrong, to right, to becoming something legendary, just for everything to be dependent by rng and it ruins everything. I think you have an idea how hype this run was to watch. Watching that ending was like watching a movie with no happy ending. I loved this video though. I hope a time like this will happen again.

So I looked up the Broken Hearts, if you revive after getting 12 of them, you'll get a fake heart that resets to broken with any new item obtained. Basically your health becomes The Lost's but worse, grabbing Genesis was the best choice there

I’ve been subbed a year and a half and have never witnessed something so absolutely epic. Even with the ending, this is the ultimate Isaac run for sure, didn’t even need sacred heart or Godhead

Heartbroken added three broken hearts while you already had nine-while you did revive with an extra red heart, you internally were meant at 12. I saw this covered in a video about Magic Skin. You could actually see this right before you used Genesis. You respawned with 12 broken hearts-effectively dead.
to prepare for this run i got some snacks, sat on the couch, got a job, a wife, a child, died of old age and gave my child this video link as testament
To everyone screaming about the missed diplopia. He could have possibly died each time he picked up diplopia and both r keys. He also would have lost them both using genesis. Genesis saved the run.

I think that you’re underestimating the power of spawning items in the secret room then rolling them. There are so many powerful/game breaking items in the secret room pool that you should consider. If you read this, I love the content and thank you for making it sin!
Please do not feel bad for the RNG at the end. I watched this in full and could not tear my eyes away. SO many incredible moments that kept me on the edge of my seat (I think I would have cried laughing if the game actually crashed from all those upgrades). I love your videos!
I believe what was happening during the latter part of the run where you kept on dying for picking up anything was that since picking up heartbreak while already on 9 broken hearts, the game essentially put you at a full rack of only broken hearts. At this state, the game gives you a temporary form of HP which instantly kills you as soon as you get an item or get a health up.
The full racks of broken hearts should have killed him much faster. Once you have 12 of them, you cannot rely on the game to consider you as being alive. It is miracle that Genesis intervened at almost the last moment. Sinvicta blew through all 9 lives, went from Eden to T Lazarus, then to regular Lazarus, then finally to the blue baby form, only to be saved by the bell.

WAIT A MINUTE. 1:40:15 after he got Champion Belt, Diplopia spawned because he had restock… he could have gotten it for two R keys and made everything a lot easier.
I think the only way to do a Full House Clear is with "School Bag" 2 "R keys" and a "? Card", with out relying on Mega Stan Blue Baby RNG void spawn or mods. Lamb - Victory Lap Hush - Delirium - "? Card" Mother - 1st "R Key" Blue Baby - Mega Stan - 2nd "R Key" during 2nd Phase, when he his skull is cracked (very start of the animation) and starts descending before the cutscene. Dogma - Beast - Final ending cutscene. Again all circumstantial of getting the RNG of the items above and having the skill to do it.
Such an incredible run, you should be fully proud of yourself for managing to get as far as you did. I rarely like videos, but this one really deserved it. GG's man

I would’ve used alabaster box in the secret room and then used the dice shard and i wouldn’t have gone devil so I could get two items
