Emergency Room Chronicles: Pericarditis Exposed | Symptoms, Diagnosis, and Treatment
Pericarditis is an inflammation of the pericardium, the sac that surrounds the heart. It causes sharp chest pain that is central, worsens with deep inspiration, and is positional. There are two main groups of causes: infectious and non-infections. Of the infectious causes, viruses are by far the most common. Bacterial and fungal infections, however, can also cause pericarditis. Non-infectious causes include things like cancer, lupus, rheumatoid arthritis (RA), trauma to the chest, heart attack, and even certain medications. Sometimes, we don’t know the underlying cause of pericarditis and so we call it idiopathic.
In order to settle on the diagnosis of pericarditis, other life threatening causes of chest pain should be ruled out. Clinicians need to get a good history and physical exam on the patient. During this exam, sometimes a pericardial friction rub can be auscultated. An EKG should be completed and may show widespread ST elevation. Lab and imaging tests will often be done. Lab tests may include a CRP, ESR, white blood cell count, and troponin. A chest x ray, echocardiogram, and CT can be completed to help further evaluate the cause of the patient's chest pain. A complication that can happen with pericarditis is a pericardial effusion, where fluid builds up in the pericardial space. The treatment of pericarditis depends on the underlying cause and if there are any complications. Bacterial infections require antibiotics. Mild viral and other non-infectious pericarditis may simply require supportive care. Anti-inflammatories, like ibuprofen or aspirin, may be given for pain control. Doctors may sometimes prescribe steroids or colchicine. Most pericarditis patients are able to discharge home and follow up with their doctor in a week. Patients with more severe pericarditis or complications will usually need to stay in the hospital and get admitted.

    I am dairy allergic and eat low carb. Accordign to cronometer the past week, my phosphorus is like 4 times my calcium intake. I have been fatigued, new dental problems, trouble staying asleep.

      Thanks for the question. Personally, I have not see that, but HCTZ is known to cause or worsen hypercalcemia.

      You’re correct. Tylenol is the brand name. The active ingredient is acetaminophen in basic Tylenol. But there is also Tylenol cold and flu, Tylenol pm, etc. and those have other ingredients in addition to acetaminophen. Thanks for the input.

      Hyperkalemia effectively raises the resting membrane potential of the cell, so now any stimulus, even a small one can cause hyperactivity.
