Solving the Dental Crisis in the United States
In this video, I share a new initiative I have developed as part of my doctoral research at George Washington University. We are tackling a major issue in the dental field-the fragmented education pathways for dentists, dental hygienists, and dental therapists. These divisions significantly impact access to care and professional dynamics within the industry.
I have designed a tiered educational model that unifies these pathways, allowing professionals to progress from an associate degree to a doctorate. This model aims to increase the number of midlevel providers, such as dental therapists, who are essential in addressing the growing disparities in oral healthcare. If you wonder how dental therapy works or why it has not gained wider acceptance in the U.S., this video offers insights and solutions.
Some of the common questions we address in this video include:
What is a dental therapist, and how do they differ from a dentist or dental hygienist?
Why is dental therapy crucial for improving access to care in underserved communities?
How would a tiered educational model benefit both providers and patients?
What are the challenges in implementing a unified educational system in dentistry?
How can midlevel providers like dental therapists reduce healthcare costs and expand preventive care?
I will discuss how this new model could reshape dental education, improve access to care, and reduce systemic barriers in both education and healthcare. I also introduce a concept map that outlines the benefits of this tiered system, including its impact on affordability, access to care, and collaboration between dental professionals.
If you are a stakeholder in education, healthcare, or dental policy, this video is for you. Even if you are simply curious about the future of dental care, feel free to listen in and ask questions.

Therabreath (US) seems to be the same as The Breath (uk) I think I will get The breath mouthwash as it is easier to buy here in Hungary. I hope it is the same.?
From what I can tell, you’re correct.
Is Ultradex the same as closys but under another name?
I had not heard of UltraDex until now. It appears to be a British product line, but from what I can tell, it seems to be the same as Closys
@@DentistrywithDerik Thank you - I think I remember now Dr ellie saying it was the english version. However you can't get it here in Hungary unless you want to pay a high shipping cost and sometimes taxes. She also suggested Act anticavity so I;m not sure who to trust. I already bought the ACT ages ago but only used it a couple of time because it seemed to make my gums worse.
I recently realized using mouthwash over the years eroded my teeth and made them sensitive.
you da man... thank you!!
Thank you, I wish good and healthy teeth for all of your life.👏🙌
Nice! I remember a couple years ago listerine was compounding my gum pain and I went to the dentist they sold me some stuff that was gentle and non abrasive and now I never use listerine and I always side eye crest and Colgate products. But since I haven’t been to the dentist in a while my gum pain is back and I had to push my current dentist appointment forward for circumstances out of my control and was at the store looking for something non abrasive and I’m glad I picked thera breath because everything else there was on your no go list. It seems to help alot but it does have a chemical aftertaste that takes a while to go away. Not sure if it’s the mint or the cpc but my gums inflammation has gone down and the pain has subsided a bit.
Glad you saw some improvement!
@@DentistrywithDerik yes sir! Thanks for the video!
Am buying the sonic 😊
Thank you so much for this video. Sharing knowledge about health is like sharing invaluable treasure.
You have actually cleared the doubt. Thank you.😊
Equate alcohol free?
I like how you explain and are straight forward.😊
Ive definitely heard the opposite of this as rotating brush heads cover more area but it's all the preference of each hygienist
I heard that on avergae women have more acidic mouths than men too so may be even more important! I want to see tootpaste and mouthwash studies done comparing efficacy of products for women compared to men!
YES DERIK - CALL THOSE CONFLICTS OF INTEREST PAPERS OUT!!! thank you for actually giving your opinion on sonic vs oral - I've been researching for an hour or so, and so many people say its just preference and mouth size / ability to reach back of mouth with smaller brushead - I really appreciate you pointing out the risks of oralb style! I'm chooing sonic thanks for helping me make the decison :D
You’re welcome!
THE ONE’S ON THE “BAD LIST” ARE THE ONE’S MY DENTIST RECOMMENDS?!?!?! I watched this twice… I can’t believe how much I’ve been led astray. I see the posters in the dentist office about acidity and have changed what I eat/drink based on this info… WHY CONTINUE TO RECOMMEND BAD MOUTHWASH?!?!?
That’s a great question! Sadly a lot of providers don’t give it much thought and worse the companies that make these products heavily leverage their influence in dental offices.
Interesting. I’d been using sonicare for 20 years and recently got an oral b at my hygienist’s recommendation. I have very crooked teeth and I felt like it did a better job of hugging every side of each individual tooth. Hearing what you say, I may continue to use both toothbrushes, one am one pm. When using the oral b I’ll try to avoid the gum area. I water floss each time as well so that should do the trick?
I use fluoride MW to prevent cavity and Listerine’s zero alcohol sensitivity MW. Lately, it’s been causing gum/teeth sensitivity. What can I do?
Best oral health regiment
Is Closys Ultra Sensitive mouthwash safe to use 4 times per day? I know the ph is safe but what about the active ingredient chlorine dioxide?
Should I try it ?
I got decay and areas of gums disease. I think i need a special toothbrush. The dentist didnt say.
An honest dentist? Good to see, and good info 👍
$29 on Amazon currently
Does Closys reduce Nitric Oxide
which is the best mouthwash to use

Gottem’
Okay I got this based on reviews and the vibration is SO DAMN INTENSE. Even on soft setting! The toothpaste falls off of it. Is this normal? It’s my first electric toothbrush and it tickles my teeth. I don’t like it. Am I doing it wrong? How do you use it? Do I need a less intense toothbrush or am I just not used to this yet?
Watch my video on Rotating or Sonic Brush? Which is Better? th-cam.com/video/UhN0B2XDPRI/w-d-xo.html. I show how to use one correctly there. You may be using it wrong.
Sir, I an a dinosaur dentist. Graduated 41 years ado from dental school. I am learning a lot from you. Thanks.
This looks nice, but there are several Philips models that are the same price or less. Even if this is as good (and I assume it is), the risk of the off brands is not being able to find replacement parts, heads, etc…. Nice review.
@@jlmcgrot it’s very comparable to Sonicare. The one major benefit is it includes 8 replacement heads where Sonicare only gives you one. The heads also tend to be less expensive to order on Amazon.
So should i continue flossing or not?
Conflicting messages and I think because it's a commercial thing. My dentist recommended me to buy the Sonicare 4300 and I must say, what a Great brush. Have been using the Oral B for decades and always sensitive and bleeding gums. I've been using the Sonicare for about two months and my gums don't bleed anymore and the pain is gone. And I like the brushing mode of the Sonicare much more. The feeling of clean teeth is more noticable.
Several days ago I watched another review by another dentist influencer, and he recommended to buy Orsal B, because Sonicare is more harsher on your teeth. If doctors get something from these companies, maybe there is a reason why they do recommend one brand over another.
why you not mention about brushing the ''Occlusal surface'' ??
@@Abhi-iz6qr great question. I’ll be redoing the video shortly to address that oversight. See my most recent video “sonic or rotating”.
Most dentists say both rotating and sonic are good and refuse to pick a winner. Thank you for picking a side. I'd be curious on your thoughts of the laifen toothbrush that a lot of TH-camrs have been pushing lately.
@@jasonjorgensen9888 I’ll check it out
I love Love LOVE my Sonicare! I certainly wish I had one prior to abusing my gums with improper brushing technique. "Brushing teeth is NOT sawing logs!" With the damage done, however, my Sonicare has kept me from exacerbating the issue and my RELIGIOUS use of a fluoride mouthwash has kept me from having any problems with sensitive teeth.
Nice video, good info and not too long. Might give the sonic a go next time and I will aim to use my current rotating one better now I know how to. Sub + Like and good luck with your channel :)
@@Gunship thanks!
Thank you for your insight
😱😱😱😱
Hands down the best for me is the Sonic brush with the plaque tip C2 head.
That is a good choice!
Just back from the dentist, she explicitly recommended a rotating toothbrush. Why do you strongly advice against it? It's so annoying to see so much contradicting advice. We as a patient will never know who to trust.
@@nmap-p- that is a fair question. I will be posting my next video on this very subject this week. Stay tuned!
There is a extra gentle mode on the rotating toothbrushes. Sonicare unfortunately doesn’t clean as good for me.
I'm sorry, but I have to ask this. I'm pretty new to flossing. And I kinda snap a lot in few places where my teeth are tight and my teeth on the snapped area are kinda shaky when I push it with my tongue. But no bleeding at all. I'm concern about my shaky teeth now..I feel no pain when chewing food but the little shake gives me chills.😢. Movement happens only when I push it tho
If your teeth are mobile, you need to get professional assistance as soon as possible.
I always had bleeding gums. When I started flossing regularly it went away. I’m going to keep doing it
That’s a good idea
A&H Advanced white has an RDA value of 106 which is considered highly abrasive!!
Anything over 200 is considered highly abrasive. It’s also why I only recommend Peroxicare and Sensitive. Their RDA values are much lower (under 55).
@@DentistrywithDerik The chart I looked at 101- 150 is highly abrasive and 151-250 is considered harmful. The whitening agent is likely the cause. I personally use closys which is at 53. Any other toothpaste seems to give me canker sores inside my cheek. At $13 a tube I hope it's as good as the other brands!!
@@Louis275 Closys is excellent for canker sores - mostly due to its high pH and gentle ingredients. I haven’t fount a “perfect” toothpaste. Maybe I’ll make a “wishlist” video for toothpaste manufacturers to consider :)
@@DentistrywithDerik I had what may have been an abscess last friday on my upper gum. It was a white spot and the gums looked a little inflamed around it. I was recommended to do a saltwater rinse a couple of times a day. By saturday I started to see improvement. By sunday it was completely gone. I haven't had those in a long time, so I was surprised to see it. I'm a perio patient so I go to the dentist every three months instead of six. I normally don't use mouthwashes so will using a mouthwash prevent them from happening again? I read that saltwater rinse maintains a neutral pH in the mouth, which is less hospitable for harmful bacteria. Obviously, I don't want to keep using saltwater so is something like closys silver mouthwash just as good??
I'm getting it! 🪥
A very big appreciation to Dr Amade on TH-cam@DrAmade, For helping me get rid of my six years chronic bad breath and infections with his natural herbs medication, May God continue to bless your good work sir
I’m a guygienist too. Let’s gooooo 🤘🏻
Good luck in your career bro!
Hello. Thank you for the advice. I want to invest to sonicare brush. One, more expensive one has gums care option. Is it worth to buy it or more basic option will do? Thank you for your time.
The basic ones still work quite well. The more expensive ones just have more features
Even with using a manual toothbrush, doing circular motions wouldn’t hurt the gums right?
A manual brush with soft bristles, using small circular motions can work, as long as you’re thorough and gentle.
I am not understanding this. Why is the pH of something you swish around in your mouth for thirty seconds relevant at all? What is the baseline pH of your mouth, and how long will the duration of the pH change within your mouth after use of a mouthwash be? What about the pH of food and drinks? How would the significance of a low pH mouthwash compare to basic eating and drinking throughout the day? Should people not consume citrus fruits whatsoever?
I think you can watch the video he recommended
That is a great question. I may need to make a video on this as well!
Could be used Closys before the toothpaste and RT-3 after for risk of cavities? So essential both over the same session? Do Closys Ultra sensitive is ok too?
Yes to both your questions
I have found Enamel On paste by Premier with ACP to help GREATLY with sensitivity. How do you feel about this ingredient?
