    This video was quite informative. Thank you. I was diagnosed Type 2 in 2008. After years of moderate success and setbacks, I am on the right path. My a1c is at 6.2 (down from 14.4 in June, 2023). My LDL is 55, triglycerides, 51 and HDL, 45. My LPa is 79. Recently had an echo, my cardiologist was happy with the results. However, I suffer from high blood pressure, which is managed with Losartan Potassium (100mg) and Amlodipine (10mg). What kind of healthy foods can I add to my diet to help lower my LPa?

    My Lpa was 240!!! LDL is 196 HDL 106 Tri 67 BP normal A1C 5.7 (elevated) Glucose 93 CT calcium heart scan score 0 plaque. I'm waiting for LipoB results. I schooled my Dr.to auth these tests. I eat extremely healthy, no seed oils, low carbs, exercise daily, pasture raised organic everything... 61 124 5'2 post menopause no HRT high stress. I'm shocked and am amazed and concerned. I heard Niacin is effective but just heard you say no.

      Hi! I would focus on lowering your LDL, apoB and A1c levels with a high Lp(a). Our goal is to optimize all your cardiometabolic metrics to reduce the risk of high Lp(a). A CAC score of 0 indicates no hardened plaque but there can still be soft plaque in the arteries brewing. Niacin used to be given to people with high Lp(a) levels and research now shows that it increases the risk of blood clots so it is advised against.

I know sugar and carbs are a culprit but I eat low everything so I'm so confused. I lost 6lbs because when I took the tests I was 130!

      Why would you avoid HRT when you have HIGH LPa?? HRT (estrogen specifically) LOWERS cholesterol and LPa. You're making a mistake.

      I had heart palpitations and my pcp sent me to a cardiologist at 35 and he sent me to get a CAC scan and my score was 600! I ended up having a CTA that showed 99% blockage in my RCA and 75% in my LAD but when having a heart cath performed I only have about 10-20% in both arteries. I guess a blessing but still worrisome. My lipid specialist says it will only increase over time but on Repatha a meds to try to help. Thank you for the helpful info! I’m now 38 so I have learned so much.. and not scared anymore but glad I caught it!

    The average LDL cholesterol level of a centenarian without medications is about 110 mg/dL which means in mid life it was a little higher. There are no long lived healthy populations or centenarians that have LDL cholesterol levels under 90 mg/dL except for a few anomalies.

      Yes best genetic profile is higher cholesterol with no atherosclerosis. But not everyone is like this so must prevent atherosclerosis to live longer.

      @@beepbeepnj2658 Median value is around 100 mg/dL, so the number you gave of 110 mg/dL is an example of what I was referring to. The value of 70 mg/dL is where no atherosclerosis occurs so higher than this also.

The CIMT test results show the truth.

    The last thing you want to do is tamper or lower your Lp(a) levels which are in your genetics to live a long healthy life. "Remarkably, one-quarter of the centenarians had high Lp(a) serum levels even though they never suffered from atherosclerosis-related diseases." 1998 G. Baggio. Low levels of Lipoprotein(a) can destroy your health. 2012 article title: Low Lipoprotein(a) Concentration Is Associated with Cancer and All-Cause Deaths: A Population-Based Cohort Study (The JMS Cohort Study).

    I just got mine done and LPa was 191 others are slightly elevated by a point or so but I am diabetic and I don’t have complications from diabetes but likely insulin resistance. I don’t really want a statin so I’m trying to see if I can get it down naturally. Is that possible

      When you have a high Lp(a), the target cardiometabolic metric parameters are at a much lower range than the general population. You can optimize your cardiometabolic metrics naturally through nutrition. Lp(a) can fluctuate but research shows it is not significant in reduce of MACE (major adverse cardiovascular events), which is why targeting these metrics with a high Lp(a) is more advisable.

    What he's saying isn't consistent with what I observe in my patients. Most of those with heart attacks and strokes have LDL less than 100, and those on statins who's LDL have dropped as low as 30s to 50s seem to get recurrent strokes and heart attacks.

      It may help reduce Lp(a) by 20-25%. I would also stress the importance to get apoB low and optimize all cardiometabolic metrics to help with reducing the risk of future heart disease and it's complications.     I found out I have Lp on 300 , my cholesterol score is 4,2, b.sugar 4.1 , everything else was perfect, only thet lipoprotein . I cut out red meat and Idint use seed oils at all (few years) , not drinking, no smoking ,never. Running, lifting ,mtb ,mountaineering is my lifestile, have u somthing more to recommended to me? 😅 thx

      Hi! I would focus more on foods you are adding in your diet to protect your blood vessel health and that will help optimize all your cardiometabolic risk factors. Having a high Lp(a) can accelerate plaque formation and increase risk of heart failure and Afib HOWEVER if you take a more aggressive risk management approach you can live a long and healthy life with high Lp(a).       @@zocentive3691 Could you please clarify? Usually, measurements of LP(a) are in mg/dl or nmol/L. Rarely, in mg/L. as you reported. The type of measurement would make a big difference in what your number means.

    This is all good advice. I would comment that some patients have a genetic predisposition to heart disease, without showing any of those risk factors. I am one. Early death from heart disease runs in my father's side of the family. At age 46 I requested a CAC test, and had a score > 1000. I was told my numerous primary care doctors and cardiologists up until that point that all looked good. None of them tested Lp(a), which I had to test on my own (after starting statin therapy). I have tested it 4 times so far, with results ranging from normal (74 nmol/L) to slightly elevated (123nmol/L). Lp(a) wasn't tested during my father's lifetime, but his LDL-C was normal. My main point is if heart disease runs in your family, don't rely on blood tests alone. Get a CAC and/or CIMT at age 40 or younger. Treat Apo(B) aggressively.

      Thank you for your comment! I agree. I speak in other videos and my other platforms about how to know if you are at heightened risk. Lp(a) needs to be tested and if high, we do need to achieve a more aggressive reduction in these cardiometabolic metrics I mentioned in this video. This includes ApoB. CAC and CIMT testing can help also stratify risk to then have a more proactive risk management strategy. Knowing your risk is so important in taking action early and knowing it needs to be at stricter standards!

    Can you discuss low blood pressure? How does a patient know how low is too low? There seems to be no well defined cut point.

      Hi! Low blood pressure is not desirable either. If your blood pressure drops too low, your body's vital organs do not get enough oxygen and nutrients. Most doctors will adjust medications either when it gets too low or they are symptoms of low blood pressure present (like dizziness, fainting, confusion, blurry vision, etc). We do not want blood pressure to get below 90/60mmHg. If any of your BP values are persistently below these levels (either diastolic or systolic), I would highly recommend telling your physician as medications likely need to be adjusted for. I hope that helps!

    I feel like you are handed a death sentence with this - incredibly stressful - don't smoke, don't drink try to be healthy as well having an autoimmune to tackle i have been told I have a lipoprotein score of over 1500 plus overall cholesterol score of 6.4 - Dr then says, i am begging you to go on a statin! The frustrating thing about this is there is nothing that can be done - a statin prolong your life for just a month - just need to embrace each day.

      I completely understand the stress that comes with an elevated Lp(a). There are several trials currently going on with medications to lower this value. In the meantime, I would focus on lowering apoB and LDL cholesterol as we have seen that people can live in their 100s with high Lp(a) as long as they have well controlled apoB and LDL values.       @HA-kw7vq Are you actually sure that '1500' is about Lipoprotein (a) i.e. Lp(a) or another 'lipoprotein'? Also that '6.4' seems a score about glycated hemoglobin (glycated hemoglobin), not a cholesterol score. Anyway search on youtube for 'Living with elevated Lp(a)'.

    I've tested my Lp(a) 3 times. I have to order it myself since primary care has no idea what Lp(a) or Apo(B) is. I've gotten 76, 109, and 123 nmol/L. I remain unconvinced that Lp(a) is a once in a lifetime test and cannot be influenced by nutrition profile.

      There are foods, medications and supplements that decrease lipoprotein a but not at an impactful degree to lower risk. Research shows that the fluctuations do not have a significant enough decline to have an impact on cardiovascular risk. Once it is high it is a sign there is heightened risk and we need to implement stricter protocol which is why it is only clinically recommended to test it once.

      My Lp(a) was down 32% in 12 months so I am not convinced either.

      ​@@sybillc263 I wish I knew, I tried a million things and half don't work. I did make lifestyle changes aimed at reversing liver cirrhosis, including periodontal treatment, which may have helped. According to my dentist there is no more new plaque buildup.

    Thank you so much for your videos. I just stumbled upon them from watching the plant based kidney dietician. You both are doing such an amazing service to the public. I was diagnosed with diabetes 12 years ago (also have hypertension for many years) and advice from physicians was to lose weight but NEVER mentioned the damage that can be done to the kidneys and vascular system. The only reason I found out recently is bc my Sister-in law was diagnosed with CKD 3B and I asked what was the reason she said hypertension and I started researching and found out diabetes is also major cause of CKD and it is non-reversible once you get it. I do not understand why I was never given this information-baffling bc information is power. Glad I have it now to hopefully stave off the disease. Thanks again for what you do bc also found out that diabetes is a major risk for cv issues. I know physicians do not have a lot of time but I absolutely would have listened but never getting the information is just again baffling that this is never discussed with patients that you will ruin your kidneys and vascular health it was take this pill and you prob will need insulin as you age-thank goodness I have been able to stay with metformin. My new mission in life is to reverse this now that I know you can. Thanks for spreading the word!!!!

    My husband has been diagnosed with CHF, he’s know for years but in the last month even with edema in legs/feet/stomach he has lost 12% of his body weight. I am in Canada and 1 of the worst healthcare in the world, years to see specialist/testing. I know the prognosis is 3-18 months. He has stopped taking his diuretics and high blood pressure as it caused his gout to go nuts, So my questions is… What can I do to make his last months comfortable as he isn’t eating much and drinks less than 2 cups of liquids a day?

Still no help from the heart clinic, they put him on meds, it gives him gout, he goes off them after phoning the clinic a dozen times in 3 months and still doesn’t hear back.. His health is deteriorating fast every day.. Canadian healthcare is the worst..

    3:18 don't get more than 500 mg of calcium at once. calcium supplements are fine [but wait, what about the risk of nanoparticles in calcium carbonate?. oh, i just had a chat with gpt 4 on the topic, and it said "DIETARY sources of calcium do not typically cause hypercalcemia (excess calcium in the blood), a risk factor for arterial calcification". i think that's important to specify: the 500 mg threshold only applies to supplements. so i guess i can keep drinking my 500 ml of fat-free milk for dinner, even though it gives me 600 mg of calcium].

    I have heart failure and I have the opposite of cachexia, dry, always hungry, gaining ( non fluid) weight. But my docs say I should not actively be trying to lose weight.

    So, the ApoB particles decide to commit "suicide" and hit artery walls once they realize that there are too many of them? That makes absolutely no sense to me. If the particle had the ability to enter the walls, everybody would accumulate plaque. Some particles would enter, the others don't. The more particles the more plaque. I have not heard any explanation how this magical threshold works that makes the particles misbehave. What is the physiological mechanism and cause for that, besides inflammation - which Dr. Dayspring rigorously brushed off in another video.

      There is a few different things happening. 1. If there is too much the particles get stuck, like any plumbing system. 2. The body will start storing fat in odd places after a tipping point like in organs and around organs.

    My problem with experts is that they don't see the bigger picture. They are entrenched in their expertise. So, are there big ApoB LDLs that are actually "good"? If there are (and I'm sure of it) - then maybe a high non-HDL cholesterol can be extremely healthy in such a person ? What about the long term harmful side effects of Statins? Do they accelerate dementia? Do they promote insulin resistance ? Myopathy - the heart is sort of a muscle as well... What if the person has no inflammation (as in, chronic) and a low blood pressure ? Do you really need statins if you have big ApoB LDLs that are protective and not atherogenic, have low BP and no inflammation? Lowering non-HDL per say is a questionable goal when out of context !!! In my opinion interviewing one person, an expert, is missing the point. If you're lucky, you get a piece of the puzzle right. We need a panel of experts from different expertise to tackle a subject from as many angles as possible. Sometimes halves truths can have a negative end effect.
