Great info! I do however, think the amount of taxes paid by the citizens to finance government healthcare in those countries should be addressed in the cost "triangle". For example, in the UK, tax for those making approximately $50-$125k pay 40% in income tax. I'll take our system any day although improvements can still be made to reduce costs. We also do have "Medicare" which is definitely a benefit which needs to be in the conversation.

Wow! A spine surgeon doctor with a heart and concern for the interests of the patient! I twice visited a neurologist who was 45 minutes late for each appointment; and would barely let me get a word in edgewise to the conversation about surgery! All he could mumble was "you need surgery". I had a barrage of questions to ask him, but he was in too much of a hurry to care! What is the objective of the surgery? On what part of my spine anatomy would such a surgical procedure be performed? Show me why the MRI leads you to this conclusion or strategy? And why there? What would the surgical procedure actually accomplish? And would it resolve my issues completely? Or would I need to come back later possibly for another surgery? What are the risks? Are there any guarantees that this will fix all of my pain and neuropathy? If so, are they in writing? And then if it fails to accomplish the objectives; will you refund the money back to me and my insurer? Will it take multiples trips to the operating table to effect total relief of the involved pinched nerve roots or spinal canal? What is the healing process feel like? How long typically does it take?.......my God, as a critical thinker; these questions roam my mind and those of others!! Is the doctor more interested in collecting money for the office visit than he is in caring for the patient? Then "eff" him and move on to another! Thank-you doctor!

My father is 80 years old. Looking at his complaints of continuous tinkling in both hands (till fingers) and two legs, we gone for MRI. Looking at MRI report doctor has suggested to go for ACDF surgery. They also suggested first try with few medications for two weeks. My question : Is there any medication instead surgery. Given my father is 82 years, is it okay to go for surgery at this age? Or any medication/cure ?

I'm possibly facing a fusion of L5 S1, I do not have any disc left at all. So the question I have, Is a fusion of that area okay? My doc said that part of the back doesnt move much.
Thank you for reaching out. I am unable to comment on your case specifically (since I am not familiar with the details). However, a single-level L5-S1 fusion is well-tolerated by the majority of patients, without significant functional deficit or restriction of range-of-motion [especially if the disc has mostly degenerated/ worn-out].
Had spinal fusion on L5 S1 almost a year ago started to develop severe, numbness and weakness in my legs and feet in my fifth month. Now it can hardly walk or move. They found nerve damage by the S1. I wonder if it can be repaired or I’m screwed for life.

O bviously I am unable to diagnose your condition or make treatment recommendations, but it could be helpful to get a second opinion from another surgeon (preferably at an academic medical center, where the surgeons are used to dealing with complex/ difficult cases). I am a 55 year old white woman , never smoked,never drink, never use drugs, I have been taking Advil since I was 15 years old because of a snowmobile accident low back fracture now I am going in for neck surgery C 3-4 fusion June 12th 2024 3 month recovery question is will I be able to lift my granddaughter anymore

However, one of the most common reasons why patients may have persistent back pain after spine surgery is because the fusion has not healed. What about dancing with twisting on the waist? Will it worsen my spondylolisthesis?
Dancing is unlikely to worsen the spondylolisthesis itself , but the symptoms may indeed worsen.
Good Timely information as i have a meeting on monday with the Surgeon to discuss, "What now" for my Scoliosis and Stenosis, which has become quite debilitating.

I Will have surgery. Im beyond Bad. To say the least. Ut im going to Research before then. If i can be 50% Better,what alone would ve Amazing.... I have this exact same situation with the same 3 disks. I am wondering if the Non-Fusion Motion Preservation Lumbar Spine Surgery would work for this situation . Where is your practice at?
For a three-level disc issue, most surgeons would advise ACDF instead of a disc-replacement/ motion preservation. Your videos are very through thank you. Have had to push for EVERY step of solving back pain. Tried alot--finally MRI latest was epidural at inter-laminar Have constantly complained of pain and and fecal incontinence this shot eveny with valium HURT alot did not work don't want another. Going to try to try chiropratic I do have l4-l5 pinched nerve.

in this video "Failed spinal fusion: How I fixed it" where laminectomy & fusion was performed bony arch was removed. Here similar procedure but facet joint was removed. What decides which part is removed?
Using the MRI/ CT the surgeon can determine how much bone needs to be removed to free-up the nerve. It varies on a case-by-case basis.
I've seen many videos, but this one is a world championship in explaining and visualizing the topic

re. waiting too long: are there any spine problems which have stronger indication that waiting or relying on the rehabilitation is not a good option and should be treated with surgery rather sooner than later? (not talking about severe prob. with cauda equina)
re. realistic expectations: how to ask about possible outcome of the spine surgery? patients usually are interested about the outcome. Is it possible to say that some symptoms/procedures can be healed/works better, close to 100%? If yes, which one?

I had a spinal fusion. L4 -S1. I was not told to give up smoking. My fusion didn't take. I ended up having a 360 fusion through front and back 8 hour surgery. My second surgeon said he would not operate unless I gave up smoking. I did give up and it was the best decision ever. My fusion fused but unfortunately I now have adjacent segment disease so I need another fusion above, and now I might have flat back syndrome so not sure what will happen there.

@SpineSurgeonSpeaks I fully agree with what you have said. It's a bummer ( sorry just my terms of people not understanding) it's a bummer that people still have thoughts that smoking can be fine no matter what. Babies are compromised from smoking, elderly people are compromised from smoking. The sooner people get real the least problems there will be.

Yes, DCS is a real phenomenon. The most common example is when a nerve is pinched in the neck (i.e. cervical radiculopathy) and the same nerve is pinched at a second location in the arm (such as carpal tunnel syndrome).
@@SpineSurgeonSpeaks that explains exactly what I’m dealing with. I am a sheet metal worker by trade and had carpal tunnel surgery 21 years ago. Worked great. Advancements in hand tools in the last 20 years has really helped with less manual hand cutting. I have chronic neck pain with both spinal and foraminal stenosis. On occasion it’s feels like my carpal tunnel has come completely back, especially at night. In a week or two later it just seems to disappear. The Surel nerve in my left leg has no response. I notice with the more painful cervical flareups my left foot gets painfully numb and is starting on my right foot as well. But not sure if related. Thank you so much for your rely!

Is MRI sufficient of diagnose L4/5 spondylolisthesis or special RTG is required? Can symptoms of L4/5 spondylolisthesis be dynamic and change from for ex. one leg to another leg or to central pain (days period)?
The best way to diagnose spondylolisthesis is with standing flexion-extension X-rays. The MRI will show how much stenosis (i.e. compression of the nerves) is present. Symptoms can (and do) fluctuate in severity and laterality.

Sir, this explanation was v. helpful, thank you. Could you shed some light on the following topic: what should be the relation between symptoms and MRI/RTG, should always be confirmation in the image of the reported symptoms and clear answer why the symptoms happen and what exactly should be remove/decompresed? What is image is more or less the same over a period of time but symptoms changed: from pain in the left leg to pain in the right leg or central pain? How such symptoms could be explained with similar image?

Usually, most patients with moderate-severe stenosis by MRI will have moderate-severe symptoms. I have had Cauda Equina symptoms a several years had to push doctor and finally got MRI l4-l5 pinched nerve,l3-l4 grade 1 spondylolisthesis with disc protrusion osteophytes ll2-l3 nild broad-based disc bulge, l1-l2 narrowing disc, mild retrolisthesis, small disc bulge, t12-l1 mild disc bulge degenerative fact change L5-S1 mild disc protrusion mild degenerative fact change ligamentum flavum hypertrophy (Severe right neural foraminal narrowing I am 71 female.

Hi, Doctor! If someone has had sciatica for 5 years, but it isn’t disabling and they can still do all the things they love, would you recommend they get surgery just due to the length of time that they’ve had it? (No red flag symptoms, no concerning MRI findings, no neurological symptoms, etc.)
