Both autism and ADHD have generated increased attention in recent years, and they are found together much more commonly than by chance alone. Although they can often be confused, the symptoms characteristic of each are fairly distinct and separable. Clinically, in the last decade, I have been seeing much more underdiagnosis of ADHD, and overdiagnosis of autism spectrum conditions.

Soooo… BCAAs, tempted to increase my dietary intake but I’m on SSRIs (plus my Ritalin). My brain-fog and fatigue are off the charts. I’m willing to try just about anything that isn’t likely to cause harm.

I was diagnosed (very old-school) w/ Asperger's and ADHD in 1996. Obviously, were I to qualify now, it'd be ASD. But just treating ADHD has removed effectively every barrier to success, function, and QoL I've faced. Sleep quality and daytime energy aren't perfect, but I think even quite a few normals are in the same stratum as I am there.
Is that your new book behind you?
I've heard that (difficulties with) emotional regulation and time-blindness are common symptoms of ADHD. Are these aspects widely accepted or still contentious?

There is also a discussion about seeing the autism “spectrum” not as a variation of severity of autism, but rather as a variation of the various symptoms and aspects of autism. There is talk about the spiky profile of autism, where an autistic person may have some abilities that are particularly strong while others are particularly weak. And there is a lot of variation between autistic people in terms of which the strengths and weaknesses are. (Of course they all share weaknesses in social interaction. But there are many more aspects to autism.) This is somewhat similar to the clusters of abilities and disabilities in ADHD people that were mentioned in the video. Some researchers in the UK even tentatively brought up the idea of calling it the autistic “constellation” or “constellations”, to make clear that it’s not a grading of something like severity, but recognizing various different profiles in autistic people.

Great video. I think there is a fine line between seeking validation with a mental health diagnosis and seeking identity with a diagnosis. I’ve seen a lot of people become diagnosed with something and that becoming their entire personality.
Does qelbree reduce sperm quality/motility in men long term? Planning for kids.
I'm not aware of any evidence that it harms or improves male fertility.
I didn't know I had ADHD since childhood, but after having COVID I suddenly have had a debilitating struggle with being an adult with ADHD.
I would like to ask this! Antibiotics!! I am taking a round of antibiotics for an ear infection now. Common broad spectrum and I swear theu are messing with my mind. Anxiety through the roof. A buzzing insecure feeling. ADHD symptoms through the roof. I just went shopping for late (as usual) Christmas presents. I lost them at the store!! I think at the ATM. I was soooo depressed I decided that the only thing to do, was to buy them all over again, as if nothing had happened. Money gone. I messed up at work yesterday. I acted out in stead of keeping cool. I made a fool pf myself and harmed my work prospects. So I am asking, can common antibiotics of the kind given, say for swimmers ear (outer-ear infection) affect the mind? PS. I am not taking any meds now. (other than finasteride to save what is left of my hair)
It's not a common reaction, but yes, some antibiotics are known to cause a wide variety of psychological effects. I had one patient who experienced mild hallucinatory effects and confusion with erythromycin.

I started watching and got bored.
This would explain my whole life. I was like this as a kid and continue to be so as an adult. I was diagnosed with inattentive ADHD a few years ago and I have borderline personality disorder, persistent depression, PMDD, anxiety/panic attacks, and OCD. I started having panic attacks when I was a teenager and would suddenly derealize/dissociate out of nowhere in crowded, chaotic, or loud environments, even when I wasn't feeling overwhelmed or uncomfortable to begin with. But I am also a highly sensitive person/HSP, whatever you want to attribute that to, so I have always assumed the world was just too much for me. I was a quiet and shy kid who grew up to have a lot of social anxiety especially after being bullied a lot. I have had a few psych evals that all found slow processing speed while everything else looked pretty good, although working memory was always kind of iffy too.

What’s the next couple steps up for adhd meds starting with buproprion? Would it go Straterra next? And ending with adderal?
Each individual is different. Some of it would depend on the need to move on from buproprion - no benefit? insufficient benefit? or side effect issues? It's not unreasonable to try Strattera next. Some might move on to a Ritalin/methylphenidate product before trying Adderall/amphetamine.
I wanted to ask a few questions but I tuned In late.
I take Cymbalta for depression and anxiety. I also have adhd and took stimulants in the past, but prefer not to take them. My doctor mentioned Qelbree. Being an SNRI like Cymbalta, would it be redundant to take Qelbree? Could it potentially increase side effects, or would it be more likely to help? I have read that too much SNRI increases anxiety for some people. Thank you.
The available evidence is that Qelbree (I have a video on this) is not a simple SNRI, but is affecting subsets of serotonin receptors in ways that other medications do not, so there is certainly potential that it could help. The actual norepinephrine reuptake is weak, and that seems to be the action most closely associated with anxiety. But individual results can certainly vary.

I wouldn't say belsomra is special, just unique since it was one of the first attempts at the theory. It is a failure and never showed much potential. Especially when the FDA only approved it and more than half the dose of the dose used in research. Including the creators, nobody had hopes in it. Hopefully something else comes out that 1- induces HEALTHY sleep 2 - doesn't create a quick tolerance 3 - doesn't cause a high or sleep walk behaviors 4 - causes a restful feeling after waking from a full night's sleep.
Failure is rather harsh. Many people are using it and are very satisfied with how it helps them sleep.

Hey Doc, could you make a video about Dexedrine? Perhaps your clinical practice's observations/comparisons? Also, do you think it could be a good way of navigating this adderall shortage? I really appreciate my IR adderall because it enables me to get my work done and then be able to get good relaxation in the evening. It's also been the best option for my insomnia. I'm on buproprion and clonidine, as well.
Never never take adhd meds. Im 16 mo ths still in withdrawal off concerta. There's a shortage because it's. Being over prescribed.
I appreciate you sharing your experience, but it's unreasonable and unfair to make sweeping generalizations based on it.
Absent hyperbole there is no more evil comorbidity than depression, as far as I'm concerned. Due to the time blindness inherent to ADHD. Whereas to the non so disordered, a deep depression may last bitter months, before some intervening agent gets sought out, by the afflicted, with ADHD literal years can thus trickle, like the sands of a broken hourglass, through one's fingers. Of course, gone forever, from a person's life... in such a state of abject misery. Hence why the scepticism I personally hold, towards so many claimants of the disorder, as some sort of gift or superpower, borders frankly upon naked contempt.

Maybe it's just me but I have the feeling that ADHD people experience depression in a very different way, like having all the ADHD symptoms amplified instead of pure sadness, I feel like we're quite resilient to sadness
Thanks John for a nice summary on a complicated topic. With regards to nutrition my main thing that I highlight other than omega 3 is Vitamin D. Vitamin D deficiency, especially in latitudes above the 30s is a significant problem. The second issue is a heavily processed food based diet which can be a challenge for those who are impulsive around food or with low appetite from stimulants. With regards to plant based/vegan diets, eating a well balanced vegan diet takes skills that require a fairly large chunk of executive function. Finding a neuroaffirming Dietitian can be tricky but they are out there
Some recent studies have indicated more people have Vitamin D deficiencies than one would expect. There have been a few studies that IF you have Vitamin D deficiency and depression, then Vitamin D supplements can help, but that just administering Vitamin D to everyone who is depressed doesn't demonstrate much benefit.

If one is taking buproprion to target ADHD, and no depression is in the picture, then within 2-3 days one would have a very good idea of how well that particular dose is working for ADHD, and it would make sense to increase the dose if benefits and side effects have been minimal.
I have accomodations at work but it feels unfair to other people. And a bunch of people were laid off recently.
If they are helping you preform more effectively in your role, and you think that they would help others as well, maybe they should become overall company policy. And unless you were doing the lay-offs, it's probably best to leave the responsibility for those decisions with the people who made them, not the people who were affected by them.

John thanks for picking up on this idea! I'm reassured you haven't found anything more than I had. Maybe my menopausal brain rot isn't as bad I thought. I'm pleased to say I'm doing way better now I'm on a combination of HRT, plus lamatrogine and clonidine after my diagnosis of AuDHD.

It seems to be a very interesting medication with purported pro-cognitive effects. I’m wondering if the hefty cost is worth it given that the Lexapro I currently take is very cheap in comparison
If the Lexapro is working pretty well, then it might not be worthwhile switching. I have had some people who responded well to vortioxetine who had not found relief from depression from other medications. I've certainly seen people who were more alert and functional, but it's not like the serotonin 7 receptor activity is adding ten IQ points to every user's cognitive abilities.
I think people with ADHD and combiend with ASP and ODD tend to lie to themselves, forget things, and gaslight a lot more than a "normal" person and then shield behind their "diagnosis". I'm speaking from experience since I have experience this in the past with friends. It's not like they have an upper hand, I'd describe it like something they do subconciously. I have no interested in taking advanatge of them nor can I be gaslit cus I'm too aware but its hard not to notice when people do it. I think it starts with taking accountability for argument and how you go about difusing. I'd argue that is a skill issue on either party.
People who have ADHD have to eat a clean diet with less sugar, process seed oil fats, and toxins. Many people who start well in a medication and later it stops working is because you dirty your genes with toxins and bad diet and not knowing the science of herbs and plants and proper macro nutrition. I like medical grade Methylene blue a few drops in the morning with lemon. DMG, TMG, L-theanine, Gotu kola, Fulvic Acid or Primavie Shilajit, Lion's Mane extract, vitamin B3 Niacinamide 3x a day or NMN, Quality pre-pro-postbiotic

If starvation decreases urine ph, how quickly does this happen ie would fasting do it?
I haven't looked at the actual studies, but the claims I have read say that the most significant changes in urinary pH happen within 12-24 hours of starting a fast.

As of now, your videos are the best resource for the latest approach to treating ADHD for us late diagnosed women. I am no longer shocked how lacking studies are and how much I rely on anecdotal info from patients as well as professionals’ personal findings. Thank you for condensing a confusing and frustrating subject into succinct and well organized facts/findings.
I'm glad that the videos are of help. As some have pointed out when I rely on the scientific approach, anecdotes can provide some useful information, but they should always be identified as such, and their significance for the treatment of others is limited until more extensive study can be done.
