Love your videos. They seem well researched. Im a PA-R. I hope whatever the RCP is planning to release about our PA colleagues that PAs are safe and supported, whatever that means for them. And I hope doctors follow the law and Good medical practice guidance on workplace behaviour and decisions. May supportive doctors speak up against unacceptable behaviour.
Pyleonephritis may not change - but the treatment will be dependent on the severity, (which abx, fluids, admission, escalations, sepsis protocol) and that is only interpreted by experience and exposure through vast amount of training - especially whilst working in the NHS with limited resources /beds /in-patient support. Reading textbooks is great, but it will not compare to a doctor haven’t seen this 20-30 time as a resident before working independently. That’s what ensures good clinical decisions rather than following guidance.
Hey, do think that after regulation, that UK PAs may be able to work abroad like in America?
No fucking chance. The level of education/ training might be ok for a national health service trying to keep it on the cheap and willing to eat the liability.. (with huge amounts of backlash and criticism) But in private sector/ overseas? Fuck no is any business considering that health economics risk worth it! Might as well just poach the actual doctors/lead nurses.. as they are doing Uk Pa are being introduced in the nhs as they can’t leave. That’s the whole point, they don’t have any value on the international market, so as the NHS haemorrhages highly qualified staff with reputable training.. to overseas/ private there is a safeguard, to keep the baseline / cheap / national service afloat. If you had any comprehension of the role and the international medical market, you would understand this is!
@@Bfg12327 Oh I definitely hope so for UK PAs as it will be rubbing it in the Drs who bully them. Right in their faces for them to move abroad like to America to earn over $120K as it seems that America respects their PAs more than UK Drs do. As an outsider looking in, I'll definitely wish that for UK PAs plus the UK PAs curriculum is based on the American curriculum. Only difference is that the US PAs can prescribe as they were the ones who came into the UK to introduce the profession 20 years ago. So regulation is one winning step forward and the UK PAs can move abroad where they are much more appreciated. They just need regulation which they're already achieving in just less than a month and then prescribing rights for them.
Hello Push, In this climate i find it highly unlikely. I think it would defeat the main argument used in our creation “ shortage of doctors and PAs being one of the solutions”. I am aware PAs in the USA can work here at present whether this will be reciprocal in the future is yet unknown.

NPs and PAs did not go to medical school and thus, their knowledge base is not that of a medical doctor. Although NPs and PAs are highly trained, there should be a medical license on the premises, serving as oversight. However, insurance companies want cheaper labor and doctors do not want anyone working under their licenses, leaving them legally liable for mistakes made..So, it was a win/win for the industry having NPs and PAs in place, but again, NPs and PAs are not medical doctors, increasing the risk of improper diagnoses, injury or fatal outcomes in the patient population.
This is such an unfortunate and toxic dynamic. There are so FEW PAs in the U.K.! There are many health issues among the VAST population, especially among the poor and minorities. PAs can help meet the healthcare needs of the entire country. This dynamic has been initiated by a growing number of MD and patient advocates ("stethoscope creep"🤦🏽♂️). Unfortunately, anecdotal occurrences are being used as "supportive data".😠 We're doing "fairly well" in the U.S. But, I think we will see a "trickle-over" effect in the U.S. I remind doctors that the PA profession was LITERALLY STARTED and perpetuated, by medical doctors!😮 Keep up the great work you are doing! You guys have a tough fight ahead of you but, I think that things will eventually work out for the BEST, Lord willing (insha'Allah)! 🙏🏽😊👋🏽👨🏽⚕️

Hello Jaquline, thanks for watching and much appreciated. I would advise you not to do it. The direction of travel is uncertain and there are no reassurances form the necessary bodies. What you are paying for and what you may get will vary. If other avenues (i.e. GEM) are available to you take them. I can't in good conscience recommend this path to you.
Please think carefully before you do the PA course. Most PAs who graduated last year and this year are without a job (check the internet). Things will improve job wise after the GMC regulates PAs. But the role is likely going to be a lot different to what it is now. As the brother has said - do a GEM course in medicine or become a nurse. It's sad for the PA profession but there are so many flaws with the role.
My son is a PA - and has just lost his job from this. Insurers are refusing to cover those practices who still want to keep their PA's. 3 years post-grad in nursing, followed by 2 years masters as a PA. Worked in the ventilated ICU during the height of Covid.. And now, after dedicating the last 5 years of his life for the healthcare qualifications for this, he's been thrown on the scrapheap. The treatment PA's have had has been reprehensible.
That's really unfortunate but that's our current reality. The matter of indemnity will prove herculean and could underpin some more redundancies.
First of all, best wishes to your son and I wish him every success for the future. It is NOT his fault. It is the government and the NHS at fault for selling PAs false hopes. PAs have a very important role in healthcare - as ASSISTANTS to Doctors. Very clearly, they should never have been working in Primary Care diagnosing undifferentiated patients who have not been triaged by the GP first (which is not practical anyway as it creates more work). Remember - a PA can only see what their mind is trained to see. A GP has top A level grades and goes through intense competition to get into medical school and then undergoes 10 years training before being a GP. A PA can have poor A level grades, take a degree in Botany, get into a PA school and graduate just 2 years later after exams no one fails (the exams were shown online and a layman could get a lot of the questions right. Also Apologies if you son does not fit this description - a lot of PAs have poor A levels and non-related first degrees). Does anyone in their right mind think a PA can do what Doctors do? Of course not. They should not be diagnosing, prescribing or ordering results. BUT - in secondary care, they can take histories, do bloods and cannulas, clerk patients, do post operative care, look after patients on the ward and do multiple other very useful things to HELP doctors. In Primary Care, they can do what the practice nurse does. This is the reality. Best of luck to PAs - they need their scope clarifying and reduced from what they currently do. That would sort the issue.
Clearly your son is an exception to what I described and full respect to him. It's just that the PA role has been mis-sold and needs the scope sorting.

Genuine question-you seem like an intelligent guy, why did you think it would be possible to be seeing on differentiated patients and diagnosing independently after two years? You knew where the bar was set, a foundation doctor can’t do that after five years of focus training on that career path why would you be able to do it in two years? You also knew the role was dependent, and any job will adapt and change. It seems that you were genuinely sold a lie, but I’m not sure how you fell for it- your university and leadership need to be questioned.
Unfortunately, it doesn’t matter why you got into this role, or what you were promised, it’s only matters that every major college and stakeholder has decided that the training for PAs isn’t enough to do what you want. I’m sorry you were sold a lie

@@RAO.NUTRITION two major problems with the PA course - it's only recognised in the UK where it is currently being heavily restricted (correctly) and as it is UNACCREDITED it is worthless for anything other than being a PA in the UK - unless you're ok with misrepresentation on your CV.

Your videos are VERY engaging! I'm a PA in Washington, DC! Are you in U.K.? I've really enjoyed being a PA, for more than 20 years. I only make half of what MD/DOs make. I think that is MORE than fair, though. They have had FAR more training and, owe MUCH more in student loans. I get along with MDs because I "slow my role" and an am respectful to their greater training and position.👋🏽👨🏽⚕️
Hello Mr Omar, thanks for watching. You are much more experienced than myself by multiple decades. I am from the UK and I agree that Doctors indeed are more extensively trained than PAs. We just have to do the best we can and acknowledge our own limitations.

Why don't they just call themselves physicians associates? At my practice they mislead patients by introducing themselves as part of the medical team. Get a tad pissed off when you ask what their job title is. Feels like like to think they are doctors.
The word Physician in the title is completely wrong and misleads the public. A lot of patients think they are Doctors. This is going to cause a storm when issues start coming to light and people start getting sued. Rename PAs to Nurse Practitioners. A large number of PAs revel in their patients thinking they are doctors and deliberately play along. This does not apply to ALL PAs - but it applies to a LARGE number.

I think the major issue that arises in the minds of med students is why bother going into medical school instead of a PA course if the outcome is the same. Especially when the training after medical school is significantly longer. Less about retaining knowledge. That same idea about knowledge can be said between doctors and nurses.
