Forrest MacNeil's father shoots him in the left shoulder, and then while Forrest is screaming in agony, his father shoots him again in the same spot. For his third and final attempt, before collapsing due to blood loss, Forrest moves to position the apple on his head in the same spot that his father has fired the first two arrows. Which is very close to the open doorway. So the final arrow flies into the next room, hitting Lucille in the chest as she walks past unsuspecting. They are both taken to the hospital, where Forrest realizes that "doing a William Tell" doesn't require the apple being hit. Which he should have thought of after the first arrow, but unfortunately for Forrest, he is a stupid lunatic.
