Patch had a Microship in his Eye at the time that gave him the essence of Stefano, so he was acting as Stefano, Hope Also had one in her head that Made her act as Princess Gina Von Amberg and They wanted their Long lost partners Back, Gina Wanted John while Stefano was determined to finally have Marlena, so He Brainwashed her!

I don't really understand why Marlena is having an affair with Steve....
Steve was brainwashed into thinking he was Stefano at the time. Hope was Princess Gina and they were trying to break up John and Marlena at this time.
