The song represents the post-war trauma invading everyday life, how normal people (especially those in vietnam) had ruined lives from what they had seen and done and experienced. It matches so well with the theme in this game of all the broken mental patients. Hell, mr. grimm's whole story is based in Vietnam.
I like how the beginning of the song is played everytime you boot the game. I first heard this song, not in Twisted Metal, but I think in CoD Black Ops (if not that, then I heard it on a classic rock station) and I think that this was a perfect song to choose for this game.
This the most dark, disturbing videogame storyline ive ever seen, when was a kid i really like this game but never play it alone coz the background music are to scare for me and the drivers faces were more disturbing but very cool too, even now i stil kinda scared to see or hear the background music and the cutscenes
I wouldn't say it was the last, but it's certainly hard to compete with. Especially when you compare it to Head-on and the PS3 one which was basically "Twisted Metal Black 2: Grim Dark".
This game being my first twisted metal instantly got me hooked to the series to the point where I plan on buying and beating twisted metal 1 through 2012.

I always took this song as someone inner monologing about slipping deeper and deeper into psychosis, which still kind of fits theme of this game lol.
