I watch Carl and Geek and Defender be faced with an apparent choice on the level of “get the powerup or not”, and yet even after so many levels that are slightly more complex than that and require for instance “get the powerup and then get hit”, they still get confused and ask “then what do I do?” after trying both.
I think it may have been due to climbing the vine. The saw knocked him back, but then he immediately started climbing the vine again, which canceled the knockback. Still, I was just as surprised as he was.
