    My dentist said it wouldn’t be possible to move me molars forward because I’m too old and the jaw bone is set I think he called it? I’m missing my second premolar, the space is bigger than the size of a premolar. I’m 21 years old and considering to try close it with braces instead of getting an implant.

      Bone never gets to a point where it wont move. We have moved the bone even in 80 year old patients. I am 93 years old and I can get mine moved if I needed to. I am not trying to hurt anyone's feelings, but it is not correct. Bone can be moved. Your teeth can be moved into that spot. At 21, you are a very young person.

      @@DrBillWyatt thank you for that information! I have been reading into implants and while doing so stumbled across the fact that the teeth can in fact be moved foreword. Seeing results such as yours made me consider that as a better option than implants. Honestly I have had the feeling that my orthodontist simply doesn’t want to do it instead of not being able to, they have taken two years of treatment with braces already and not even tackled the molar issue at all yet so I’m looking into getting a second opinion and will bring up the possibility of moving the molars foreword for sure now that I know it’s very much possible to do so.

      @@oceanstaiga5928 My opinion is & I dont want to make enemies of orthodontists or people who treat with implants, they are all wonderful. But it is also wonderful to be able to take care of natural teeth & move them into positions that even dentist dont realize that tooth can be moved there. To me, this is far better treatment if one has time, especially if you are having your teeth straighten. This can be part of original diagnosis and should be. I can do a case like this and take no longer to finish it by using natural teeth. We have many videos showing before and after. Age has very little to do with it.

    My 13 year old still has baby teeth but is missing four of her permanent ones. She has a baby canine that the orthodontist says needs to be extracted because is barely holding on and it is taking the place of her adult canine which already came out. The. She is missing 2 permanent molars in the same upper quadrant. There is only one baby molar in and the orthodontist says that could stay in. The orthodontist plans to use Invisalign to close the gaps but warned that the baby tooth could fail when trying to move it. Could I please get your thoughts about this? I don’t want her to get an implant or a bridge and prefer to have gaps closed.
    Thank you!

      This is an extremely complicated case. I have seen deciduous teeth that had nothing to cause the roots to resorb stay in the mouth for several years. It's hard to know and predict exactly what will happen. You can see a case that I did where the young man had only 19 of his permanent teeth. We put the teeth that he had in the correct place as well as we could and I worked with a restorative man who bridged the whole mouth. You can see his case here: th-cam.com/video/pSTh4lUO2rk/w-d-xo.html

    Hi Doctor,
    Great vid. I got braces at 21. Due to severe overcrowding, I had two upper pre-molars removed and two bottom molars as these had filings in already. Overbite was also very deep.
    It’ll be nearly two years since I got my braces and only recently have I got my power chains to close the gaps. How long do you think it’ll take for molar gaps to close?

    • @DrBillWyatt
      @DrBillWyatt  5 ปีที่แล้ว +7

      It is best to start closing gaps immediately upon extracting the tooth. I like to have braces on somebody prior to their extractions and we start closing the space immediately. If a space is allowed to stay open for a considerable period of time, the bone closes in over the top and you have to push the tooth through a knife edge of cortical bone which is much more difficult than going through cancellous bone. But it can be done, but takes much longer.

    • @ygigani609
      @ygigani609 5 ปีที่แล้ว

      Thanks for the response Doc. With my overbite being so deep, upper braces were attached first to push them back.
      Next the bottom braces were put on and numerous times brackets have become loose as the upper teeth keep hitting the lower brackets. Has become very frustrating but slowly but surely.

      @@ygigani609 Dont try moving the jaw when you have a deep bite case. Level the bite first. Go look at the cases with intruding arches and how we leveled the bite and then you can move the jaws around. If you have a deep bite and you try to move the upper arch back, you are going to push the mandible back also. And you could come back down with a case where you are pushing the condyle into the retrodiscal tissue. Look this up in the videos.
      OPEN THE BITE before you do all this.

      @@DrBillWyatt Just curious Dr. Wyatt if this would be a case for fill banding. I have seen ful bandinding recommended for adults with large gap closure needed. Given that brackes were breaking frequently I would think full wrap around bands on all teeth would be better. Iwoild think full banding would be a help with the poor patients very deep overbite. Certainly sounds like a case could be made for full banding for this patient. Headgear also?!

    Thanks for the insights. Instead of the TAD and closing space would transplantation of the 3rd molars to the space of the 6 year old molars be an option?

    • @DrBillWyatt
      @DrBillWyatt  4 ปีที่แล้ว +1

      It is kind of difficult for me to tell you what to do. If you do not want a tooth or a group of teeth to move when you pull against it, you can put a TAD in a cortical bone toward the distal of the tooth and bond the TAD to the tooth with some good bonding agent. You can take this tooth or this group of teeth, pull teeth back from the front without the front teeth moving back. The back teeth will move forward all the way. We show that in one particular video. It is true that the teeth where you have it bonded to a screw, the screw is not going to move unless it moves a whole segment of bones. The use of TADS or little screws has made a lot of difference. It is ridiculous how much they charge for the little screws. They shouldn't be more that a few dollars. Some of them here runs 60-70$ a screw. They do a marvelous job when you want to move posterior teeth to the front without bringing the upper front teeth back.

    Is it better to just close space with missing tooth than implant. I heard that implant bone graft will also experience resorption within 5 years similar to denture. So dental implant has only lifespan of 5 years

    • @DrBillWyatt

      I don't think this is correct. I don't think we actually know exactly how long an implant will last.

    Hi, Dr. Wyatt. I went to an orthodontist consultation yesterday about my options of closing space due to missing tooth and a very bad tooth that needs a crown. I wanted to extract similar teeth and use my wisdom teeth to replace all four, but the doctor says he's worried about my bone loss and extracted tooth areas are "miles" apart. He suggested to crown the bad tooth and install braces to widen my space just to have an implant, which sounds like more $$$. How can I find an orthodontist that will perform the closing of spaces plus removing existing teeth to use wisdom in place of it?

    • @kawayama12
      @kawayama12 3 ปีที่แล้ว

      Get a bone in the extracted areas. It'll prevent bone loss. And you should find another dentist.

      Most people doing orthodontics are not going to offer you a treatment that will cost a few hundred dollars and is far superior to implants, bridges or whatever the heck you want to put it. They need to put their own teeth. Everyone has been writing to me the same problem you are having because no orthodontist want to do it. This is the right and the best solution to this problem. Use your own teeth. Yet the schools (many of them) wont even teach it. What is wrong with people???? Use your own teeth to fill space.

      Yes you can move teeth all over the place if you learn how to bend wire and put force on the teeth. Teeth will move as long as you live. I have moved peoples teeth into 90's but never started anybody that was 90.

    Hello doctor, I am a 17 year old girl and i had my 1st premolar shaved down and put a cap on it due to cavity, i hate the feeling of having a fake tooth in my mouth and i can't really afford to go to a doctor to ask about this, i was wondering whether i can have what's left of my shaved tooth extracted and have the space closed with braces?

      If you have a wisdom teeth on that side of the mouth, then its possible.
      If there is a proper crown on your shaved tooth, then there is nothing wrong with it. Crowns can last a good while so you can keep it.

    • @DrBillWyatt
      @DrBillWyatt  5 ปีที่แล้ว +1

      The problem might be that 12 molars are in but the wisdom teeth are not. If you needed to put the wisdom tooth in, into one of the spaces, then you will have to wait till the tooth got there.
      Just example- If you lost a 6 year molar, you can move the 2nd molar fwd into that space and the developing wisdom tooth would tend to drift forward to some extent.
      If this was a lower tooth, the upper second molar would tend to drop into the space before the wisdom tooth would erupt, so one would have to make retainer to hold the upper second molar from dropping down, while you wait on the wisdom tooth to erupt.
